Özet

İnme sonrasında görülen bilişsel bir bozukluk olan ihmal, hastanın fonksiyonel iyileşmesini yavaşlatmakta, rehabilitasyon sonuçlarını ve hastanın günlük yaşam aktivitelerinde bağımsızlığını olumsuz etkilemektedir. Çalışmamızda tek taraflı uzaysal ihmali olan subakut-kronik dönemdeki iskemik inme hastalarında görsel tarama eğitimi ve ayna tedavisiyle kombine edilmiş düşük frekanslı repetitif transkraniyal manyetik stimülasyonun (rTMS); ihmal semptomları, motor iyileşme, günlük yaşam aktiviteleri ve yaşam kalitesi üzerine olan etkilerini taklit uyarımla karşılaştırarak değerlendirmek amaçlanmıştır. Çalışmaya dahil edilen ihmali olan 22 hasta randomizasyon şemasına göre, aktif rTMS (n=11) ve taklit rTMS (n=11) olarak 2 gruba ayrıldı. Aktif gruptaki hastalara 2 hafta boyunca toplam 10 seans ve her seans 20 dakika sürecek şekilde elektroensefalogram 10/20 sistemine göre P3 noktasına karşılık gelen sağlam posterior parietal korteks hedef alınarak toplam 1200 atımdan oluşan 1 Hz'lik düşük frekanslı rTMS uygulandı. Diğer gruptaki hastalara aktif koil ile aynı görünüme sahip taklit koille, benzer protokol uygulanarak taklit rTMS uygulandı. Ayrıca her iki gruptaki hastalara rTMS uygulamalarından sonra görsel tarama eğitimi ve ayna tedavisi uygulandı. Hastalar tedavi öncesinde, tedavi bitiminde, tedavi bitiminden 2 hafta ve 4 hafta sonra olmak üzere toplam 4 defa değerlendirildi. Çalışmada sonuç ölçütü olarak; Yıldız Silme Testi (YST), Çizgi Bölme Testi (ÇBT), Catherine Bergego Skalası (CBS), Brunnstrom Hemipleji İyileşme Evrelemesi, Mini Mental Durum Testi (MMT) ve İnme Etki Ölçeği (İEÖ) kullanıldı. Aktif rTMS grubunda YST ve ÇBT sonuçlarında tedaviden sonraki tüm ölçümlerde taklit gruba göre istatistiksel olarak anlamlı iyileşme saptanmış olup iyileşmenin en az 4 hafta devam ettiği görüldü (p<0.05). Aktif rTMS grubunda CBS skorlarında taklit gruba göre anlamlı fark saptanmazken; grup içi değerlendirmelerde aktif grupta tedavi öncesine göre tedavi bitiminden sonraki tüm ölçümlerde, istatistiksel olarak anlamlı düzeyde iyileşme izlendi (p<0.05). Taklit grubunda ise CBS skorlarında grup içi değerlendirmelerde tedavi bitiminden sonraki hiçbir ölçümde iyileşme saptanmadı (p>0.05). Aktif grupta Brunnstrom-Alt değerlerinde grup içi iyileşme saptanırken taklit grubunda Brunnstrom-Üst ve El değerlerinde grup içi iyileşme saptandı ancak gruplar arasındaki ölçümlerde Brunnstrom değerlerinin hiçbirinde anlamlı farklılık elde edilmedi (p>0.05). İEÖ-Hafıza ve MMT değerlerinde tedavi sonrası tüm değerlendirmelerde gruplar arası fark aktif grup lehine anlamlı bulunurken grup içi değerlendirmelerde her iki grupta da anlamlı farklılık saptanmadı (p>0.05). Aktif rTMS grubunda İEÖ-İletişim sonuçlarında tedavi sonrası değerlendirmelerde taklit gruba göre istatistiksel olarak anlamlı farklılık elde edildi (p<0.05). İEÖ diğer alt testlerinde gruplar arası anlamlı farklılık izlenmedi (p>0.05). Çalışmamız sonucunda subakut-kronik dönem iskemik inmeli ve tek taraflı uzaysal ihmali olan hastalarda görsel tarama eğitimi ve ayna tedavisi ile birlikte uygulanan düşük frekanslı rTMS uygulamasının ihmal semptomlarını azalttığı ayrıca günlük yaşam aktivitelerinde iyileşme sağladığı gözlemlendi. rTMS'nin ihmal tedavisi için konvansiyonel rehabilitasyona ek olarak uygulanmasının tedavi süresini kısaltması ve rehabilitasyon programına uyum sağlaması açısından avantaj sağlayabileceği; bu nedenle TMS için kontrendikasyonu olmayan hastalarda tedavi programında yer alabileceği düşünülmektedir.

Neglect, a cognitive disorder seen after stroke, delays the functional recovery of the patient and negatively impacts rehabilitation outcomes and the independence of the patient in activities of daily living. In our study, we aimed to evaluate the effects of low-frequency repetitive transcranial magnetic stimulation (rTMS) combined with visual scanning training and mirror therapy on neglect symptoms, motor recovery, activities of daily living, and quality of life in subacute-chronic ischemic stroke patients with unilateral spatial neglect compared to sham stimulation. A total of 22 patients with neglect were included in the study and randomly allocated into two groups: active rTMS (n = 11) and sham rTMS (n = 11). Patients in the active group received 1 Hz low-frequency rTMS, consisting of 1200 pulses per session for a total of 10 sessions over 2 weeks, with each session lasting 20 minutes and targeting the intact posterior parietal cortex corresponding to the P3 point according to the electroencephalogram 10/20 system. Patients in the sham group received sham rTMS with a sham coil that appeared identical to the active coil, following a similar protocol. Additionally, both groups participated in visual scanning training and mirror therapy following each rTMS session. Patients were assessed a total of 4 times: before the treatment, at the end of the treatment, 2 weeks after the end of the treatment, and 4 weeks after the end of the treatment. As outcome measures, Star Cancellation Test (SCT), Line Bisection Test (LBT), Catherine Bergego Scale, Brunnstrom Hemiplegia Recovery Staging, Mini Mental State Examination Test (MMSE), and Stroke Impact Scale (SIS) were used. In the active rTMS group, there was a statistically significant improvement in Star Cancellation Test (SCT) and Line Bisection Test (LBT) results at all posttreatment assessments compared to the sham group, and this improvement continued for at least 4 weeks (p<0.05). Although there was no significant difference in CBS scores between the active rTMS and sham groups, intragroup evaluations showed a statistically significant improvement in the active group in all measurements after the end of treatment compared to before the treatment (p<0.05). In the sham group, no improvement was observed in CBS scores at any post-treatment measurement (p>0.05). The active group showed improvement in Brunnstrom-Lower scores, while the sham group demonstrated improvement in Brunnstrom-Upper and Hand scores; however, no significant difference was found between the groups in any of the Brunnstrom scores (p>0.05). In SIS-Memory and MMSE scores, significant differences favoring the active group were observed across all post-treatment assessments, although intragroup evaluations showed no significant differences for either group (p>0.05). The active rTMS group also demonstrated a statistically significant improvement in SIS-Communication scores in post-treatment assessments compared to the sham group (p<0.05). No significant differences were found between the groups in the other SIS subtests (p>0.05). Our study showed that low-frequency rTMS combined with visual scanning training and mirror therapy reduced neglect symptoms and improved activities of daily living in patients with unilateral spatial neglect following ischemic stroke in the subacute-chronic phase. Adding rTMS to conventional rehabilitation for neglect treatment may provide advantages, such as reducing treatment duration and improving adherence to the rehabilitation program, and it is thought that it can be included in the treatment program for patients who have no contraindications for TMS.
