Özet
Çölyak hastalığı; genetik yatkınlığı olan bireylerde gliadin ve ilgili prolaminlere karşı anormal immün yanıtın oluştuğu, ince bağırsak hasarı ile sonuçlanan glutene duyarlı enteropatidir. Çölyak hastalığının en etkili tedavisinin, yaşam süresince sıkı glutensiz diyet olduğu bildirilmektedir. Çölyak hastalığı tanısı alan hastalara HLA haplotiplerinin benzer olması nedeniyle otoimmün tiroiditin eşlik ettiği bilinmektedir. Bu çalışmanın amacı; ÇH tanılı yetişkin hastaların otoimmün tiroidit hastalığı ile ilişkisini ortaya koymak, ÇH tanısı alan hastaların otoimmün tiroidit açısından taranmasının ve takibinin önemini vurgulamak ve erken tanı sayesinde hastalığın prognozunun iyileşmesi ile otoimmün hastalık prevelansını düşürmek ayrıca ÇH ve otoimmün tiroiditin beraber seyrettiği hastalarda glutensiz diyetin hastalığın prognozunun iyileşmesi üzerindeki etkisini çeşitli parametrelerle birlikte değerlendirmektir. Çalışma 2014-2024 tarihleri arasında Gaziantep Üniversitesi Şahinbey Araştırma ve Uygulama Hastanesi Gastroenteroloji bölümünde Çölyak Hastalığı tanısı alan ve ve takip edilen 18 yaş ve üzeri olan yetişkin hastaların, hastane bilgi yönetim sistemi üzerinden retrospektif olarak değerlendirilmesi sonucu gerçekleştirilmiştir. Çalışmanın sonucunda çölyak hastalarının %33,3'ünde otoimmün hastalıklar tespit edilmiş, çölyak hastalarına %13,5 oran ile otoimmün tiroidit eşlik etmiştir. Çölyak tanılı hastalarda glutensiz diyet öncesi-sonrası doku TG düzeyleri ve aktif şikayet durumları kıyaslandığında istatistiksel açıdan anlamlı bir fark tespit edilmiştir. Otoimmün tiroditli çölyak hastalarının glutensiz diyet öncesi ve sonrası TSH ve T4 ortalama düzeyleri karşılaştırılmış, istatistiksel olarak anlamlı bir fark bulunmuştur; T3, anti-TPO ve anti-TG ortalama düzeyleri karşılaştırılmış, istatistiksel olarak anlamlı bir fark bulunamamıştır. Çalışmamızda ÇH tanılı yetişkin hastaların otoimmün tiroidit hastalığı ile ilişkisinin olduğu ayrıca ÇH ve otoimmün tiroiditin beraber seyretiği hastalarda glutensiz diyetin hastalığın prognozunun iyileşmesi üzerinde olumlu etkisinin olduğu bazı parametrelerle birlikte sunulmuştur.

Celiac disease; It is a gluten-sensitive enteropathy in which an abnormal immune response against gliadin and related prolamins occurs in genetically predisposed individuals, resulting in small intestine damage. It is reported that the most effective treatment method for celiac disease is a strict gluten-free diet for life. It is known that patients diagnosed with celiac disease are accompanied by autoimmune thyroiditis due to similar HLA haplotypes. The aim of this study is to reveal the relationship between adult patients diagnosed with CD and autoimmune thyroiditis, to emphasize the importance of screening and following up patients diagnosed with CD for autoimmune thyroiditis, to reduce the prevalence of autoimmune diseases by improving the prognosis of the disease thanks to early diagnosis, and to evaluate the effect of gluten-free diet on improving the prognosis of the disease in patients with CD and autoimmune thyroiditis together with various parameters.The study was conducted as a result of a retrospective evaluation of adult patients aged 18 and over, who were diagnosed with Celiac Disease and followed up in the Gastroenterology Department of Gaziantep University Şahinbey Research and Practice Hospital between 2014 and 2024, through the hospital information management system. As a result of the study, autoimmune diseases were detected in 33.3% of celiac patients, and 13.5% of celiac patients were accompanied by autoimmune thyroiditis. A statistically significant difference was detected when tissue TG levels and active complaints were compared before and after the gluten-free diet in patients with celiac disease. The mean TSH and T4 levels of celiac patients with autoimmune thyroiditis before and after the gluten-free diet were compared, and a statistically significant difference was found; Mean levels of T3, anti-TPO and anti-TG were compared, and no statistically significant difference was found. In our study, it was presented with some parameters that there is a relationship between CD and autoimmune thyroiditis in adult patients and that gluten-free diet has a positive effect on improving the prognosis of the disease in patients with CD and autoimmune thyroiditis together.
