Özet

Amaç: Hızlandırılmış İyileşme Protokolleri (ERAS) cerrahi sürecin tüm aşamalarında yapılan müdaheleleri optimize ederek, hasta sonuçlarını iyileştirmeyi hedefleyen multidisipliner bir yaklaşımdır. Düşük akım uygulmasıda bilinen avantajları nedeniyle ERAS protokolleri içerisinde yer alabilir. Düşük akım anestezide, kullanılan anestezik miktarının düşmesine bağlı olarak maliyetlerin azalması, atık miktarının azalması ve mukosiliyer aktivitenin bozulmaması gibi faydaları olduğu bilinmektedir. Çalışmamızda lomber disk cerrahisi geçiren hastalarda uygulanan düşük akım anestezi ile yüksek akım anestezinin derlenme ve kognitif fonksiyonlar üzerine olan etkileri karşılaştırmayı amaçladık.Böylece hızlı derlenme sağlanacak günlük aktivitelere daha erken dönülelebilecektir. Gerekçe ve Yöntem: Prospektif gözlemsel tek merkezli çalışmamızda 01.10.2023 – 01.10.2024 tarihleri arasında Sancaktepe Şehit Prof. Dr. İlhan Varank Eğitim ve Araştırma Hastanesi'nde genel anestezi altında lomber disk cerrahisi yapılan, ASA (American Society of Anesthesiologists) skoru 1-2 olan ve 18-65 yaş aralığındaki gönüllü 75 hastada düşük akım(n=37 grup 1-0,5lt/dk) ve yüksek akım (n=38 grup2- 2lt/dk) anestezi uygulanan gruplar arasında postoperatif Derlenme süreleri Modifiye Aldrete Skoru ile Postoperatif kognitif fonksiyonları TDT (Trieger Dote Testi) ve SMMT (Standartize Mini Mental Durum Testi) testleri ile değerlendirildi.Kullanılan inhaler ajan, remifentanil miktarı, ağrı skorları ve olası yan etkiler karşılaştırıldı. Bulgular: Çalışmamızda hastaların yaş, cinsiyet, ASA skorları, anestezi ve cerrahi süresi açısından iki grup arasında anlamlı fark bulunmamıştır. Postoperatif kognitif fonksiyonlar açısından değerlendirildiğinde 15 dk. 60 dk. ve 120. dk'larda TDT'lerde kaçırılan noktalar grup 2'de anlamlı olarak daha fazlaydı.(p<0,001) SMMT değerleri incelendiğinde preoperatif dönemde her iki grup arasında anlamlı bir fark olmadığı gibi postoperatif 120.dk'da grup 2 'de anlamlı olarak daha düşük olduğu görülmüştür.(p<0,001) Modifiye aldrete skoru incelendiğinde 9'a ulaşma zamanı grup 1'de anlamlı olarak daha kısaydı.(p<0,001) Kullanılan gaz ve remifentanil miktarı grup 2'de anlamlı olarak daha fazla bulundu.(p<0,001').Bulantı, kusma, ek analjezi ihtiyacı ve VAS değerleri açısından gruplar arasında anlamlı fark olmadığı gözlendi. XI Sonuç: ERAS protokolleri rehberliğinde, modern anestezi makineleri ve deneyimli anestezi doktorlarıyla uygulanan düşük akım anestezi yöntemi; ekonomik olması ile çevre ve çalışan güvenliğini sağlamasının yanında, hastaların anesteziden derlenme sürelerinin hızlandığı, kognitif fonksiyonların daha hızlı toparladığı, opioid kullanımını azaltarak postoperatif bulantı ve kusmayı azaltarak hasta konforunu arttırdığı, hastane kalış süresini kısalttığı ve maliyeti düşürdüğü bulunmuştur. Bu da gösteriyor ki Düşük akım anestezisi lomber disk cerrahisi olgularında ERAS protokollerinde yer alabilecek avantjlar sağlayabilir bu konuda yapılmış ilk çalışma olup daha geniş serili çalışmaların yapılmasına ihtiyaç vardır.

Aim:.Enhanced Recovery After Surgery (ERAS) protocols are a multidisciplinary approach aimed at improving patient outcomes by optimizing interventions at all stages of the surgical process. Due to its well-known advantages, low-flow application can be included within ERAS protocols. In low-flow anesthesia, benefits such as reduced costs due to the decreased amount of anesthetics used, reduced waste, and the preservation of mucociliary activity are known. Our study aims to compare the effects of low-flow anesthesia and high-flow anesthesia on recovery and cognitive functions in patients undergoing lumbar disc surgery. This way, faster recovery will be achieved, and daily activities can be resumed earlier. Material (patients)and methods: In our prospective observational single-center study, conducted between October 1, 2023, and October 1, 2024, at Sancaktepe Şehit Prof. Dr. İlhan Varank Training and Research Hospital, 75 volunteer patients aged 18-65 with an ASA (American Society of Anesthesiologists) score of 1-2 who underwent lumbar disc surgery under general anesthesia were included. Patients were divided into two groups: low-flow anesthesia (n=37, Group 1 - 0.5 L/min) and high-flow anesthesia (n=38, Group 2 - 2 L/min). Postoperative recovery times were evaluated using the Modified Aldrete Score, and postoperative cognitive functions were assessed using the Trieger Dot Test (TDT) and the Standardized Mini-Mental State Examination (SMMSE). The amount of inhaled agent used, remifentanil consumption, pain scores, and potential side effects were compared between the groups.. Results: In our study, no significant differences were found between the two groups in terms of age, gender, ASA scores, anesthesia, and surgery duration. When postoperative cognitive functions were evaluated, the number of missed points in the TDT at 15, 60, and 120 minutes was significantly higher in group 2 (p<0.001). Examining the SMMSE values, there was no significant difference between the two groups in the preoperative period; however, at 120 minutes postoperatively, group 2 showed significantly lower scores (p<0.001). When the Modified Aldrete Score was evaluated, the time to reach a score of 9 was significantly shorter in group 1 (p<0.001). The amounts of gas and remifentanil used were found to be significantly higher in group 2 (p<0.001). There XIII were no significant differences between the groups in terms of nausea, vomiting, need for additional analgesia, and VAS values. Conclusion: Guided by ERAS protocols and utilizing modern anesthesia machines and experienced anesthesiologists, the low-flow anesthesia method has been found to not only be economical and ensure environmental and worker safety, but also to accelerate recovery times from anesthesia, improve cognitive function recovery, reduce postoperative nausea and vomiting by decreasing opioid use, enhance patient comfort, shorten hospital stays, and reduce costs. This demonstrates that low-flow anesthesia can provide advantages in lumbar disc surgery cases that could be included in ERAS protocols. As this is the first study on this topic, further studies with larger series are needed.
