Herpetik keratit hastalarının klinik ve konfokal bulguları ile gözyaşındaki enflamatuar sitokin ve büyüme faktörlerinin değerlendirilmesi

Özet
AMAÇ: Herpetik keratit hastalarının klinik bulguları ile gözyaşındaki enflamatuar sitokin düzeylerinin ve kornea üzerine etkisinin in-vivo konfokal mikroskopi ile incelenmesi. GEREÇ-YÖNTEM: Çalışmamızda herpetik keratit tanısı almış 32 hastanın aktif atak geçirmekte olan 32 gözü birinci grubu oluşturdu. Aynı hastaların atak geçirmeyen diğer gözleri ikinci grubu oluşturdu. Sağlıklı gönüllülerden oluşan 18 kişinin 18 gözü ise kontrol grubunu oluşturdu. Her üç gruptan da gözyaşı örnekleri toplandı ve gözyaşında enflamatuar biyo-belirteçlerden olan NGF, VEGF, IL-2, IL-6, IFN-G, Sunstance P gibi sitokinler ELISA yardımıyla ölçüldü. Herpetik keratit hastalarının her iki gözüne de in-vivo konfokal mikroskopi ölçümü yapılarak epitel hücresi, ön stroma keratosit, arka stroma keratosit, endotel hücre sayısı, dentritik hücre sayısı ve Image-J programı kullanılarak korneal sinir pleksus ölçümü yapıldı. Ayrıca her gruba esteziyometre testi yapılarak korneal duyarlılıklar not edildi. Elde edilen veriler istatistiksel olarak hesaplandı ve p<0.05 istatistiksel olarak anlamlı kabul edildi. BULGULAR: Hasta ve kontrol gruplarının yaş ve cinsiyetleri benzer olarak saptandı (p>0,05) NGF değeri kontrol grubunda hasta ve sağlam gözlere göre istatistiksel olarak anlamlı yüksek bulundu(p=0,029). Diğer ELISA sonuçları için gruplar arasında anlamlı fark bulunmadı(p>0,05). Esteziyometre ölçümleri ile NGF değerleri arasında anlamlı bir ilişki saptanmadı (p>0,05). Hasta göz grubunda epitel hücre sayısı ve sinir pleksusu sayısı sağlam göz grubuna göre istatistiksel olarak anlamlı düşük saptandı (p değerleri sırasıyla; p=0,017, p=0,005). Hasta gözlerde endotel sayısı da düşük, ön ve arka stromal keratosit sayısı yüksek saptandı, ama fark istatistiksel olarak anlamlı bulunmadı (Hepsi, p>0.05). Korneal damarlanma olan ve olmayan hastaların sitokin düzeyleri arasında farklılık bulunmadı (p>0,05). Hasta gözlerin %53,6'sında çok sayıda dentritik hücre varken, bu oran diğer gözlerde sadece %8 idi (p<0,001). Hasta gözlerin dentritik hücre sayılarına göre sitokin düzeylerinin karşılaştırıldığında dentritik hücre sayısına göre sitokin düzeyleri arasında istatistiksel olarak anlamlı farklılık tespit edilmedi (p>0,05). Çalışmaya dahil edilen hastaların dentritik hücre sayıları ve esteziyometre değerleri arasında istatistiksel olarak anlamlı korelasyon varlığı tespit edildi (p<0,05). SONUÇ: Çalışmamızda HK hastalarının her iki gözünde de NGF düzeyinin kontrol grubuna göre düşük çıktığı(p=0,029) ve diğer enflamatuar sitokinlerin ise sağlıklı bireylerle benzer düzeylerde olduğu gösterildi. Hasta sayımızın az olması, enflamatuar sitokinlerin seviyesinin kişisel varyasyonlar göstermesi, dilüsyonel faktörler ve hastaların topikal antiviral ve steroid kullanıyor veya kullanmış olması farklılığın çıkmamasının bir nedeni olabilir. Ayrıca alınan gözyaşı örnekleri atak sonrası ilk 3 gün içinde alındığı ve enflamasyonun yaklaşık 10. gününde başlayan T hücrelerine bağımlı evreyi içermediği için enflamatuar sitokinlerde farklılık çıkmamış olabilir. Hasta bireylerin hasta gözlerinin ortalama esteziyometre değeri 3,34 iken sağlıklı gözlerinin ortalama esteziyometresi 5,53 olarak bulunmuştur. Bu durum hasta bireylerin sağlıklı gözlerinin korneal duyarlılığının hasta gözlere göre daha iyi olduğunu ama kontrol grubuna göre daha kötü olduğunu göstermiştir. Herpetik keratit genelde tek taraflı olmasına rağmen bilateral etkileri olan ve sağlıklı gözü de etkileyebilen bir hastalıktır. IVKM ölçümlerinde enfeksiyona sekonder değişiklikler gösterilmiştir. Hasta gözlerde epitel hücre sayısı(p=0,017) ve sinir pleksusu (p=0,005) istatiksel olarak anlamlı derecede azalmış bulunurken endotel hücre sayısı daha düşük olmakla beraber istatistiksel olarak anlamlı fark bulunmadı. Ayrıca hasta gözlerde istatistiksel olarak anlamlı olmasa da ön keratosit ve arka keratosit sayısında artış bulundu. Bu durum hastalık sırasındaki keratosit aktivasyonuna bağlandı.

PURPOSE: Investigation of Clinical Findings in Herpetic Keratitis Patients, Levels of Inflammatory Cytokines in Tears, and Their Effects on the Cornea Using In-vivo Confocal Microscopy. MATERIALS AND METHODS: In our study, the first group consisted of 32 eyes from 32 patients diagnosed with herpetic keratitis who were experiencing an active attack. The second group was comprised of the unaffected eyes of the same patients. The control group consisted of 18 eyes from 18 healthy volunteers. Tear samples were collected from all three groups, and inflammatory biomarkers in the tears, such as NGF, VEGF, IL-2, IL-6, IFN-G, and Substance P, were measured using ELISA. In-vivo confocal microscopy was performed on both eyes of the herpetic keratitis patients to measure epithelial cells, anterior stromal keratocytes, posterior stromal keratocytes, endothelial cell count, dendritic cell count, and corneal nerve plexus using the Image-J program. Additionally, all groups underwent esthesiometry tests to record corneal sensitivities. The obtained data were statistically analyzed, p<0.05 was considered statistically significant. FINDINGS: The ages and genders of the patient and control groups were found to be similar (p>0.05). The NGF value was statistically significantly higher in the control group compared to both the patient and healthy eyes (p=0.029). No significant difference was found for other ELISA results between the groups (p>0.05). A significant relationship between esthesiometer measurements and NGF values was not established (p>0.05). In the patient eye group, the number of epithelial cells and nerve plexus was statistically significantly lower compared to the healthy eye group (p values: p=0.017, p=0.005, respectively). Endothelial cell count was also lower in patient eyes, while the anterior and posterior stromal keratocyte counts were higher, but the differences were not statistically significant (all p>0.05). No differences were found in cytokine levels between patients with and without corneal neovascularization (p>0.05). Dendritic cells were present in 53.6% of patient eyes, while this rate was only 8% in other eyes (p<0.001). When comparing cytokine levels based on dendritic cell counts in patient eyes, no statistically significant difference was found (p>0.05). A statistically significant correlation was established between the dendritic cell counts and esthesiometer values of the patients included in the study (p<0.05). RESULT: In our study, it was shown that NGF levels were lower in both eyes of HC patients compared to the control group (p=0.029) and other inflammatory cytokines were at similar levels to healthy individuals. The small number of our patients, individual variations in the levels of inflammatory cytokines, dilutional factors and the fact that the patients are using or have used topical antivirals and steroids may be the reasons for the lack of a difference. In addition, since the tear samples were taken within the first 3 days after the attack and did not include the T cell-dependent phase that begins approximately on the 10th day of inflammation, there may have been no difference in inflammatory cytokines. The average esthesiometer value of the patient eyes of the patient individuals was 3.34, while the average esthesiometer value of the healthy eyes was found to be 5.53. This showed that the corneal sensitivity of the healthy eyes of the patient individuals was better than the patient eyes but worse than the control group. Although herpetic keratitis is generally unilateral, it is a disease that has bilateral effects and can also affect the healthy eye. IVKM measurements showed changes secondary to infection. While the number of epithelial cells (p=0.017) and nerve plexus (p=0.005) were found to be statistically significantly decreased in diseased eyes, the number of endothelial cells was lower, but no statistically significant difference was found. In addition, an increase in the number of anterior keratocytes and posterior keratocytes was found in diseased eyes, although not statistically significant. This was attributed to keratocyte activation during the disease.
