Özet

Giriş ve Amaç: Özofagus atrezisine yönelik düzeltici operasyonlardan sonra erken dönemde en sık görülen komplikasyonlar anastomoz darlığı ve anastomoz kaçağıdır. Bu çalışmada özofagus atrezisi onarımı ve buna bağlı komplikasyonlardaki deneyimimizi değerlendirerek erken dönemde görülebilecek komplikasyonlar için risk faktörleri ve bu komplikasyonların uzun dönemdeki etkilerini belirlemeyi amaçladık. Materyal ve Metot: İstanbul Tıp Fakültesi Çocuk Cerrahisi Kliniği'nde 1983-2023 yılları arasında özofagus atrezisi ve trakeoözofageal fistül onarımı yapılan hastalar çalışmaya dahil edildi. Bu hastalar cinsiyet, antenatal tanı varlığı, cinsiyet, aile öyküsü, doğum ağırlığı, doğum haftası, anne yaşı, eşlik eden anomaliler, tanıda kullanılan yöntem, özofagus atrezisi tipi, yapılan cerrahi müdahale, arcus aortanın yönü, defekt uzunluğu, kullanılan intraoperatif uzatma yöntemi, mekanik ventilasyon ve oksijen desteği ihtiyacı, anastomoz kaçağı, anastomoz darlığı, rekürren TÖF görülmesi, uzun dönemde semptomatik özofagus darlığı, gastroözofageal reflü, trakeomalazi, diğer komplikasyonlar, uygulanan özofagus dilatasyonu sayısı, ek girişim ihtiyacı ve takip sonucu parametreleri açısından retrospektif olarak değerlendirildi. Hastalar ameliyat sonrası erken dönemde anastomoz darlığı olan ve olmayanlar ve anastomoz kaçağı olanlar ve olmayanlar olmak üzere ikişer gruba ayrıldı. Hastaların demografik, klinik ve cerrahi özelliklerinin ve geç dönemde mortalite ve morbidite sebeplerinin bu komplikasyonlarla ilişkisini anlayabilmek adına bu gruplar kendi aralarında karşılaştırıldı. Sonuçlar: Çalışmaya dahil edilen 116 olgunun 65 tanesi (%56) erkek, 51 tanesi (%44) kızdı. 7 olgu (%6) 1500 gramın altındaydı. 11 olgunun (%9,5) tanısı kesin olarak antenatal ultrasonografiyle konulmuştu. 100 olgu (%86,2) Gross Tip C, 12 olgu (%10,3) Gross Tip A, 3 olgu (%2,6) Gross Tip D, 1 olgu (%0,9) ise Gross Tip E idi. 21 olgu (%18,1) uzun aralıklı özofagus atrezisi niteliğindeydi. 95 olguya (81,9%) primer onarım, 11 olguya (%9,5) geciktirilmiş primer onarım, 6 olguya (%5,2) kolon interpozisyonu, 2 olguya (%1,7) mide 2 çekilmesi uygulandı. 15 olguda (%12,9) anastomoz kaçağı, 32 olguda (%27,5) erken dönemde anastomoz darlığı görüldü. Veri analizi sonucunda doğum ağırlığı (p=0,032) ve defekt uzunluğu (p=0,041) ile anastomoz kaçağı arasında anlamlı ilişki bulundu. İlk beslenme yaşı ile hem anastomoz kaçağı (p=0,000) hem anastomoz darlığı (p=0,009) arasında anlamlı ilişki bulundu. Anastomoz kaçağı ile erken dönemde rekürren trakeoözofageal fistül (p=0,009) ve uzun dönemde ek cerrahi müdahale ihtiyacı (p=0,032) arasında anlamlı ilişki bulundu. Anastomoz darlığı ile geç dönemde rekürren trakeoözofageal fistül (0,004), semptomatik özofagus darlığı (p=0,000), çoklu özofagus dilatasyonu ihtiyacı (p=0,000) ve ek cerrahi girişim ihtiyacı (p=0,000) arasında anlamlı ilişki bulundu.

Background and Aim: Early anastomotic stricture and leakage are two of the most common complications after esophageal atresia repair. The purpose of this study was to review our experience in the repair of esophageal atresia and complications after the initial operation and to assess the risk factors regarding said complications and their long term outcomes. Material and Methods: Patients who underwent esophageal atresia and tracheoesophageal fistula repair between 1983 and 2023 at Istanbul Faculty of Medicine, Department of Pediatric Surgery were included in the study. Parameters including gender, presence of antenatal diagnosis, gender, family history, birth weight, gestational age, maternal age, accompanying anomalies, method used in diagnosis, type of esophageal atresia, type of surgical intervention, direction of arcus aorta, length of defect, intraoperative extension method used, need for mechanical ventilation and oxygen support, anastomotic leakage, early anastomotic stenosis, tracheoesophageal fistula recurrency, symptomatic esophageal stenosis, gastroesophageal reflux, tracheomalacia, other complications, number of esophageal dilatations performed, need for additional surgical intervention and follow-up outcome were evaluated retrospectively. Patients were divided into two groups as those with and without anastomotic stenosis and those with and without anastomotic leakage in the early postoperative period. These groups were compared among themselves in order to understand the relationship between the demographic, clinical and surgical characteristics of the patients, causes of mortality and morbidity in the long term follow up and these complications. Results: Of the 116 cases included in the study, 65 (56%) were male and 51 (44%) were girls. 7 cases (6%) were under 1500 grams at birth. 11 cases (9.5%) were definitively diagnosed by antenatal ultrasonography. 100 cases (86.2%) were Gross Type C, 12 cases (10.3%) were Gross Type A, 3 cases (2.6%) were Gross Type D, 1 case (0.9%) was Gross Type E. 21 cases (18.1%) were long-gap esophageal atresia. 95 cases (81.9%) were primary repair, 11 patients (9.5%) underwent delayed primary repair, 6 cases (5,2%) underwent colon interposition, and 2 patients 4 (1,7%) underwent gastric transposition. Anastomotic leakage was seen in 15 cases (12.9%) and early anastomotic stenosis was seen in 32 cases (27,5%). Length of the gap (p=0,041) and birth weight (p=0.032) had significant correlation with anastomotic leakage while age at the first oral feeding had significant correlation with both anastomotic leakage (p=0,000) and anastomotic stenosis (p=0,009). Anastomotic leakage had significant correlation with need for additional surgical intervention(s) (p=0,032) and early recurrent tracheoesophageal fistula (p=0,009). Early anastomotic stricture had significant correlation with symptomatic esophageal stenosis (p=0,000), need for multiple esophageal dilatation (p=0,000), need for additional surgical intervention(s) (p=0,000) and late recurrent tracheoesophageal fistula (p=0,004)
