Özet

Amaç: Meme kanserinde etkili bir tedavinin sağlanmasında en önemli parametre neoadjuvan kemoterapi (NAKT) sonrası elde edilen patolojik tam yanıttır. Ancak neoadjuvan kemoterapiye yanıtı öngörebilen klinik ve patolojik parametreler hala net olarak ortaya konabilmiş değildir. Çalışmamızda, ucuz ve kolay ulaşılabilen inflamatuar ve nutrisyonel beliteçlerle patolojik tam yanıt (pCR) arasındaki ilişkinin araştırılması amaçlandı. Bu parametrelerin patolojik tam yanıtı öngördürücü etkilerinin değerlendirilmesi hedeflendi. Gereç ve Yöntem: Sancaktepe Şehit Prof. Dr. İlhan Varank Eğitim ve Araştırma Hastanesi Onkoloji Polikliniği'ne Aralık 2022-Aralık 2024 tarihleri arası başvuran 18 yaş üstü, NAKT almış lokal ileri evre kadın meme kanseri hastaları çalışmaya dahil edildi. Çalışmaya dahil edilen hastaların demografik verileri, antropometrik ölçümleri, komorbiditeleri, kan grupları, menopoz durumları, aldığı kemoterapi rejimleri, tümör histopatolojk özellikleri Tıbbi Onkoloji arşivindeki hasta dosyalarından kaydedildi. Hasta grubu patolojik tam yanıt olan ve olmayan olmak üzere iki gruba ayrıldı. Hastaların kemoterapi öncesi ve operasyon öncesi olmak üzere nötrofil/lenfosit oranı (NLR), platelet/lenfosit oranı (PLR), prognostik nutrisyonel indeksi (PNİ), yaygın kanser inflamasyon indeksi (ALİ), controlling nutritional status (CONUT) skorları hesaplandı. Bu parametreler patolojik tam yanıt olan ve olmayan gruplar arasında karşılaştırıldı. Elde edilen verilerle istatistiksel analizler yapıldı. Bulgular: Çalışmaya 73 hasta dahil edildi ve ortalama yaş 52,86±10,85 yıldı. Hastaların 56'sında östrojen reseptörü (ER) pozitifliği, 49'unda progesteron reseptörü (PR) pozitifliği mevcuttu. Ortanca ER değeri %80 (2-95) olarak, ortanca PR değeri %30 (0-70) olarak, ortanca Kiel 67 antijeni (Ki-67) değeri %30 (20-60) olarak saptandı. Çalışmadaki hastaların 28'inde insan epidermal büyüme faktörü (HER2) pozitifliği mevcuttu. 6 hastanın silver in situ hybridization (SISH) testi sonrasında HER2 pozitifliği saptandı. Hastalar basit moleküler alt tiplerine göre sınıflandırıldığında: 34'ü HER2+ (HR+/-), 31'i HER2 – (HR+) ve 8'i üçlü negatifti. Patolojik yanıt durumlarına göre değerlendirildiğinde; 24'ünde (%32,9) pCR (RCB0) olduğu saptandı. Tam yanıt olmayanlarda MD Anderson skoru ortanca değeri 3,19 (1,79-3,95) saptandı. HER2 pozitifliği olanların ve HER2+ (HR+/-) moleküler alt tipinde olanların patolojik tam yanıt oranı daha yüksek olup istatistiksel olarak anlamlıydı (p=0,002, p=0,001). Tedaviye patolojik tam yanıtı olan hastalarda HER2 hedefli tedavi alanların oranı, tedaviye tam yanıt olmayan hastalara göre daha yüksek olup istatistiksel olarak anlamlıydı (p=0,005). Çalışmadaki hastaların patolojik yanıtlarına göre NAKT öncesi ve operasyon öncesi laboratuvar parametreleri değerlendirildiğinde anlamlı fark saptanmadı. İnflamatuar ve nutrisyonel belirteçler olan NLR, PLR, ALİ, PNİ belirteçlerinin patolojik tam yanıt yani tedaviye yanıt durumu ile anlamlı bir ilişkisi bulunmadı. Hastaların patolojik yanıtları lipit profili ve CONUT skoruna göre değerlendirildiğinde; HDL-Kolesterol, LDL-Kolesterol, trigliserid, CONUT skorunda anlamlı fark saptanmadı. Total kolesterol düzeyinde fark saptandı. Tedaviye yanıtı tam olan hastalarda total kolesterol düzeyi, tedaviye tam yanıt olmayan hastalara göre daha yüksek olup istatistiksel olarak anlamlıydı (p=0,024). Tedaviye tam yanıt olan hastalarda sigara kullanım oranı, tedaviye tam yanıt olmayanlara göre daha düşük olup istatistiksel olarak anlamlıydı (p=0,046). Sonuç: Çalışmamızda HER2 pozitif tümörlerde daha yüksek oranda pCR olduğunu ve HER2 hedefli tedavinin başarılı etkisini gösterdik. Bir prognostik faktör olarak sigara kullanımının pCR üzerinde negatif etkide olduğunu gözledik. Çalışmamızda, İnflamatuar ve nutrisyonel belirteçler olan NLR, PLR, ALİ, PNİ ve CONUT skorlaması ile NAKT almış meme kanserli hastalarda pCR arasında anlamlı bir ilişki bulunmadı. Total kolesterol düzeyi yüksek olan hastaların, pCR oranının daha fazla olduğunu ortaya bulduk. İlerleyen yıllarda yapılacak daha kapsamlı çalışmalardan elde edilecek sonuçlarla meme kanserli hastalarda total kolesterol düzeyinin baz alındığı yeni prognostik belirteçler oluşturulabilir.

Aim: The most crucial factor in delivering effective treatment for breast cancer is achieving a pathological complete response (pCR) following neoadjuvant chemotherapy. However, the clinical and pathological parameters that can accurately predict this response have not yet been clearly defined. Our study aimed to identify predictors of pCR using inexpensive and easily accessible inflammatory and nutritional markers. Materials and Methods: The study focused on female patients with locally advanced breast cancer who were 18 years of age or older and received treatment at the Sancaktepe Şehit Prof. Dr. İlhan Varank Training and Research Hospital Oncology Polyclinic between December 2022 and December 2024. All participants received neoadjuvant chemotherapy. Anthropometric measurements, blood types, comorbidities, chemotherapy regimens, menopausal state, tumor histological features, and demographic data were collected from patient files. The patient group was divided into two groups: those with and without pathological complete response. Before chemotherapy and surgery, we calculated the neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), prognostic nutritional index (PNI), advanced lung index (ALI), and controlling nutritional status (CONUT) scores. These parameters were analyzed to compare groups based on the presence or absence of a pathological complete response. Results: The study involved 73 patients with a mean age of 52.86 ± 10.85 years. Among the patients, 56 were positive for estrogen receptors (ER), and 49 were positive for progesterone receptors (PR). The median ER value was 80% (2-95), while the median PR value was 30% (0-70). The median antigen Kiel 67 (Ki-67) value was 30% (20-60). Additionally, 28 patients tested positive for the human epidermal growth factor receptor 2 (HER2). HER2 positivity was confirmed in 6 patients following the silver in situ hybridization (SISH) test. Based on their molecular subtypes, the classification revealed that 34 patients were HER2 positive (HR+/-), 31 were HER2 negative (HR+), and 8 were triple negative. When evaluating the pathological response status, it was found that 24 patients (32.9%) achieved a pCR (RCB0). The median MD Anderson score for those without a complete response was 3.19 (1.79-3.95). The rate of achieving a pCR was significantly higher among patients with HER2 positivity and HER2+ (HR+/-) molecular subtype (p=0.002, p=0.001). The number of patients who received a pCR after HER2-targeted therapy was significantly higher than those who did not (p=0.005). This difference was statistically significant. When examining the laboratory parameters before neoadjuvant chemotherapy and before surgery, no significant differences were found based on the pathological responses of the patients in the study. The inflammatory and nutritional markers, including the prognostic nutritional index (PNI), advanced lung index (ALI), neutrophil-lymphocyte ratio (NLR), and platelet-lymphocyte ratio (PLR), did not significantly correlate with the pCR, which measures the efficacy of the treatment. Furthermore, when assessing patients' pathological responses to their lipid profiles and the CONUT score, no significant differences were detected in HDL cholesterol, LDL cholesterol, triglycerides, or the CONUT score. However, a significant difference was noted in total cholesterol levels: patients exhibiting a pCR to treatment had higher total cholesterol levels than those who did not (p=0.024). This finding was statistically significant. Additionally, patients who achieved a pCR to treatment had significantly lower smoking rates than those who did not, and this was statistically significant (p=0.046). Conclusion: In our study, we showed a higher rate of pCR in HER2-positive tumors and the successful effect of HER2-targeted therapy. We observed that smoking as a prognostic factor has a negative effect on pCR. In our study, no significant relationship was found between the inflammatory and nutritional markers NLR, PLR, ALI, PNI, and CONUT scores and pCR in breast cancer patients who received NAKT. We found that patients with high total cholesterol levels had a higher pCR rate. New prognostic markers based on total cholesterol levels can be created in breast cancer patients with the results obtained from more comprehensive studies to be conducted in the coming years.
