Özet
Giriş: Viral solunum yolu enfeksiyonları, çocukluk çağında önemli bir mortalite ve morbidite nedenidir. Solunum yolu enfeksiyonlarına yol açan viral etkenlerinin immün sistem hücresel elemanları üzerinde direkt veya indirekt etkisinin olduğu bilinmektedir. Amaç: Bu çalışmada solunum yolu viral enfeksiyon sürecinde lenfositlerin oransal ve sayısal olarak değişikliğe uğradığı düşünülerek lenfosit alt gruplarında oluşturabileceği oransal veya sayısal etkilerinin değerlendirilmesi amaçlanmaktadır. Gereç ve Yöntem: Bu çalışma retrospektif kesitsel türde bir çalışmadır. Çalışma grubunu, S.B.Ü. İzmir Dr. Behçet Uz Eğitim ve Araştırma Hastanesi Çocuk Sağlığı ve Hastalıkları Klinikleri' nde 1 Ocak 2018 - 31 Ekim 2023 tarihleri arasında solunum yolu enfeksiyonu nedeniyle tedavi görmüş, viral enfeksiyon etkeni saptanıp enfeksiyon sürecinde lenfosit alt tiplendirme tetkiki ile değerlendirilmiş toplam 101 olgu oluşturmaktadır. Hastaların saptanmış olan viral solunum yolu enfeksiyon etkenine göre; cinsiyet, yaş, ek hastalık, enfeksiyon döneminde görülen lenfosit alt tiplendirme tetkikinde ve total lökosit sayısı (WBC), absolü nötrofil sayısı (ANS), absolü lenfosit sayısı (ALS)'nda görülen değişiklikler ve hastane yatış tanıları incelemeye alınmıştır. Bulgular: Çalışma grubunda solunum yolu enfeksiyonu olarak en sık pnömoni (n=76) olup en sık görülen etkenler de RSV ve Rhinovirüs olarak saptandı. (N=28, N=24). WBC, ALS ve ANS değerleri karşılaştırıldığında tüm etkenler arasında İnfluenza grubunda diğerlerine göre daha düşük ortalama değerler mevcuttu. Lenfosit alt tiplerinde yaşa göre patolojik sonuçlarda en sık Adenovirüs etken iken, Adenovirüsün en çok T ve NK hücreyi, İnfluenza'nın en çok B hücreyi etkilediği görüldü. Etken grupları arasında istatistiksel açıdan anlamlı fark yaratan parametrelerde İnfluenza diğer etkenlere göre bu parametreleri anlamlı derecede etkilemekteydi. Solunum sıkıntısı nedeniyle sistemik kortikosteroid alan olgularda almayan olgulara göre özellikle İnfluenza'da CD4, RSV ve Rhinovirüs enfeksiyonlarında CD3, CD4, NK hücrelerde görülen düşüklük anlamlıydı. RSV tanılı sistemik kortikosteroid tedavisi verilen olgularda CD19 lenfosit oranı yüksekliği ile tedavi gün sayısının pozitif yönde ilişkili olduğu saptandı. Sonuç: Viral solunum yolu etkenlerinin lenfosit alt tip dağılımı üzerindeki etkileri etkenlere göre değişkenlik göstermektedir. WBC değerlerinde görülen düşüklük influenza etkeni için diğer viral etkenlere göre ayırıcı bir bulgu iken adenovirüsde NK hücre düşüklüğü klinik değerlendirmede yol gösterici olabilir. Klinikte ağır solunum sıkıntısı nedeniyle akut bronşiolit ve hışıltı tanısıyla sistemik kortikosteroid kullanılan olguların lenfosit alt tip dağılımında viral etkenlere göre farklı etkiler görülmüştür. Bütün bu veriler ışığında çalışmamızın etken-enfeksiyon-tedavi kaskadında yeni yaklaşımlara ışık tutabileceğini düşünmekteyiz.

Introduction: Viral respiratory infections are an important cause of mortality and morbidity in childhood. It is known that viral factors that cause respiratory tract infections have a direct or indirect effect on the cellular elements of the immune system. Objective: In this study, we aimed to evaluate the proportional or numerical effects that may occur on lymphocyte subgroups, considering that lymphocytes change proportionally and numerically during the respiratory tract viral infections. Material and Methods: This study is a cross-sectional study. The case group consists of a total of 101 cases who were treated for respiratory tract infection at Izmir Dr. Behçet Uz Training and Research Hospital Child Health and Diseases Clinics between January 1, 2018 and October 31, 2023 and the viral infection agent was detected and evaluated with lymphocyte subtyping examination during the infection process. According to the detected viral respiratory tract infection agent of the patients; changes in the lymphocyte subtyping examination and total leukocyte count (WBC), absolute neutrophil count (ANS), absolute lymphocyte count (ALS) during the infection period were examined. Gender, age, treatment of systemic corticosteroids, comorbidities and hospitalization diagnoses of the patients were also examined. Results: In the study group, the most common respiratory tract infection was pneumonia (n=76), and the most common viruses were RSV and Rhinovirus. (N=28, N=24). When WBC, ALS and ANS values were compared, lower mean values were seen in the Influenza group compared to the other viruses. Adenovirus was detected as the most common virus in pathological lymphocyte subtype examination results. It was observed that Adenovirus affected mostly T and NK cells in contrast to Influenza affected B cells the most. Influenza significantly affected parameters that created a statistically significant difference between viral groups compared to other viruses. The decrease in CD4 cells especially in Influenza, CD4 and NK cells in RSV and Rhinovirus infections, was significant in patients who received systemic corticosteroids due to respiratory distress, compared to those who did not. It was found that in cases diagnosed with RSV and given systemic corticosteroid treatment, there was a positive correlation between the high CD19 lymphocyte ratio and the number of days of treatment. Conclusion: The effects of viral respiratory tract infections on lymphocyte subtype distribution vary depending on virus type. While low WBC values are a distinctive finding for Influenza, low NK cells in Adenovirus infections can be a guide in clinical evaluation. Different effects were observed depending on the viruses on the lymphocyte subtype distribution of the cases in which systemic corticosteroids were used with the diagnosis of acute bronchiolitis and wheezing due to severe respiratory distress. In light of all these data, we think that our study may shed light on new approaches in the agent-infection-treatment cascade.
