Özet

Rektum kanserinde sağ kalımın artışı ile rektum cerrahisinin yaşam kalitesi üzerindeki etkilerinin tespitinin yapılması da önemli noktalardan biri haline gelmiştir. Rektum kanseri tedavisinde tümör lokalizasyonu ve hastalığın evresi de dikkate alınarak, aşağı anterior rezeksiyon kolorektal anastomoz ve koloanal anastomoz tekniği uygulanabilmektedir. Çalışmamızda yaşam kalitesinin uygulanan aşağı anterior rezeksiyon kolorektal anastomoz ve koloanal anastomoz ameliyat tekniği ile ilişkisi analiz edilerek hastalığın ve uygulanan tedavi yönteminin hastaların yaşamlarını nasıl etkilediğine dair görüş sağlanması amaçlanmıştır. Bu doğrultuda etik kurul onayı alınarak Ocak 2019 ile Haziran 2024 tarihleri arasında Mersin Üniversitesi Hastanesi Genel Cerrahi Kliniği'nde rektum kanseri tanısı alan hastalar geriye dönük olarak incelendi ve dışlama/dahil edilme kriterleri dikkate alınarak hastalar aşağı anterior rezeksiyon (n=126) sonrası kolorektal (n=84) veya koloanal anastomoz yapılan (n=42) olmak üzere iki grup şeklinde çalışma yürütüldü. Katılımcılardan onam alındıktan sonra sırasıyla Kişisel Bilgi Formu, EORTC QLQ-C30 ve EORTC QLQ-CR29 Yaşam Kalitesi Ölçeği, Wexner Skoru, Fekal İnkontinans Yaşam Kalitesi Ölçeğini oluşturan sorular katılımcılara yöneltildi ve veriler toplandı. Sonuçta iki grup arasında tekniğe göre en belirgin farkın fekal inkontinans ve dışkı sıklığı durumlarında olduğu görüldü. Koloanal anastomoz yapılan grup olgularında Wexner skoruna göre değerlendirildiğinde inkontinans durumu oranı, kolorektal anastomoz yapılan grup olgularına göre istatistiksel olarak anlamlı düzeyde yüksek saptandı (p=0,004; p<0,01). Koloanal anastomoz yapılan grupta %28,6 oranında inkontinans görülürken, bu oran kolorektal anastomoz grubunda %8,3'tü. Koloanal anastomoz grubunda ortalama Wexner skoru 5,24±6,32 iken, kolorektal anastomoz yapılan grupta ortalama Wexner skoru 2,45±4,92'dir. Koloanal grubu olguların EORTC QLQ-CR29 yaşam kalitesi ölçeği“Gaz Çıkarma”(p=0,005),“Dışkı Sıklığı”(p=0,039),“ Utanma”(p=0,042) boyutlarından aldıkları puanlar, kolorektal grubu olgulara göre istatistiksel olarak anlamlı düzeyde yüksek saptandı(p<0,05). Rektum kanseri cerrahisinde yaşam kalitesinde etkili bir diğer bir unsur da cinsel fonksiyondur. Cinsel fonksiyon durumu ile ilgili sorular sorulduğunda hastaların verdikleri cevaplar sonucunda gruplara göre olguların EORTC QLQ-CR29 yaşam kalitesi ölçeği“İmpotans”,“Disparoni”ve“Cinsel İlgi”boyutlarından aldıkları puanlar, istatistiksel olarak anlamlı farklılık göstermedi (p>0,05). Her iki grup için cinsel ilgi puan ortalaması sırasıyla 44,93±34,24 ve 41,67±30,21 şeklindedir ve medyan değer 33,3'tür.Bağırsak disfonksiyonları ve fekal inkontinans yaşam kalitesinde etkili temel unsurlardan biri olsa da kolorektal anastomoz ve koloanal anastomoz yapılan olguların EORTC QLQ-C30 yaşam kalitesi ölçeği“Genel Sağlık Durumu”alt boyutundan aldıkları puanlarda, istatistiksel olarak anlamlı farklılık olmadığı görüldü (p>0,05).Bu da göstermektedir ki yaşam kalitesini etkileyen birçok faktör bulunmaktadır.

Determining the role of rectal surgery on quality of life has become one of the most important issues in the rising survival rate of rectal cancer. Lower anterior resection colorectal anastomosis and coloanal anastomosis techniques can be used in the treatment of rectal cancer, taking into account tumor localization and stage of the disease. In our study, we aimed to analyze the relationship between quality of life and surgical technique of low anterior resection colorectal anastomosis and coloanal anastomosis, and to provide an opinion on how the disease and treatment method affect the life of patients. Therefore, with the approval of the ethics committee, patients diagnosed with rectal cancer at the General Surgery Clinic of Mersin University Hospital between January 2019 and June 2024 were retrospectively reviewed and the study was conducted in two groups as colorectal (n=84) and coloanal anastomosis (n=42) after low anterior resection (n=126), taking into account the exclusion/inclusion criteria. Therefore, with the approval of the ethics committee, patients diagnosed with rectal cancer at the General Surgery Clinic of Mersin University Hospital between January 2019 and June 2024 were retrospectively reviewed and the study was conducted in two groups as colorectal (n=84) or coloanal anastomosis (n=42) after low anterior resection (n=126), taking into account the exclusion/inclusion criteria. After consent was obtained from the participants, personal information form, EORTC QLQ-C30 and EORTC QLQ-CR29 Quality of Life Scale, Wexner Score, Fecal Incontinence Quality of Life Scale questions were administered to the participants and data were collected. The results showed that the most significant difference between the two groups according to technique was in fecal incontinence and stool frequency. When evaluated according to the wexner score in the group of patients who underwent coloanal anastomosis, the rate of incontinence was found to be statistically significantly higher than in the group of patients who underwent colorectal anastomosis (p=0.004; p<0.01). Incontinence rate was 28.6% in coloanal anastomosis group and 8.3% in colorectal anastomosis group. The mean wexner score was 5.24±6.32 in the coloanal anastomosis group and 2.45±4.92 in the colorectal anastomosis group. Another factor that affects quality of life after rectal cancer surgery is sexual function. As a result of the patients responses to the sexual function status questions, the patients scores on the impotence, dyspareunia, and sexual interest dimensions of the EORTC QLQ-CR29 Quality of Life Scale did not show a statistically significant difference (p>0.05). The mean sexual interest scores for both groups were 44.93±34.24 and 41.67±30.21, respectively, and the median value was 33.3. Although bowel dysfunctions and fecal incontinence are among the basic factors affecting quality of life, it was observed that there was no statistically significant difference in the scores of the patients who underwent colorectal anastomosis and coloanal anastomosis from the“General Health Status”sub-dimension of the EORTC QLQ-C30 quality of life scale (p>0.05). This shows that there are many factors affecting quality of life.
