Özet
Amaç: ORİ (Oksijen rezerv indeksi), çoklu dalga boylu nabız ko-oksimetresi temelli, invaziv olmayan ve sürekli bir parametre olup, 0 ile 1 arasında değişen birimsiz bir değer ile parsiyel arteriyel oksijen basıncı (PaO2) 100-200 mmHg aralığını göstermeyi amaçlar. Bu sayede ORİ'nin hipoksemiyi ve hiperoksemiyi öngörebileceği düşünülmektedir. ETCO2, ekspirasyon sonunda ölçülen karbondioksit basıncını gösterir ve genellikle parsiyel arteriyel karbondioksit basıncı (PaCO2) ile ilişkilidir. Bu çalışmada, beyin ölümü tanısı amacıyla apne testi uygulanan hastalarda; ORİ ile PaO2 ve ETCO2 ile PaCO2 arasındaki ilişkinin belirlenmesi amaçlanmıştır. Gereç ve yöntem: Prospektif, gözlemsel çalışmamıza yoğun bakım ünitesinde yatmakta olan, beyin ölümü tanı sürecinin başlatılabilmesi için tüm ön şartları sağlayan, 1.nörolojik muayenesi beyin ölümü ile uyumlu olan ve tanı amacıyla apne testi uygulanmasına karar verilen 18 yaş ve üzeri 35 hasta dahil edildi. Rutin monitörizasyona ek ORİ ve ETCO2 monitörizasyonu yapıldı. Apne testi rutin şekilde uygulandı. Apne testi öncesi, başlangıç, 3. dakika, 5. dakika ve apne testi sonu vital bulgular, ORİ,“Pleth variability indeks”(PVİ), perfüzyon indeksi (Pİ), arteriyel kan gazında (AKG) pH, PaCO2, PaO2, arteriyel oksijen satürasyonu (SaO2) değerleri ile test öncesi, apne başlangıç ve apne sonu ETCO2 değerleri kaydedildi. ORİ ile PaO2, periferik oksijen satürasyonu (SpO2) ve SaO2 ilişkisi ve ETCO2 ile PaCO2 ilişkisi incelendi. Bulgular: 35 hastaya istatiksel analiz yapıldı, 6 hasta çalışmadan çıkarıldı. Apne testi öncesi ölçümde ORİ değeri ile PaO2, SpO2 ve SaO2 arasında pozitif yönlü orta kuvvette ilişki saptandı. (Sırasıyla r=0.494 p=0.003; r=0.395 p=0.019; r=0.416 p=0.013). Apne başlangıç ölçümünde ORİ ile SaO2 arasında pozitif yönlü orta kuvvette ilişki saptandı (r=0.371 p=0.028). Apne 3. dk ölçümünde ORİ ile PaO2 arasında pozitif yönlü orta kuvvette ilişki saptandı (r=0.366 p=0.030). Apne testi 5. dk ölçümünde ORİ değeri ile PaO2 ve SaO2 arasında pozitif yönlü güçlü ilişki saptandı (Sırasıyla r=0.569 p<0.001; r=0.573 p<0.001). Apne 5. dk ölçümünde ORİ ile SpO2 arasında pozitif yönlü orta kuvvette ilişki saptandı (r=0.458 p=0.006). Apne sonu ölçümde ORİ değeri ile PaO2 , SpO2 ve SaO2 arasında pozitif yönlü güçlü ilişki saptandı (Sırasıyla r=0.588 p<0.001; r=0.635 p<0.001; r=0.650 p<0.001). PaO2 değeri 200 üzeri olan olguların tahmininde ORİ için“cut off”değeri 0,440 olarak hesaplandı. Test öncesi ve apne başlangıç ölçümlerinde ETCO2 değerleri ile PaCO2, arasında ilişki olmadığı görüldü. Apne sonu ölçümde ise ETCO2 ile PaCO2 arasında pozitif yönlü orta kuvvette ilişki saptandı (r=0.423 p=0.011). Apne testi başlangıcına göre test sonunda ölçülen PaCO2 - ETCO2 gradyanı anlamlı yüksek bulundu (p<0.001). Sonuç: Apne önkoşulu olan normokarbinin değerlendirilmesinde ETCO2 monitörizasyonu PaCO2'i doğru yansıtmayabileceğinden yarar sağlamayacaktır. ORİ nispeten ılımlı hiperoksik aralığa kadar PaO2 ile güçlü ilişilidir. ORİ, bulduğumuz 0,44“cut off”ile apne testi öncesinde PaO2'in 200 mmHg üzerine çıkıp çıkmadığı konusunda fikir vererek test öncesi AKG ölçüm sıklığını azaltabileceğini düşünmekteyiz. ORİ'nin apne testi süresince SpO2 monitörizasyonuna ek olarak kullanılması hipokseminin daha erken fark edilmesini sağlayabilir.

Aim: ORI is a non-invasive and continuous parameter based on multi-wavelength pulse co-oximetry, providing a unitless value ranging from 0 to 1, intended to reflect arterial oxygen partial pressure (PaO2) within the 100–200 mmHg range. As a result of this, ORI is thought to have predictive value for both hypoxemia and hyperoxemia. ETCO2 indicates the partial pressure of carbon dioxide measured at the end of expiration and is generally correlated with arterial carbon dioxide pressure (PaCO2). This study aims to determine the relationship between ORI and PaO2, and between ETCO2 and PaCO2 in patients undergoing an apnea test as part of brain death diagnosis. Materials and methods: A total of 35 patients aged 18 years or older who were hospitalized in the intensive care unit, met all prerequisites for the initiation of brain death determination, had findings consistent with brain death on the first neurological examination, and for whom the apnea test was deemed necessary were included in this prospective observational study. In addition to routine monitoring, ORI and ETCO2 monitoring were performed. The apnea test was conducted routinely. Vital signs, ORI, pleth variability index (PVI), perfusion index (PI) and arterial blood gas (ABG) parameters including pH, PaCO2, PaO2, and arterial oxygen saturation (SaO2) were recorded at five time points: before the test, at the beginning of apnea, at the 3rd minute of apnea, at the 5th minute of apnea, and at the end of apnea. ETCO2 values were recorded at three time points: before the test, at the beginning of apnea, and at the end of apnea. The relationships between ORI and PaO2, peripheral oxygen saturation (SpO2), and SaO2, as well as between ETCO2 and PaCO2, were analyzed. Results: Statistical analysis was performed on 35 patients. In the measurements before the test, a moderate positive correlation was observed between ORI and PaO₂, SpO2, and SaO2 (r=0.494, p=0.003; r=0.395, p=0.019; r=0.416, p=0.013, respectively). At the beginning of apnea, a moderate positive correlation was found between ORI and SaO2 (r=0.371, p=0.028). At the 3rd minute of apnea, a moderate positive correlation was observed between ORI and PaO2 (r=0.366, p=0.030). At the 5th minute of apnea, a strong positive correlation was found between ORI and PaO2, and between ORI and SaO2 (r=0.569, p<0.001; r=0.573, p<0.001, respectively). A moderate positive correlation was also found between ORI and SpO2 at the 5th minute of apnea (r=0.458, p=0.006). At the end of apnea, a strong positive correlation was observed between ORI and PaO2, SpO2, and SaO2 (r=0.588, p<0.001; r=0.635, p<0.001; r=0.650, p<0.001, respectively). The cutoff value of ORI for predicting PaO2 values above 200 mmHg was calculated as 0.440. No correlation was observed between ETCO2 and PaCO2 in the measurements taken before apnea test and at the initiation of the apnea test. At the end of apnea, a moderate positive correlation was found between ETCO2 and PaCO2 (r=0.423, p=0.011). The PaCO2–ETCO2 gradient measured at the end of the test was significantly higher compared to the beginning of the apnea test (p<0.001). Conclusion: ETCO2 monitoring may not be useful in evaluating normocarbia, which is a prerequisite for the apnea test, as it may not accurately reflect PaCO2. ORI shows a strong correlation with PaO2 up to a relatively moderate hyperoxic range. With the cutoff value of 0.44 identified in this study, ORI may provide an estimate of whether PaO2 exceeds 200 mmHg prior to the apnea test, potentially reducing the frequency of pre-test ABG measurements. Additionally, the use of ORI alongside SpO2 monitoring during the apnea test may facilitate earlier detection of hypoxemia.
