Recurrent hypoglycemia, commonly resulting from intensive insulin therapy or an unregulated diet, acts as a metabolic stressor with potential long-term systemic effects. While its neurological consequences are well documented, its broader impacts on genomic stability remain insufficiently explored. Environmental stressors can induce epigenetic changes in cells, and repeated exposure may render these changes permanent. This study aims to investigate whether repeated hypoglycemia affects telomere regulation in ocular cells, which are highly dependent on glucose. Telomeres preserve chromosomal ends, maintaining genome stability and determining cellular lifespan. Recent findings show that telomere stability is regulated not only by telomerase and the shelterin complex but also by TERRA (telomeric repeat-containing RNA), a long non-coding RNA transcribed from subtelomeric regions. TERRA binds to telomeres and exists in two nuclear forms: free (fTERRA) and hybridized (hTERRA). Disruption in TERRA homeostasis is associated with cellular aging and tissue degeneration. In this study, a mouse model of insulin-induced hypoglycemia was developed using 56 Balb/C mice divided into acute, chronic, and control groups. Total RNA, DNA, and DNA:RNA hybrids were isolated from ocular tissues. Telomere length, fTERRA, and hTERRA expression levels were assessed using qRT-PCR. Results showed a significant reduction in hTERRA levels in the acute hypoglycemia group, while fTERRA expression remained unchanged across all hypoglycemic groups. However, hTERRA expression was significantly reduced in both acute and chronic recurrent groups. All hypoglycemia groups exhibited telomere shortening, most prominently in the acute group. No significant sex-based differences were observed. These findings suggest that even a single severe hypoglycemic event can lead to irreversible molecular damage, while repeated episodes may impair telomere stability and TERRA regulation, contributing to genetic instability. Understanding these mechanisms may offer insights into ocular aging in diabetes and support the development of telomere-targeted therapies.
