Özet

Giriş ve Amaç: Meme kanseri, kadınlarda en sık görülen malignite olup kansere bağlı ölümlerin önde gelen nedenlerindendir. Neoadjuvan kemoterapi (NAK) sonrası elde edilen patolojik tam yanıt (pCR), uzun dönem sağkalımın önemli bir göstergesidir. Ancak memedeki yanıt (B-pCR) ile aksiller lenf nodlarındaki yanıtın (N-pCR) birbirinden bağımsız seyredebildiği ve prognoz üzerine farklı etkiler gösterebildiği bilinmektedir. Bu çalışmada NAK uygulanmış cN(+) hastalarda B-pCR, N-pCR ve toplam pCR (T-pCR) oranları ile sağkalım sonuçları; ayrıca yalnızca memede pCR ve yalnızca aksillada pCR alt grupları incelendi. Gereç ve Yöntem: Ocak 2017 – Aralık 2024 arasında SBÜ Bakırköy Dr. Sadi Konuk Eğitim ve Araştırma Hastanesi Tıbbi Onkoloji Kliniği'nde NAK almış ve cerrahi uygulanmış 171 kadın hasta retrospektif olarak incelendi. Hastaların klinik ve patolojik T/N evreleri, moleküler özellikleri, uygulanan tedavi protokolleri ve cerrahi tipleri kaydedildi. Progresyonsuz sağkalım ve genel sağkalım analizleri Kaplan–Meier yöntemi ile hesaplandı, gruplar log-rank testiyle karşılaştırıldı; prognostik faktörler Cox regresyon modeliyle değerlendirildi. Bulgular: Yanıt oranları: B-pCR %26,3, N-pCR %40,4, T-pCR %22,2 olarak bulundu. Sadece memede pCR %18,4, sadece aksillada pCR %81,6 olarak bulundu. Medyan OS 78 ay, PFS: 55,9 ay olarak saptandı. B-pCR grubunda OS ve PFS, B-non pCR'ye göre anlamlı derecede daha uzundu (OS: 78 vs 62,3 ay, p=0,021; PFS: 66,8 vs 51,5 ay, p=0,002). N-pCR grubunda da benzer şekilde OS (74,0 vs 60,2 ay, p=0,003) ve PFS (68,0 vs 47,0 ay, p<0,001) anlamlı olarak daha yüksekti. T-pCR elde edilenlerde hem OS (78 vs 62,4 ay, p=0,013) hem de PFS (67,5 vs 52,1 ay, p=0,002) üstün bulundu. Alt grup analizinde OS; sadece memede pCR elde edilenlerde 55,6 ay, sadece aksillada pCR elde edilenlerde 67,7 ay olup, iki grup arasında istatistiksel fark saptanmadı (p>0,05). Cox regresyon analizinde; tek değişkenli analizlerde B-pCR, N-pCR, T-pCR, vasküler invazyon, ileri T evresi ve metastaz varlığı OS ile ilişkili bulundu. Çok değişkenli analizde sadece metastaz varlığı bağımsız kötü prognostik faktör olarak bulundu. PFS için tek değişkenli analizde pCR varlığı, tümör çapı >36 mm, vasküler invazyon, HER2 pozitifliği ve yüksek T/N evresi olumsuz etkenlerdi. Çok değişkenli analizde yalnızca vasküler invazyon bağımsız risk faktörü olarak anlamlı bulundu. Sonuç: Neoadjuvan kemoterapi sonrası hem memede hem aksillada patolojik tam yanıt elde edilmesi, progresyonsuz sağkalım ve genel sağkalımı anlamlı ölçüde iyileştirmektedir. Sadece meme veya sadece aksillada tam yanıt elde edilmesi anlamlı bulunmamıştır. Bununla birlikte çok değişkenli analizde metastaz varlığı ve vasküler invazyonun bağımsız prognostik faktörler olduğu görülmüştür. Bu sonuçlar, tedavi yanıtının yalnızca memedeki tümörle değil, aksiller lenf nodu durumuyla birlikte değerlendirilmesi gerektiğini göstermektedir.

Introduction and Aim: Breast cancer is the most common malignancy in women and one of the leading causes of cancer-related deaths. Pathological complete response (pCR) after neoadjuvant chemotherapy (NAC) is considered an important predictor of long-term survival. However, responses in the breast (B-pCR) and in the axillary lymph nodes (N-pCR) may occur independently and have different prognostic implications. This study evaluated B-pCR, N-pCR, and total pCR (T-pCR) rates and their impact on survival outcomes in cN(+) patients treated with NACT. In addition, subgroups with pCR limited to the breast or to the axilla were analyzed. Materials and Methods: A total of 171 female patients who received NAC and subsequently underwent surgery at the Medical Oncology Clinic of SBÜ Bakırköy Dr. Sadi Konuk Training and Research Hospital between January 2017 and December 2024 were retrospectively analyzed. Clinical and pathological T/N stages, molecular features, treatment protocols, and surgical types were recorded. Progression-free survival (PFS) and overall survival (OS) were estimated using the Kaplan–Meier method, compared with the log-rank test, and prognostic factors were evaluated with Cox regression analysis. Results: Response rates were as follows: B-pCR 26.3%, N-pCR 40.4%, and T-pCR 22.2%. Among partial responders, 18.4% achieved pCR in the breast only, and 81.6% in the axilla only. Survival: Median OS was 78 months, and median PFS was 55.9 months. OS and PFS were significantly longer in the B-pCR group compared with B-non pCR (OS: 78 vs. 62.3 months, p=0.021; PFS: 66.8 vs. 51.5 months, p=0.002). Similarly, N-pCR was associated with superior OS (74.0 vs. 60.2 months, p=0.003) and PFS (68.0 vs. 47.0 months, p<0.001). T-pCR was also associated with improved OS (78 vs. 62.4 months, p=0.013) and PFS (67.5 vs. 52.1 months, p=0.002). Subgroup analysis: OS was 55.6 months in patients with pCR limited to the breast and 67.7 months in those with pCR limited to the axilla; however, the difference was not statistically significant (p>0.05). Cox regression: In univariate analysis, B-pCR, N-pCR, T-pCR, vascular invasion, advanced T stage, and presence of metastasis were associated with OS. In multivariate analysis, only presence of metastasis remained an independent adverse prognostic factor. For PFS, univariate analysis identified pCR, tumor size >36 mm, vascular invasion, HER2 positivity, and advanced T/N stage as significant factors; multivariate analysis revealed vascular invasion as the sole independent risk factor. Conclusion: Achieving pCR in both the breast and axilla after neoadjuvant chemotherapy significantly improves PFS and OS. pCR limited to either the breast or the axilla alone was not associated with significant survival benefit. Multivariate analysis identified presence of metastasis and vascular invasion as independent prognostic factors. These findings indicate that treatment response should be assessed not only in the breast but also in the axillary lymph nodes for accurate prognostic evaluation.
