Özet
Bu çalışmada, Hypericum perforatum (kantaron) yağının, deneysel diyabet oluşturulmuş gebe ratlarda doğum sonrası yara iyileşmesine etkisi histopatolojik açıdan değerlendirildi. Çalışmada 28 adet dişi Wistar albino rat, rastgele dört gruba ayrıldı: Grup 1 (Kontrol), Grup 2 (Gebe), Grup 3 (Diyabet), Grup 4 (Diyabetik Gebe). Diyabet, streptozotosin (30 mg/kg, i.p.) ile oluşturuldu; gebelik smear yöntemiyle doğrulandı. Doğum sonrası tüm gruplara sırt bölgesine insizyonel ve eksizyonel yara modelleri uygulandı. İnsizyonel yaraların tüm gruplarda tamamen iyileşmiş olması nedeniyle sadece eksizyonel yaraların histopatolojik değerlendirmesi yapıldı. Epitelizasyon açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmazken (p>0.05), kantaron uygulanan gruplarda epitel hücre yenilenmesinin arttığı gözlendi. İnflamasyon, gebe grupta şiddetli iken, kantaron uygulanan diyabetik gebe grupta anlamlı düzeyde azaldı (p<0.05). Fibroblast proliferasyonu, gebe grupta en yüksek düzeyde gözlenirken, kantaron uygulanan grupta hafif şiddetliydi (p<0.001). Neovaskülarizasyon ise gebe grupta en yoğun şekilde gerçekleşti (p<0.05). Sonuç olarak, kantaron yağı uygulamasının, özellikle inflamasyonu azaltma ve epitelizasyonu destekleme açısından yara iyileşmesini olumlu yönde etkilediği, ancak gebelik ve diyabetin birlikte olduğu durumlarda bu sürecin daha karmaşık bir yapıya sahip olduğu belirlenmiştir.

In this study, the effect of Hypericum perforatum (St. John's wort) oil on postnatal wound healing was histopathologically evaluated in pregnant rats with experimentally induced diabetes. A total of 28 female Wistar albino rats were randomly divided into four groups: Group 1 (Control), Group 2 (Pregnant), Group 3 (Diabetic), and Group 4 (Diabetic Pregnant). Diabetes was induced using streptozotocin (30 mg/kg, i.p.), and pregnancy was confirmed by vaginal smear test. After delivery, all animals underwent inscisional and excisional wound modeling on the dorsal region. As incisional wounds fully healed in all groups, only excisional wounds were evaluated histopathologically. Although no statistically significant difference was found between the groups in terms of epithelialization (p>0.05), epithelial cell regeneration was increased in the groups treated with Hypericum perforatum. Inflammation was most severe in the pregnant group, whereas it significantly decreased in the diabetic pregnant group treated with St. John's wort (p<0.05). Fibroblast proliferation was highest in the pregnant group and milder in the diabetic pregnant group (p<0.001). Neovascularization was significantly increased in the pregnant group compared to the control and diabetic pregnant groups (p<0.05). In conclusion, topical application of Hypericum perforatum oil had a positive effect on wound healing, particularly by reducing inflammation and supporting epithelialization. However, it was observed that the coexistence of pregnancy and diabetes made the wound healing process more complex.
