''ROMATOİD ARTRİT HASTALARI İÇİN OLUŞTURULMUŞ İDEAL BESİN PİRAMİDİNİN HASTALIK AKTİVİTESİNE VE BAĞIRSAK MİKROBİYOTASINA ETKİSİ'' Ülger KAÇAR MUTLUTÜRK Erciyes Üniversitesi Sağlık Bilimleri Enstitüsü Beslenme ve Diyetetik Anabilim Dalı, Doktora Tezi, Mayıs 2025 Danışman: Prof. Dr. Betül ÇİÇEK ÖZET Romatoid artrit (RA), bağırsak mikrobiyotasının değiştiği inflamatuar bir otoimmün hastalıktır. Diyet bağırsak mikrobiyotayı etkileyen temel çevresel faktörlerden birisidir. Bu araştırma RA'da İdeal Besin Piramidi'nin bağırsak mikrobiyotası, hastalık aktivitesi, biyokimyasal bulgular ve antropometrik ölçümler üzerindeki etkisini belirlemek amacıyla yürütülmüştür. Dahil edilme kriterlerini karşılayan RA tanısı almış otuz hasta rastgele diyet ve kontrol gruplarına ayrıldı ve 12 hafta boyunca takip edildi. Sosyodemografik özellikler, hastalık aktiviteleri, antropometrik ölçümler, kan ve gaita örnekleri tüm katılımcılardan çalışma başlangıcında ve sonunda alınıp karşılaştırıldı. Bağırsak mikrobiyota kompozisyonu 16S rRNA gen dizilimi ile belirlendi. Diyet grubuna RA için İdeal Besin Piramidi uygulandı, kontrol grubuna ise herhangi bir müdahale yapılmadı. Çalışma sonunda diyet grubu (n=16) kontrol grubuyla (n=14) karşılaştırıldığında; vücut kompozisyonu (vücut ağırlığı (p=0,002), Beden Kütle İndeksi (BKİ) (p=0,015), yağ yüzdesi (p=0,014), yağ kütlesi (p=0,008), bel çevresi (p<0,001), bel/boy oranı (p<0,001), kalça çevresi (p<0,001), bel/kalça oranı (p=0,031), boyun çevresi (p=0,001), bilek çevresi (p=0,001), boy/bilek çevresi (p=0,001), el kavrama gücü (p<0,001)) ve hastalık aktiviteleri (hassas eklem sayısı (p<0,001), Hastalık Aktivite Skoru-28-Eritrosit Sedimentasyon Hızı (DAS28-ESR) (p<0,001), Hastalık Aktivite Skoru Skor-28-C Reaktif Protein (DAS28-CRP) (p<0,001), Basit Hastalık Aktivite İndeksi (SDAI) (p=0,008), Görsel Analog Skala (VAS)-ağrı (p<0,001), Kısa Form Sağlık Anketi (SF36) tüm parametreleri (p<0,05), Romatoid Artrit Yaşam Kalitesi (RAQoL) (p<0,001), Sağlık Değerlendirme Anketi (HAQ) (p<0,001) arasında anlamlı farklar bulunmuştur. İnflamasyon belirteçleri olan CRP (p=0,015) ve ESR (p=0,001) diyet grubunda anlamlı olarak azalırken, kontrol grubunda her ikisi de anlamlı olarak artmıştır (sırasıyla p=0,002, p=0,001). Çalışmanın sonunda gruplar arasında düşük yoğunluklu lipoprotein (LDL)-kolesterol ve toplam kolesterol düzeylerinde anlamlı farklılıklar bulunmuştur (sırasıyla p=0,013, p=0,008). Çalışmanın sonunda anlamlı bir fark olmasa da diyet grubunda kontrol grubuna göre bağırsak mikrobiyotasının çeşitliliği, zenginliği ve kompozisyonunda değişiklikler gözlemlenmiştir. Alfa çeşitliliğini gösteren Simpson, Shannon ve Chao-1 indeksleri çalışma sonunda diyet grubunda kontrol grubuna göre daha yüksek bulunmuştur (p>0,05). Çalışmanın sonunda, filum düzeyinde Firmicutes diyet grubunda artarken kontrol grubunda azalmıştır; Bacteroidetes ise diyet grubunda azalırken kontrol grubunda artmıştır. Diyet grubunda cins düzeyine bakıldığında, Prevotella, Bacteroides, Lachnospira, Clostridium, Blautia, Sutterella, Streptococcus, Lactobacillus ve Collinsella görece bolluğu azalırken; Faecalibacterium, Roseburia, Oscillospira, Ruminococcus, Akkermansia, Coprococcus, Parabacteroides, Gemmiger'in görece bolluğu artmıştır. Diyet grubunda tür düzeyine bakıldığında; Prevotella copri, Bacteroides fragilis, Prevotella stercorea, Bacteroides uniformis göreceli bolluğu azalırken, Faecalibacterium prausnitzii, Roseburia faecis, Bacteroides ovatus, Akkermansia muciniphila, Coprococcus eutactus, Gemmiger formicilis, Ruminococcus bromii, Bifidobacterium longum türlerinin görece bolluğu artmıştır. Kontrol grubunda ise, Prevotella, Prevotella copri artarken, Faecalibacterium, Bacteroides, Faecalibacterium prausnitzii azalmıştır. Sonuçlar, RA'da kullanılan İdeal Besin Piramidi'nin potansiyel olarak yararlı mikrobiyal bakterilerin bolluğunu artırarak bağırsak mikrobiyotasını düzenlediğini göstermektedir. Ayrıca obezite, inflamasyon, kan lipid profili ve hastalık aktivitesi üzerinde de olumlu sonuçlar göstermiştir. Bulgular, İdeal Besin Piramidi kullanımının RA hastaları için pratikte önerilebileceğini göstermektedir.

'THE EFFECT OF THE IDEAL FOOD PYRAMID FOR RHEUMATOID ARTHRITIS PATIENTS ON DISEASE ACTIVITY AND GUT MICROBIOTA' Ülger KAÇAR MUTLUTÜRK Erciyes University Graduate School of Health Sciences Department of Nutrition and Dietetics, Ph.D., Thesis, May 2025 Advisors: Prof. Dr. Betül ÇİÇEK ABSTRACT Rheumatoid arthritis (RA) is an inflammatory autoimmune disease in which the gut microbiota is altered. Diet is one of the main environmental factors affecting the gut microbiota. This study was conducted to determine the effects of the Ideal Food Pyramid on gut microbiota, disease activity, biochemical findings and anthropometric measurements in RA. Thirty patients diagnosed with RA who met the inclusion criteria were randomly assigned to diet and control groups and followed up for 12 weeks. Sociodemographic characteristics, disease activities, anthropometric measurements, blood and stool samples were taken from all participants at the beginning and end of the study and compared. Gut microbiota composition was determined by 16S rRNA gene sequencing. The Ideal Food Pyramid for RA was applied to the diet group, while the control group did not receive any intervention. When the diet group (n=16) was compared with the control group (n=14) at the end of the study; body composition (body weight (p=0.002), Body Mass Index (BMI) (p=0.015), fat percentage (p=0.014), fat mass (p=0.008), waist circumference (p<0.001), waist/height ratio (p<0.001), hip circumference (p<0.001), waist/hip ratio (p=0.031), neck circumference (p=0.001), wrist circumference (p=0.001), height/wrist circumference (p=0.001), hand grip strength (p<0.001)) and disease activities (tender joint number (p<0.001), Disease Activity Score-28-Erythrocyte Sedimentation Rate (DAS28-ESR) (p<0.001), Disease Activity Score Score-28-C Reactive Protein (DAS28-CRP) (p<0.001), Simple Disease Activity Index (SDAI) (p=0.008), Visual Analog Scale (VAS)-pain (p<0.001), Short Form Health Questionnaire (SF36) all parameters (p<0.05), Rheumatoid Arthritis Quality of Life (RAQoL) (p<0.001), Health Assessment Questionnaire (HAQ) (p<0.001) were found to have significant differences. Inflammation markers CRP (p=0.015) and ESR (p=0.001) decreased significantly in the diet group, while both increased significantly in the control group (p=0.002, p=0.001, respectively). At the end of the study, significant differences were found in low-density lipoprotein (LDL)-cholesterol and total cholesterol levels between the groups (p=0.013, p=0.008, respectively). Although there was no significant difference at the end of the study, intestinal Changes were observed in the diversity, richness and composition of the microbiota. Simpson, Shannon and Chao-1 indices indicating alpha diversity were found to be higher in the diet group than in the control group at the end of the study (p>0.05). At the end of the study, at the phylum level, Firmicutes increased in the diet group and decreased in the control group; Bacteroidetes decreased in the diet group and increased in the control group. When looking at the genus level in the diet group, the relative abundances of Prevotella, Bacteroides, Lachnospira, Clostridium, Blautia, Sutterella, Streptococcus, Lactobacillus and Collinsella decreased; while the relative abundances of Faecalibacterium, Roseburia, Oscillospira, Ruminococcus, Akkermansia, Coprococcus, Parabacteroides, Gemmiger increased. When looking at the species level in the diet group; the relative abundances of Prevotella copri, Bacteroides fragilis, Prevotella stercorea, Bacteroides uniformis decreased, while Faecalibacterium prausnitzii, Roseburia faecis, the relative abundance of Bacteroides ovatus, Akkermansia muciniphila, Coprococcus eutactus, Gemmiger formicilis, Ruminococcus bromii, Bifidobacterium longum species increased. In the control group Prevotella, Prevotella copri increased, while Faecalibacterium, Bacteroides, Faecalibacterium prausnitzii decreased. The results show that the Ideal Food Pyramid used in RA regulates the intestinal microbiota by increasing the abundance of potentially beneficial microbial bacteria. It also showed positive results on obesity, inflammation, blood lipid profile and disease activity. The findings show that the use of the Ideal Food Pyramid can be recommended in practice for RA patients.
