Özet
Giriş ve Amaç: Çölyak hastalığı çocukluk döneminde özellikle büyüme gelişme geriliği, kronik ishal, abdominal distansiyon, karın ağrısı, iştahsızlık, anemi, romatolojik ve nörolojik hastalıklar gibi gastrointestinal ve ekstraintestinal bulgularla karşımıza çıkabilir. Çölyak hastalığı tanılı bireylerde glutensiz diyete uyumun çocuklardaki büyümeye ve beslenme parametrelerine etkisi ile ilgili yeterli sayıda veri ve çalışma bulunmamaktadır. Bu çalışmanın amacı, Sağlık Bilimleri Üniversitesi Ümraniye Eğitim ve Araştırma Hastanesi Çocuk Gastroenteroloji Polikliniğine başvuran ve ESPGHAN (European Society for Paediatric Gastroenterology, Hepatology and Nutrition) tarafından tanımlanan kriterlere göre tanı alan çölyak hastalarında, tanı anı ile 1 yıl sonraki glutensiz diyete uyum sonrası yapılan antropometrik ölçümler ve laboratuvar verilerini retrospektif olarak inceleyerek glutensiz diyetin yıllık büyüme hızına etkisini değerlendirmektir. Gereç ve Yöntem: Kasım 2016- Temmuz 2024 tarihleri arasında çölyak tanısı alan hastaların retrospektif olarak tanı anındaki ve 1 sene sonra rutin poliklinik kontrollerindeki çölyak serolojisi ve diğer laboratuvar değerleri, antropometrik ölçümleri, yaşa göre yıllık büyüme hızları, takip süresi ve glutensiz diyete uyan ve uymayan çölyak hastaları incelenerek 733 hasta tarandı. Down sendromu, Turner sendromu, Williams sendromu, serebral palsi, IgA eksikliği, takibi bırakan, vücut ölçümleri ve laboratuvar verileri eksik hastalar çalışma dışı tutuldu. 1-18 yaş arası 357 hasta çalışmaya dahil edildi. Olgular 1-5, 6-11, 12-18 yaş olarak 3 gruba ayrılarak tanı anında ve 1. yıl poliklinik kontrollerinde büyüme parametreleri ve yıllık büyüme hızları diyete uyan ve uymayanlar kıyaslanarak değerlendirildi. Ayrıca tüm olgularda başlangıç ve 1 yıl sonraki kontrollerinde anti-endomisyum (EMA) ve doku transglutaminaz (tTG) IgA, diğer laboratuvar tetkikleri ve vitamin düzeyleri diyete uyan ve uymayanlar karşılaştırılarak incelendi. Bulgular: 733 hastanın retrospektif taramasında, uygun kriterleri sağlayan 12– 199 ay aralığında toplam 357 çocuk (%62,5 kız, %37,5 erkek) çalışmaya dahil edildi. Ortalama yaş 104,2 ± 47,6 ay (8,7 ± 3,9 yıl), medyan yaş ise 105 ay (8,75 yıl) olarak bulundu. Olguların %26,6'sı 1–5 yaş, %50,7'si 6–11 yaş, %22,7'si 12–18 yaş grubundaydı. En sık başvuru nedenleri karın ağrısı (%44,8), ishal (%17,1) ve kilo alamama (%15,4) olup bunları kabızlık, boy kısalığı, tarama, iştahsızlık, dirençli anemi, kusma ve bulantı izledi. Olguların %22,4'ünde ek hastalık saptandı; en sık Tip 1 DM (%9,8) ve anemi (%6,2) idi. Hastaların %60,8'i diyete uyumlu, %39,2'si uyumsuzdu; cinsiyetler arasında fark yokken, 1–5 yaş grubunda uyum oranı (%69,5) adölesanlara göre (%50,6) anlamlı derecede yüksekti. Diyete uyum açısından değerlendirildiğinde, her üç yaş grubunda da diyete uyumlu olguların yıllık büyüme hızları, uyumsuz gruba kıyasla istatistiksel olarak anlamlı şekilde yüksekti (1–5 yaş p=0.019; 6–11 yaş p=0.019; 12–18 yaş p=0.019; p<0.05). Ancak, en yüksek büyüme hızı artışı 1–5 yaş grubunda, en düşük ise 12–18 yaş grubunda gözlendi. Diyete uyumlu grupta boy, boy SDS ve boy persentil ortalamaları tüm yaş gruplarında uyumsuzlara göre anlamlı şekilde daha yüksek bulunmuştur (1–5 yaş boy p=0.001; boy SDS p=0.001; boy persentil p=0.001; 6–11 yaş boy p=0.001; boy SDS p=0.005; boy persentil p=0.006; 12–18 yaş boy p=0.001; boy SDS p=0.001; boy persentil p=0.001; tüm değerler p<0.05). Diyete uyumlu çocukların tartı, tartı SDS ve tartı persentil ortalamaları tüm yaş gruplarında diyete uymayanlara kıyasla istatistiksel olarak anlamlı düzeyde daha yüksek bulunmuştur (1–5 yaş tartı p=0.019; tartı SDS p=0.001; tartı persentil p=0.001; 6–11 yaş tartı p=0.001; tartı SDS p=0.001; tartı persentil p=0.001; 12–18 yaş tartı p=0.001; tartı SDS p=0.001; tartı persentil p=0.001; tüm değerler p<0.05). Diyete uyumlu çocukların ortalama VKİ, VKİ SDS ve VKİ persentil değerleri tüm gruplarda uyum göstermeyenlere kıyasla anlamlı olarak daha yüksek bulunmuştur (1–5 yaş VKİ p=0.003; VKİ SDS p=0.004; VKİ persentil p=0.004; 6–11 yaş VKİ p=0.007; VKİ SDS p=0.002; VKİ persentil p=0.002; 12–18 yaş VKİ p=0.001; VKİ SDS p=0.035; VKİ persentil p=0.001; tüm değerler p<0.05). Ayrıca hemoglobin, MCV, demir, ferritin, folat, B12 ve D vitamini düzeylerinde anlamlı iyileşmeler izlendi. Serolojik olarak, uyumlu grupta tTG-IgA düzeyleri düşerken EMA büyük oranda negatifleşti; uyumsuz grubun %95'inde EMA pozitifliği sürdü. Tartışma ve Sonuç: Bu çalışmada glutensiz diyete uyum sağlayan çocuklarda, büyüme parametreleri ve laboratuvar bulgularında anlamlı düzelmeler saptandı. Özellikle küçük yaş gruplarında diyete uyum oranı daha yüksek olup, yıllık büyüme hızları da belirgin biçimde artış gösterdi. Diyet uyumunun küçük yaşlarda daha yüksek olması, özellikle adölesan dönemde uyumu artıracak eğitim, aile danışmanlığı ve yakın izlem gereksiniminin önemi vurgulanmaktadır. Diyete uyan çocukların boy, kilo ve VKİ değerlerinde hem mutlak hem de SDS-persentil bazında artış gözlenirken, uyumsuz grupta özellikle adölesanlarda gerileme dikkat çekti. Hematolojik açıdan, diyete uyan grupta hemoglobin ve MCV yükselmiş, demir, ferritin, folat, vitamin B12 ve D vitamini düzeylerinde anlamlı iyileşme kaydedilmiştir. Serolojik olarak, diyete uyumlu grupta anti-tTG düzeylerinde düşüş ve EMA'nın büyük oranda negatifleşmesi mukozal iyileşme ile uyumludur. Bu bulgular, glutensiz diyete sıkı uyumun büyüme, beslenme ve inflamatuar süreçler üzerinde belirgin yarar sağladığını ortaya koymaktadır.

Introduction and Aim: Celiac disease may present in childhood particularly with gastrointestinal and extraintestinal manifestations such as growth retardation, chronic diarrhea, abdominal distension, abdominal pain, loss of appetite, anemia, rheumatologic and neurological disorders. There is insufficient data and limited number of studies regarding the effect of adherence to a gluten-free diet on growth and nutritional parameters in children diagnosed with celiac disease. The aim of this study is to retrospectively evaluate the anthropometric measurements and laboratory data of celiac patients, who were diagnosed according to the criteria defined by ESPGHAN (European Society for Paediatric Gastroenterology, Hepatology and Nutrition) and followed at the Pediatric Gastroenterology Outpatient Clinic of University of Health Sciences Ümraniye Training and Research Hospital, at the time of diagnosis and after 1 year of adherence to a gluten-free diet, in order to assess the effect of the gluten-free diet on annual growth velocity. Materials and Methods: Between November 2016 and July 2024, a total of 733 patients diagnosed with celiac disease were retrospectively screened by evaluating their celiac serology and other laboratory parameters, anthropometric measurements, age-specific annual growth velocities, duration of follow-up, and by comparing those who adhered and those who did not adhere to a gluten-free diet. Patients with Down syndrome, Turner syndrome, Williams syndrome, cerebral palsy, IgA deficiency, those lost to follow-up, and those with incomplete anthropometric or laboratory data were excluded. A total of 357 patients aged between 1 and 18 years were included in the study. The cases were divided into three age groups (1–5, 6–11, and 12–18 years), and growth parameters and annual growth velocities at diagnosis and at the 1-year follow-up were compared between adherent and non-adherent patients. In addition, in all cases, celiac serology, laboratory tests, and vitamin levels were analyzed at baseline and at the 1-year follow-up and compared between the two groups. Results: In the retrospective screening of 733 patients, a total of 357 children (62.5% female, 37.5% male) aged 12–199 months who met the appropriate criteria were included in the study. The mean age was 104.2 ± 47.6 months (8.7 ± 3.9 years), and the median age was 105 months (8.75 years). Of the cases, 26.6% were in the 1– 5 year age group, 50.7% in the 6–11 year group, and 22.7% in the 12–18 year group. The most common reasons for admission were abdominal pain (44.8%), diarrhea (17.1%), and failure to gain weight (15.4%), followed by constipation, short stature, screening, loss of appetite, refractory anemia, vomiting, and nausea. Comorbidities were detected in 22.4% of the cases; the most common were Type 1 DM (9.8%) and anemia (6.2%). Of the patients, 60.8% were compliant with the diet and 39.2% were non-compliant; while there was no difference between genders, the compliance rate in the 1–5 year group (69.5%) was significantly higher compared to adolescents (50.6%). When evaluated in terms of dietary compliance, the annual growth rates of diet- compliant cases were statistically significantly higher than those of the non-compliant group in all three age groups (1–5 years p=0.019; 6–11 years p=0.019; 12–18 years p=0.019; p<0.05). However, the greatest increase in growth velocity was observed in the 1–5 year group, while the lowest increase was seen in the 12–18 year group. In the compliant group, the mean height, height SDS, and height percentile values were significantly higher compared to the non-compliant group in all age categories (1–5 years height p=0.001; height SDS p=0.001; height percentile p=0.001; 6–11 years height p=0.001; height SDS p=0.005; height percentile p=0.006; 12–18 years height p=0.001; height SDS p=0.001; height percentile p=0.001; all values p<0.05). Similarly, the mean weight, weight SDS, and weight percentile values of the diet-compliant children were significantly higher in all age groups compared to the non-compliant group (1–5 years weight p=0.019; weight SDS p=0.001; weight percentile p=0.001; 6–11 years weight p=0.001; weight SDS p=0.001; weight percentile p=0.001; 12–18 years weight p=0.001; weight SDS p=0.001; weight percentile p=0.001; all values p<0.05). The mean BMI, BMI SDS, and BMI percentile values of compliant children were also significantly higher in all age groups compared to non-compliant children (1–5 years BMI p=0.003; BMI SDS p=0.004; BMI percentile p=0.004; 6–11 years BMI p=0.007; BMI SDS p=0.002; BMI percentile p=0.002; 12–18 years BMI p=0.001; BMI SDS p=0.035; BMI percentile p=0.001; all values p<0.05). In addition, significant improvements were observed in hemoglobin, MCV, iron, ferritin, folate, vitamin B12, and vitamin D levels. Serologically, while tTG-IgA levels decreased in the compliant group and EMA became negative in the majority of cases, EMA positivity persisted in 95% of the non-compliant group. Discussion and Conclusion: In this study, children adhering to a gluten-free diet demonstrated significant improvements in growth parameters and laboratory findings. Adherence rates were higher at younger ages, accompanied by marked increases in annual growth velocity. The higher compliance in younger children highlights the need for education, family counseling, and close monitoring, especially during adolescence, to enhance adherence. In adherent children, increases in height, weight, and BMI were evident both in absolute and SDS-percentile terms, while non- adherent adolescents showed regression. Hematologically, significant improvements in hemoglobin, MCV, iron, ferritin, folate, vitamin B12, and vitamin D were observed in the adherent group. Serological improvements, including decreased tTG-IgA levels and EMA negativity, were consistent with mucosal healing. These findings demonstrate that strict adherence to a gluten-free diet provides substantial benefits for growth, nutrition, and inflammatory processes.
