Özet
Amaç: Pes planovalgus, çoğunlukla çocukluk döneminde esnek düztabanlık olarak görülen ve yetişkin döneminde de genellikle sert düztabanlık olarak sık rastlanan bir deformitedir. Özellikle çocukluk çağında ligaman laksitesi nedeniyle veya konjenital ayak deformitesi nedeniyle görülmektedir. Yetişkin dönemde esneklikten ziyade ayak kemik yapısı ve ligaman yapısında sertleşme nedeniyle sert düztabanlık olarak adlandırılmaktadır. Esnek pes planovalgus deformitesine sahip hastalarda cerrahi karar alındıktan sonra hangi yöntemin hangi implantla veya osteotomiyle yapılması gerektiğini belirlemek elzemdir. Subtalar vida veya kalix ile artroerezis yöntemi, Evans osteotomisi (lateral kalkaneal uzatma) yöntemi cerrahi seçenekler arasındadır. Bu üç cerrahi yöntemin de beraber değerlendirilerek elde edilecek sonuçlar esnek pes planovalgus cerrahisinde ortopedistler için cerrahi seçeneklerin karşılaştırılmasına yardımcı olacak ve ileri dönemlerde oluşacak olan artroz riskinin saptanmasında katkısı olacaktır. Gereç ve Yöntem: Çalışmamızda 9 kadavrada pes planovalgus deformitesi oluşturulup ardından cerrahi yöntemler uygulandıktan sonra tibiotalar, subtalar, kalkaneoküboid eklemlerde gelişen basınç değişimleri belirlenmiştir. Basınç değişimlerinin bir veya iki eklemde fazla oranda artmış olması ileri yaşlarda o eklem veya eklemlerdeki artroz gelişim riskini arttıracaktır. Çalışmamızda elde edilen sonuçlar sayesinde hangi ekleme hangi implant veya osteotomi yapılması gerektiği sonucuna varılmıştır. Çalışmanın özgül değeri literatürde rastlanılmayan şekilde hem kalkaneal osteotomi ve artroerezis yöntemi karşılaştırması hem de artroereziste kullanılan kalix ve vidanın karşılaştırılmış olmasıdır. Bulgular: 9 adet kadavra modelinde 3 eklemdeki normal (preop), pes planovalgus oluştuktan sonra, kalix yöntemi, vida yöntemi ve Evans osteotomisi uygulandıktan sonra basınçlar ayrı ayrı kaydedildi. Çalışmada öncelikle, PPV biyomekanik modeli oluşturulması öncesi ve sonrasında çeşitli eklem bölgelerindeki (tibiotalar, subtalar, kalkaneoküboid) basınç değerlerindeki değişim değerlendirilmiştir. Analizler, PPV modelini oluşturma işlemi öncesi VIII (preoperatif) ve sonrasında (postoperatif) eklemlerde basınç dağılımı üzerinde anlamlı değişiklikler olduğunu göstermiştir. PPV sonrası uygulanan cerrahi tekniklerin eklem içi basınç dağılımı üzerinde bölgesel olarak farklı etkileri olduğunu göstermektedir. Anterior tibiotalar eklemde kalix ile artroeresis tekniğinin basıncı azaltmada vida ile artroeresise göre nispeten daha etkili olduğu (%24'e karşı %20), posterior tibiotalar eklemde ise vida ile artroereisis ve Evans osteotomisi tekniklerinin basıncı artırmaya kalix ile artroeresisten belirgin şekilde daha fazla sebep olduğu (%41'e karşı %29) görülmüştür. En çarpıcı sonuçlar kalkaneoküboid eklemde elde edilmiş olup, Evans osteotomisi tekniğinin bu eklemdeki basıncı belirgin şekilde artırdığı (%29), vida ile artroeresis tekniğinin nötr kaldığı (%0), kalix ile artroeresis tekniğinin ise azalttığı (%29) tespit edilmiştir. Sonuç: Eklem yüzlerinde olan basınç artışlarının, ileri dönemde artroz riskini arttırmış olacağından, anterior tibiotalar eklemde risk olmadığı, posterior tibiotalar eklemde Evans osteotomisi ve vida ile artroeresis yönteminin kalix ile artroeresis yöntemine göre daha riskli olduğu ve bu yüzden kalix ile artroeresis yönteminin daha güvenli olduğu, medial subtalar eklemde risk olmadığı, lateral subtalar eklemde tüm cerrahi yöntemlerde benzer risk olduğu, kalkaneoküboid eklemde ise Evans osteotomisinin diğer cerrahi yöntemlere göre artroz riskinin daha fazla olduğu ve kalix veya vida ile artroeresis yönteminin daha güvenli olduğu sonucuna, varılmıştır.

Objective: Pes planovalgus is a deformity most commonly seen as flexible flatfoot during childhood and as rigid flatfoot in adulthood. In childhood, it usually occurs due to ligamentous laxity or congenital foot deformities. In adulthood, rather than flexibility, it is referred to as rigid flatfoot because of the stiffening of the bony and ligamentous structures of the foot. In patients with flexible pes planovalgus deformity, once the surgical decision is made, it is essential to determine which method should be performed with which implant or osteotomy. Subtalar arthroereisis with a screw or a Kalix implant, and Evans osteotomy (lateral calcaneal lengthening) are among the surgical options. Evaluating these three surgical methods together and analyzing the outcomes will help orthopedic surgeons compare surgical alternatives in flexible pes planovalgus surgery and will also contribute to identifying the risk of future osteoarthritis. Materials and Methods: In our study, pes planovalgus deformity was created in 9 cadavers, and after the application of surgical methods, the pressure changes that developed in the tibiotalar, subtalar, and calcaneocuboid joints were determined. A significant increase in pressure in one or two joints would increase the risk of developing osteoarthritis in those joints in later years. Based on the results obtained in our study, conclusions were drawn regarding which implant or osteotomy should be applied to which joint. The specific value of the study lies in the fact that, unlike in the existing literature, it compares both calcaneal osteotomy and arthroereisis methods, as well as the comparison between the Kalix implant and screw used in arthroereisis. Results: In nine cadaveric models, pressures across three joints were measured under the following conditions: normal (preoperative), after induction of pes planovalgus, and following the application of the Kalix method, the screw technique, and Evans osteotomy. The study primarily aimed to evaluate changes in pressure values in different joint regions (tibiotalar, subtalar, calcaneocuboid) before and after the establishment of the PPV biomechanical model. The analyses revealed significant alterations in intra-articular pressure distribution both preoperatively and postoperatively. Furthermore, the surgical techniques applied following PPV demonstrated region-specific differences in their effects on joint pressure. In the anterior tibiotalar joint, the Kalix and arthroereisis techniques were relatively more effective in reducing pressure compared with screw arthroereisis (24% vs. 20%). In the posterior tibiotalar joint, screw arthroereisis and Evans osteotomy resulted in a more pronounced increase in pressure than Kalix arthroereisis (41% vs. 29%). The most striking results were observed in the calcaneocuboid joint: Evans osteotomy substantially increased pressure (29%), screw arthroereisis maintained a neutral effect (0%), while Kalix arthroereisis produced a marked reduction (29%). Conclusion: Increased pressure on the articular surfaces is anticipated to elevate the long-term risk of osteoarthritis. The findings of this study indicate that no risk was observed in the anterior tibiotalar joint. In contrast, within the posterior tibiotalar joint, Evans osteotomy and screw arthroereisis were associated with a higher risk compared to the Kalix arthroereisis method, suggesting that the latter represents a safer surgical option. No risk was identified in the medial subtalar joint, whereas all surgical techniques demonstrated comparable levels of risk in the lateral subtalar joint. In the calcaneocuboid joint, Evans osteotomy was found to carry a greater risk of osteoarthritis relative to the other surgical interventions, while Kalix and screw arthroereisis appeared to be safer alternatives.
