Özet
Bu çalışmanın amacı, ASA (American Society of Anesthesiologists) Fiziksel Durum Sınıflandırma Sistemi'nin hastane anestezi maliyetleri üzerinde etkisini, özel bir sağlık grubuna ait hastaneler örneğinde incelemektir. Çalışma, grubun İstanbul'da Anadolu ve Avrupa yakasında yer alan ve en yüksek cerrahi vaka sayısına sahip iki hastanesinde, 01 Ocak 2022 ile 31 Aralık 2022 tarihleri arasında lomber disk hernisi, rinoplasti, kolesistektomi ve ortopedik kalça artroplastisi ameliyatı olan hastaların kayıtlarının retrospektif analizi ile gerçekleştirilmiştir. Çalışmaya, ASA 1-4 skoruna sahip olan ve genel anestezi altında elektif olarak belirtilen cerrahi işlemlerden birini geçiren hastalar dahil edilmiştir. Veriler, hastane bilgi yönetim sistemi aracılığıyla elde edilmiş; doğrudan anestezi maliyetleri ise kullanılan anestezik ilaçlar ve sarf malzemeleri dikkate alınarak hesaplanmıştır. Analiz sonuçları, ASA skorundaki artış ile birlikte hasta başına düşen anestezi maliyetlerinin istatistiksel olarak anlamlı şekilde arttığını göstermektedir. Özellikle ASA 3 skoruna sahip hastaların maliyetlerinin, ASA 1 ve ASA 2 skoruna sahip hastalara göre kıyasla belirgin biçimde daha yüksek olduğu saptanmıştır. Bu yönüyle çalışma, özel hastanelerde anestezi birimi özelinde gerçekleştirilen nadir maliyet çalışmalarından biri olma özelliği taşımakta ve risk sınıflandırmasının maliyet planlamasındaki belirleyici rolünü ortaya koyarak hem yönetsel karar süreçlerine, hem de sağlık politikalarına veri temelli katkı sunmaktadır.

The aim of this study is to examine the impact of the ASA (American Society of Anesthesiologists) Physical Status Classification System on hospital anesthesia costs, using hospitals affiliated with a private healthcare group as the basis for analysis. The study was conducted through a retrospective analysis of patient records from two hospitals, located on the Anatolian and European sides of Istanbul, which had the highest number of surgical cases within the group. The data cover the period between January 1 and December 31, 2022, and include patients who underwent elective lumbar disc herniation surgery, rhinoplasty, cholecystectomy, and orthopedic hip arthroplasty. The study included patients with ASA scores ranging from I to IV who underwent specified procedures under general anesthesia. Data were obtained through the hospital information management system, and direct anesthesia costs were calculated based on anesthetic drugs and consumables. The findings indicate that anesthesia costs per patient increased with higher ASA scores, and that increase is statistically significant. In particular, patients with ASA score III incurred considerably higher costs compared to patients with ASA scores I and II. In this respect, this study is one of the few cost studies conducted specifically on anesthesia units in private hospitals. It highlights the critical role of risk classification in cost planning, and offers data-driven insights to support both administrative decision-making and health policy development.
