Özet
ÖZET Amaç: Bu çalışmanın amacı, artroskopik rotator manşet tamirlerinde (RMT) biseps tendon augmentasyonu tekniği ile transosseöz eşdeğer (TOE) tekniğinin fonksiyonel sonuçlarını karşılaştırmaktır. Gereç ve Yöntem: Ocak 2022–Aralık 2024 tarihleri arasında kliniğimizde tam kat rotator manşet yırtığı nedeniyle artroskopik onarım uygulanan 40 hasta retrospektif olarak incelendi. Hastalar düğümlü TOE (n=20) ve biseps tendon augmentasyonu yapılan TOE (n=20) olarak iki gruba ayrıldı. Dahil edilme kriterleri 18–75 yaş aralığı, goutallier evre ≤3 ve en az 6 ay süren semptom varlığı olarak belirlendi. Postoperatif 1, 6. ve 12. aylarda ASES, UCLASS, Quick-DASH ve Modifiye Constant skorları değerlendirildi. Bulgular: BA grubunun ortalama yaşı 63,6±7,8; TOE grubunun 64,2±8,0 idi (p>0,05). 6. ayda her iki grupta da ASES, UCLASS, Quick-DASH ve Constant skorlarında anlamlı artış saptandı (p<0,001). Quick-DASH skoru TOE grubunda daha düşük bulundu (p=0,033). BA yapılan teknikte de komplikasyon, rüptür veya Popeye deformitesi gözlenmezken diğer grupta 1 hasta rüptür izlendi. Sonuç:Biseps tendon augmentasyonu yapılan rotator manşet onarımı, klasik TOE tekniğiyle benzer fonksiyonel sonuçlar göstermektedir. Cerrahi başarıyı hasta özellikleri, yırtık tipi ve cerrahın deneyimi belirlemektedir. Biseps augmentasyonu, seçilmiş hastalarda güvenli ve etkin bir alternatif olarak değerlendirilebilir.

ABSTRACT Objective: The objective of this study is to compare the functional outcomes of the biceps tendon augmentation technique with the transosseous equivalent (TOE) technique in arthroscopic rotator manşet repairs (RCR). Methods:A retrospective review was conducted of forty patients who underwent arthroscopic repair for full-thickness rotator manşet tears at our clinic between January 2022 and December 2024. Patients were divided into two groups: knotted TOE (n = 20) and TOE with biceps tendon augmentation (n = 20). The inclusion criteria were delineated as follows: age range of 18–75 years, Goutallier stage ≤3, and the presence of symptoms that have persisted for a minimum duration of 6 months. The ASES, UCLASS, Quick-DASH, and Modified Constant scores were evaluated at 2 and 1, and 6 and 12 months postoperatively. Results: The mean age of the BA group was 63.6±7.8, while that of the TOE group was 64.2±8.0 (p>0.05). At the six-month mark, a substantial enhancement was evident in the ASES, UCLASS, Quick-DASH, and Constant scores across both groups (p0.001). The Quick-DASH score was found to be lower in the TOE group (p=0.033). No complications, rupture, or Popeye deformity were observed in the biceps augmentation group, while a rupture was identified in one patient in the other group. Conclusion:The functional outcomes of rotator manşet repair with Biseps tendon augmentation are comparable to those of the conventional TOE technique. The degree of surgical success is contingent upon patient characteristics, the type of tear, and the surgeon's level of expertise. Biseps augmentation can be regarded as a safe and effective alternative for specific patients.
