Özet

Giriş: Multiple skleroz (MS) hastaları hastalık aktivasyonu, psödoatak veya MS ile ilişkisiz olan çeşitli nedenlerle acil servise başvurmaktadır. Bu hasta grubunun ayırıcı tanısı acil hekimleri ve nörologlar için oldukça zor olmaktadır. Bu araştırmada, değerlendirme sürecine katkı sağlamak amacıyla MS atağındaki hastaların ayırt edici özelliklerinin araştırılması hedeflendi. Yöntem: Acil serviste 01.01.2014 – 31.12.2023 tarihleri arasında MS atağı ön tanısıyla değerlendirilen 18 yaş ve üzeri hastalar hastane otomasyon sistemi üzerinden tarandı. Kontrol grubu çalışma evrenindeki hastalarla benzer özelliklere sahip olan izole baş ağrısı şikayetiyle nöroloji polikliniğine başvuran hastalar arasından random olarak seçildi. MS ön tanılı hastalara ait başvuru şikayetleri, muayene bulguları, laboratuvar bulguları ve santral görüntüleme bulguları kaydedildi. Çalışma sonunda MS atağındaki hastalar ile inaktif MS (Psödoatak) hastaları ve kontrol grubundaki hastalara ait parametreler karşılaştırıldı. Bulgular: Çalışma periyodunda acil serviste MS atağı ön tanısıyla değerlendirilen hasta sayısı 378 idi. Bunlardan MS atağı tanısı alanların sayısı 148 (%39,1) olarak belirlendi. MS tanısı olup klinik değerlendirme sonucunda MS atağında olmadığı tespit edilen inaktif (psödoatak) olguların sayısı 129 (%34,1) idi. Tespit edilen diğer en sık tanılar nonspesifik kas eklem ağrıları (n=29, %7.7) ve konversiyon bozukluğuydu (n=15, %4). İncelenen tüm hasta gruplarında kadın cinsiyet oranı belirgin şekilde yüksek saptandı (p<0,05). Aktif ve inaktif hasta grupları arasında yaş ve cinsiyet parametreleri açısından istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Acil servis başvurusunda ilk MS atağı tespit edilen hasta sayısı 18 (%12.2) idi. MS atak hastalarında en sık saptanan MR bulgusu kontrast tutan lezyonlar (n:96, %64.4) idi. MS atağındaki hastaların acil servise en sık başvuru şikayetleri duyu defisiti 70 (%47.2), motor defisit 43 (%29), görme bozukluğu 36 (%24.3) ve baş dönmesi / dengesizlik 25 (%16.8) olarak, en sık muayene bulguları da duyu defisiti (n=60,%40.5)motor defisit (47=, %31.7) ve nistagmus (n=25 %16.9) iken 35 (%23.6) hastada anlamlı muayene bulgusu saptanmadı. MS atağında olan hasta grubu ile inaktif (psödoatak) hasta grubu arasında başvuru şikayetleri ve muayene bulguları açısından istatistiksel olarak anlamlı fark saptanmadı (p<0,05). Sonuç: Acil servis şartlarında MS hastalık aktivitesini belirlemede yaş, cinsiyet, başvuru şikayetleri ve fizik muayene bulguları yeterli değildir. MR görüntüleme bulgularına ek olarak laboratuvar parametrelerinden lenfosit ve platelet sayısı, nötrofil lenfosit oranı ve CRP düzeyi MS atağını tespit etmede yol gösterici olabilir.

Introduction: Patients with Multiple Sclerosis (MS) visit the emergency department due to disease activation, pseudoattacks, or various causes unrelated to MS. Differential diagnosis of this patient group is quite difficult for emergency physicians and neurologists. In this study, we aimed to investigate the distinguishing features of patients with MS attacks in order to contribute to the evaluation process. Method: Patientsaged 18 years and older who were evaluated with a preliminary diagnosis of MS attack in the emergency department between January 1, 2014, and December 31, 2023, were screened through the hospital automation system. The control group was randomly selected from patients presenting to the neurology out patient clinic with isolated headache complaints who had similar characteristics to the study population. The presenting complaints, examination findings, laboratory findings, and central imaging findings of patients with a preliminary diagnosis of MS were recorded. At the end of the study, the parameters of patients with MS attacks were compared with those of patients with inactive MS (pseudoattack) and the control group. Findings: During the study period, 378 patients were evaluated in the emergency department with a preliminary diagnosis of MS attack. Of these, 148 (39.1%) were diagnosed with MS attack. The number of inactive (pseudoattack) cases with MS diagnosis but no MS attack based on clinical evaluation was 129 (34.1%). The other most common diagnoses identified were nonspecific muscle and joint pain (n=29, 7.7%) and conversion disorder (n=15, 4%). The female gender ratio was significantly higher in all patient group sexamined (p<0.05). No statistically significant differences were found between the active and inactive patient groups in terms of age and gender parameters (p>0.05). The number of patients with their first MS attack detected in the emergency department was 18 (12.2%). The most common MRI finding in MS attack patients was contrast-enhancinglesions (n=96, 64.4%). The most common complaints of patients with MS attack spresenting to the emergency department were sensory deficit (70, 47.2%), motor deficit (43, 29%), visual impairment (36, 24.3%), and dizziness/imbalance (25, 16.8%). The most common physical examination findings were sensory deficit (n=60, 40.5%), motor deficit (n=47, 31.7%), and nystagmus (n=25, 16.9%), while no significant physicalexamination findings were detected in 35 (23.6%) patients. No statistically significant difference was found between the group of patients with an MS attack and the group of patients with an inactive (pseudoattack) episode in terms of presenting complaints and examination findings (p<0.05). Conclusion: Age, gender, presentingcomplaints, and physical examination finding sare insufficient for determining MS diseaseactivity in emergency department settings. Inaddition to MRI findings, laboratory parameters such as lymphocyteand platelet counts, neutrophil-lymphocyteratio, and CRP levelsmay be helpful in detecting MS attacks.
