Özet

Amaç: Bu çalışmada şizofreni tanılı bireylerin klinik ve sosyodemografik özelliklerinin; saldırganlık ve dürtüsellik düzeyleri ile ilişkisini değerlendirmek, ayrıca suç öyküsünün bu değişkenler üzerindeki etkisini incelemeyi hedeflenmiştir. Yöntem: Çalışmamız tanımlayıcı ve kesitsel bir çalışmadır. Bu çalışmaya SBÜ Beyhekim Eğitim ve Araştırma Hastanesi Adli Psikiyatri polikliniklerine ve yataklı servislerine takip ve tedavi amacıyla başvuran 50 erkek, 40 kadın olmak üzere DSM-5 tanı kriterlerine uygun 90 şizofreni tanılı hasta dahil edilmiştir. Çalışmamız suç işlemiş şizofreni hastaları ve suç işlememiş şizofreni hastaları olarak 2 gruba ayrılmıştır. Hastalara sosyodemografik veri formu, Pozitif ve Negatif Sendrom Ölçeği (PANSS), Morisky İlaç Uyum Ölçeği (MİUÖ), Barratt Dürtüsellik Ölçeği (BDÖ), İçgörünün Üç Bileşeni Ölçeği (İÜBDÖ), Buss-Perry Saldırganlık Ölçeği (BPSÖ), Taylor Suç Şiddeti Derecelendirme Ölçeği (TSŞÖ), Algılanan Aile Desteği Ölçeği (AADÖ) ve Klinik Global İzlem Ölçeği (KGİÖ) uygulanmıştır. Bulgular: Elde edilen bulgular, suç öyküsüne sahip şizofreni hastalarının suç işlememiş şizofreni hasta grubuna göre Morisky İlaç Uyum Ölçeği puanın (p=0,002), İçgörünün Üç Bileşeni Ölçeği puanının (p=0,001) ve Algılanan Aile Desteği Ölçeği puanının (p=0,001) istatistiksel olarak anlamlı derecede daha düşük olduğu, Barrat Dürtüsellik Ölçeği toplam puanının (p=0,001) ve alt boyutlarının Motor Dürtüsellik (p=0,002), Plan Yapamama (p=0,010), Dikkatsizlik (p=0,001), Buss-Perry Saldırganlık toplam puanının (p=0,001) ve alt boyutlarının Fiziksel Agresyon (p=0,005), Sözel Agresyon (p=0,006), Öfke (p=0,001), Düşmanlık (p=0,030), PANNS toplam puanı (p=0,001), PANNS Pozitif belirti puanı (p=0,001), PANNS Genel psikopatoloji puanın (p=0,001) ve KGİÖ-Hastalık Şiddeti puanının (p=0,049) ise istatistiksel olarak anlamlı derecede daha yüksek olduğu sonucuna ulaşılmıştır. Sonuç: Çalışmamızda elde ettiğimiz bulgular neticesinde, şizofreni hastalarında, suç davranışının ortaya çıkmasında; tedaviye uyumun, içgörünün ve aile desteğinin belirleyici olduğunu ayrıca suç davranışının görülmesinde sosyodemografik özellikler, yüksek dürtüsellik düzeyleri, yüksek saldırganlık düzeyleri ve psikotik belirtilerin varlığı ile ilişkili olduğu sonucuna ulaşılmıştır.

Objective: The present study aimed to evaluate the relationship between the clinical and sociodemographic characteristics of individuals diagnosed with schizophrenia and their levels of aggression and impulsivity, as well as to examine the impact of a history of criminal behavior on these variables. Method: The present study was designed as a descriptive and cross-sectional study. A total of 90 patients diagnosed with schizophrenia according to DSM-5 diagnostic criteria were included in the study, comprising 50 male and 40 female participants who were admitted to the Forensic Psychiatry outpatient clinics and inpatient units of SBÜ Beyhekim Training and Research Hospital for follow-up and treatment. participants were divided into two groups based on their criminal background: schizophrenic patients with a history of criminal behavior and schizophrenic patients without such a history. All participants were assessed using a comprehensive battery of instruments, including the Sociodemographic Data Form, the Positive and Negative Syndrome Scale (PANSS), the Morisky Medication Adherence Scale (MMAS), the Barratt Impulsiveness Scale (BIS-11), the Insight Scale for the Three Components of Insight (IS-3CI), the Buss-Perry Aggression Questionnaire (BPAQ), the Taylor Crime Severity Rating Scale (TCSRS), the Perceived Family Support Scale (PFSS), and the Clinical Global Impression Scale (CGI). This multidimensional assessment approach allowed for the examination of both clinical symptom severity and psychosocial functioning, facilitating a comprehensive analysis of the relationships between impulsivity, aggression, insight, medication adherence, and family support, and their potential associations with criminal behavior in schizophrenia. Results: The findings of the present study revealed that, compared to the group of schizophrenic patients without a history of criminal behavior, patients with a criminal history exhibited significantly lower scores on several key psychosocial and clinical measures. Specifically, these patients demonstrated lower scores on the Morisky Medication Adherence Scale (p = 0.002), the Insight Scale for the Three Components of Insight (p = 0.001), and the Perceived Family Support Scale (p = 0.001), indicating poorer treatment adherence, diminished illness awareness, and weaker perceived familial support, respectively. Conversely, the same group obtained significantly higher total scores on the Barratt Impulsiveness Scale (p = 0.001), as well as on its subdimensions—Motor Impulsivity (p = 0.002), Non-Planning Impulsivity (p = 0.010), and Attentional Impulsivity (p = 0.001) reflecting greater impulsivity across multiple behavioral domains. Similarly, Buss-Perry Aggression Questionnaire results revealed higher total aggression scores (p = 0.001) and significantly elevated subscale scores for Physical Aggression (p = 0.005), Verbal Aggression (p = 0.006), Anger (p = 0.001), and Hostility (p = 0.030). Furthermore, the Positive and Negative Syndrome Scale (PANSS) total score (p = 0.001), PANSS Positive Symptom subscale (p = 0.001), and PANSS General Psychopathology subscale (p = 0.001) were all significantly higher in the criminal group, indicating a greater severity of psychotic symptoms. The Clinical Global Impression–Severity (CGI-S) score was also significantly elevated (p = 0.049), reflecting higher overall illness severity. Taken together, these results suggest that schizophrenic patients with a history of criminal behavior are characterized by poorer treatment adherence, reduced insight, lower family support, and heightened levels of impulsivity, aggression, and psychopathology, all of which may collectively contribute to the emergence and persistence of criminal behaviors within this population. Conclusion: Based on the findings obtained in the present study, it was concluded that treatment adherence, level of insight, and perceived family support play a determinant role in the emergence of criminal behavior among individuals diagnosed with schizophrenia. Furthermore, the occurrence of such behavior was found to be significantly associated with sociodemographic characteristics, elevated levels of impulsivity, increased aggression, and the presence of active psychotic symptoms. These results suggest that criminal behavior in schizophrenia cannot be attributed to a single causal factor but rather emerges from a complex interplay between clinical, psychological, and social variables. Accordingly, interventions aiming to enhance treatment compliance, improve insight, reduce impulsivity and aggression, and strengthen familial and social support systems may play a crucial role in mitigating the risk of criminal conduct within this clinical population.
