Özet
Giriş ve Amaç: Sepsis, enfeksiyona sekonder gelişip hayatı tehdit eden organ disfonksiyonu ile seyreden, yüksek morbidite ve mortalite oranları olan bir sendromdur. Herhangi bir altın standart tanı testinin olmadığı bilinmekte olup hastaların sepsise progresyonunu öngörmede skorlama sistemlerinden faydalanmak gün geçtikçe artan bir öneme sahip olmuştur. Erken Uyarı Skorları (EWS), acil serviste, hastanede yatışı sırasında veya yoğun bakım ünitesinde hastaların akut klinik kötüleşmelerini öngörmek amacıyla geliştirilmiştir. Ulusal Erken Uyarı Skoru (NEWS) vital parametrelerin değerlendirilmesine dayanmakta olup, NEWS ile bir laboratuvar unsurunu (serum laktat düzeyi) bir araya getiren NEWS-L yaygın olarak kullanılan skorlardır. SPEED skoru ise, cil servis gibi kısıtlı şartlarda, septik hastaların prognozlarını öngörmek adına, malignite ve immünosüpresif durum varlığı gibi bağışıklık sistemini doğrudan etkileyen, serum laktat düzeyini dahil ederek skorlamayı görece sepsise yönelik sınırlandıran, acil servis şartlarına yönelik geliştirilen bir skorlama sistemidir. Biz de çalışmamızda sepsis ve septik şok ile acil servise başvuran geriatrik hastalarda SPEED, NEWS ve NEWS-L skorlama sistemlerinin 28 günlük sağkalımı tahmin etmedeki prognostik performansını değerlendirmeyi amaçlamaktayız. Gereç ve Yöntem: Çalışmamız, tek merkezli, prospektif, gözlemsel bir tanısal değerlendirme çalışmasıdır. Hastanemiz acil servisine, 15.11.2023 - 15.11.2024 arasında başvurusunda Sepsis 3.0 tanı kriterlerine göre sepsis ve septik şok tanısı alan geriatrik hastalar (65 yaş ve üstü erişkinler) çalışmaya dahil edildi. Kendisi veya yakını çalışmaya katılmayı reddeden hastalar; hastanın kendisinden ve yakınından acil servis sonrası takibine dair sonlanım (28 günlük mortalite) bilgisi alınamamış hastalar; eşlik eden travma öyküsü olan hastalar çalışmaya dahil edilmedi. Hastalarımızın SOFA, SPEED, NEWS ve NEWS-L skorları hesaplanarak 28 günlük süreçte mortal seyreden ve sağ kalan hastalar istatistiksel olarak incelendi. Bulgular: Hastanemize 15.11.2023 - 15.11.2024 tarihleri arasında başvuran sepsis ve septik şok tanısı ile takip edildiği tespit edilen 501 hastadan dahil etme kriterlerini karşılayan 330'unun verileri çalışma kapsamında analiz edildi. Çalışmamıza katılan hastaların %53,9'u kadın (n=178), %46,1'i (n=152) erkekti. Yaş ortancası ise 79 (72– 84) yıldı. Çalışmaya dahil edilen hastalarda SPEED skoru ile NEWS, NEWS-L gibi erken uyarı sistemleri ile birlikte SOFA skorunun 28 günlük mortalite ve sağkalım durumundaki değerleri hesaplandı. SPEED skoru 28 günlük mortalitesi olan hastalarda 8 (6–10), SOFA skoru 6 (4–8), NEWS 15 (12–16), NEWS-L 17 (13–19) görülmüş olup; sağ kalanlarda ise SPEED skoru 6 (5–9), SOFA skoru 5 (4–6), NEWS 13 (8–15), NEWS-L 14 (10–16.0) görülmüştür. SPEED, NEWS ve NEWS-L skorlama sistemleri ile birlikte SOFA skorunun birincil sonlanım olan 28 günlük mortalite üzerine istatistiksel anlamlı fark yaratmadığı, ROC analizi sonuçlarına göre SOFA skorunda 0,644, SPEED skorunda AUC 0,653, NEWS skorunda 0,679, NEWS-L skorunda 0,694 hesaplan AUC değerleri baz alındığında, 28 günlük mortalite açısından benzer öngörü yeterliliğine sahip oldukları görülmüştür. İkincil sonlanım açısından ise acil serviste mekanik ventilasyon gerekliliği, CRP, malignite, vazopressör ihtiyacı ve LDH mortaliteyi anlamlı şekilde öngören değişkenler olarak saptanmıştır. Sonuç: Çalışmamızda, ROC eğrileri altında kalan alan açısından anlamlı bir fark olmadığı, ancak SPEED skorunun mortalite öngörmeye yönelik diğer skorlama sistemlerine benzer öngörü kabiliyeti olduğu tespit edildi. Mevcut literatürde SPEED skorunun değerlendirilmesine yönelik yapılan çalışmalar halihazırda kısıtlı olduğundan acil servisleri kapsayarak yapılacak yeni çalışmalara ihtiyaç vardır. Bu bağlamda bilhassa acil servis takip sürecine entegre yeni skorlama sistemlerinin geliştirilmesini önermekteyiz.

Introduction and Objective: Sepsis is a syndrome that develops in response to infection and is characterized by life-threatening organ dysfunction, and has high morbidity and mortality rates. It is well known that no gold standard diagnostic test is available, and the benefit of using scoring systems in predicting disease progression has become crucial. Early Warning Scores (EWS) were developed to predict acute clinical deterioration of patients in the emergency department, during hospitalization, or in the intensive care unit. The National Early Warning Score (NEWS), based on the assessment of vital parameters, and NEWS-L, combining NEWS with a laboratory component (serum lactate level), are widely used scores. In order to predict the prognosis of septic patients in limited circumstances such as the emergency department, the SPEED score is a scoring system developed for emergency department conditions and aims to assess the likelihood of sepsis by involving factors that directly affect the immune system, such as the presence of malignancy and immunosuppression, and serum lactate levels. In our study, we aim to evaluate the prognostic performance of the SPEED, NEWS, and NEWS-L scoring systems in predicting 28-day survival in geriatric patients presenting to the emergency department with sepsis and septic shock. Materials and Method: Our study is a single-centered, prospective, observational diagnostic acurracy study. Geriatric patients (adults aged 65 years and older) diagnosed with sepsis and septic shock according to the Sepsis 3.0 diagnostic criteria and presented to our emergency department between November 15, 2023, and November 15, 2024, were included in the study. We excluded the patients who refused to participate in the study themselves or whose relatives refused; patients that we could not obtain information about the outcome (28-day mortality) after the emergency department visit; and patients with a history of concomitant trauma. The SOFA, SPEED, NEWS, and NEWS-L scores of our patients were evaluated, and patients who died or survived within the 28-day period were statistically analyzed. Findings: Data from 330 out of 501 patients diagnosed with sepsis and septic shock who were followed up at our hospital between November 15, 2023, and November 15, 2024, and who met the inclusion criteria were analyzed in this study. Of the patients participating in our study, 53.9% were female (n=178) and 46.1% were male (n=152). The SPEED score and early warning systems such as NEWS and NEWS-L were used to assess the 28-day mortality and survival rates in patients included in our study. Among patients with a 28-day mortality rate, the SPEED score was 8 (6–10), the SOFA score was 6 (4–8), the NEWS score was 15 (12–16), NEWS-L 17 (13–19); in survivors, SPEED score 6 (5–9), SOFA score 5 (4–6), NEWS 13 (8–15), NEWS-L 14 (10–16.0) were noted. The SPEED, NEWS, and NEWS-L scoring systems did not significantly affect the primary outcome that was 28-day mortality and according to the results of the ROC analysis, the AUC values were 0.644 for the SOFA score, 0.653 for the SPEED score, 0.679 for the NEWS score, 0.694 for the NEWS-L score. In terms of 28-day mortality, they were found to have similar predictive adequacy. In terms of secondary outcome, mechanical ventilation requirement in the emergency department, vasopressor requirement, malignancy, CRP, and LDH were identified as variables that significantly predicted mortality. Conclusion: In our study, no significant difference was found as to the area under the ROC curves; however, the SPEED score was found to have predictive ability similar to other scoring systems for predicting mortality. Since studies evaluating the SPEED score are currently limited in the current available literature, new studies are needed to be carried out to involve emergency departments. In this context, we recommend the further development of new scoring systems, especially those integrated into the emergency department follow-up process.
