Özet
Depremler, yaşandığı bölgelerde zor şartlar altında görev yapan hemşirelerde travma, depresyon, stres, duygu durum bozukluğu, umutsuzluk, çaresizlik, kaygı, korku, hayata küsme gibi farklı psikolojik rahatsızlıkların oluşumuna neden olabilmektedir. Bu araştırma hemşirelerin travma sonrası hayata küsme bozuklukları ile umutsuzluk düzeylerinin incelenmesi amacıyla kesitsel tipte yapılmıştır. Araştırmanın evrenini Hatay Eğitim ve Araştırma Hastanesi'nde 2024 yılı Mart ayında aktif görev yapan 790 hemşire oluşturmaktadır. Bu çalışmada amaçlı örnekleme yöntemlerinden ölçüt örnekleme yöntemi kullanılmıştır. Bu araştırma ölçütleri karşılayan ve çalışmayı gönüllü olarak kabul eden 279 hemşire ile gerçekleştirilmiştir. Çalışmada veri toplama aracı olarak Travma Sonrası Hayata Küsme Bozukluğu Ölçeği, Beck Umutsuzluk Ölçeği ve Kişisel Bilgi Formu kullanılmıştır. Verilerin analizinde Shapiro Wilk normallik testi, Mann Whitney U, Kruskal Wallis testi ve Dunn-Benferroni ileri düzey çoklu karşılaştırma testi ve Spearman Korelasyon analizi testi kullanılmıştır. Beck Umutsuzluk Ölçeği toplam puanı 6,34±5,58, Geleceğe İlişkin Duygular ve Beklentiler puanı 1,45±1,63, Motivasyon Kaybı puanı 2,62±2,44, Umut puanı 2,26±2,00, Travma Sonrası Hayat Küsme Bozukluğu Ölçeği toplam puanı 29,86±17,12, Psikolojik Durum puanı 19,32±11,91ve Sosyal İşlevsellik puanı 10,54±5,79 olarak bulunmuştur. Beck umutsuzluk ölçeğinden alınabilecek min ve max puanlar 0 ile 20 arasında değişmektedir. Aynı zamanda TSHKB ölçeğinden alınabilecek min ve max puanlar 0 ile 76 arasında değişmektedir. Hemşirelerin yaş, eğitim düzeyi, çalışma yılı, deprem anında bulunulan yer, deprem enkazında mahsur kalma, depremde yakın can kaybı, depremde ev hasarı, depremde kendisinin ya da yakınının yara alma durumu, daha önce depremi yaşama, deprem sonrası yaşanılan yer, uyku sorunları yaşama, yapılan işe dikkatini verebilme ve deprem sonrası psikolojik destek alma değişkenleri ile travma sonrası hayata küsme bozukluğu ve umutsuzluk arasında anlamlı farklılıklar tespit edilmiştir. Travma sonrası hayata küsme bozukluğu ile umutsuzluk arasında pozitif yönde yüksek düzeyde bir ilişkinin olduğu belirlenmiştir. Araştırmanın sonuçları doğrultusunda, deprem sonrası hemşirelerde oluşan hafif düzey umutsuzluk ve travma sonrası hayata küsme bozukluklarının giderilmesi amacıyla hemşirelere psiko-sosyal destek sağlanması önerilmiştir.

Earthquakes can cause different psychological disorders such as trauma, depression, stress, mood disorders, hopelessness, helplessness, anxiety, fear, and embitterment in nurses who work under difficult conditions in the regions where they occur. This study was conducted as a cross-sectional study in order to examine the post-traumatic embitterment disorders and hopelessness levels of nurses. The universe of the study consists of 790 nurses who were actively working in Hatay Training and Research Hospital in March 2024. Criterion sampling method, which is one of the purposeful sampling methods, was used in this study. This research was conducted with 279 nurses who met the criteria and accepted the study voluntarily. Post-Traumatic Embitterment Disorder Scale, Beck Hopelessness Scale, and Personal Information Form were used as data collection tools in the study. Shapiro Wilk normality test, Mann Whitney U, Kruskal Wallis test, Dunn-Benferroni advanced multiple comparison test, and Spearman Correlation Analysis test were used in the analysis of the data. The total score of the Beck Hopelessness Scale was 6.34±5.58, the Future Feelings and Expectations score was 1.45±1.63, the Loss of Motivation score was 2.62±2.44, the Hope score was 2.26±2.00, the total score of the Post-Traumatic Embitterment Disorder Scale was 29.86±17.12, the Psychological Status score was 19.32±11.91, and the Social Functioning score was 10.54±5.79. The min and max scores that can be obtained from the Beck Depression Inventory range between 0 and 20. At the same time the min and max scores that can be obtained from the post-traumatic stress disorder scale range between 0 and 76.Significant differences were found between the nurses' age, level of education, years of work, location at the time of the earthquake, being trapped in earthquake debris, loss of close life in the earthquake, damage to the house in the earthquake, the status of self or a relative injured in the earthquake, having experienced an earthquake before, location lived after the earthquake, having sleep problems, being able to pay attention to the work being done, and receiving psychological support after the earthquake and post-traumatic embitterment disorder and hopelessness. It was determined that there is a high level positive relationship between post-traumatic embitterment disorder and hopelessness. In line with the results of the study, it was suggested that psychosocial support should be provided to nurses in order to eliminate mild hopelessness and post-traumatic embitterment disorders that occurred in nurses after the earthquake.
