Özet
İnfertilite, dünya çapında her yirmi çiftten birini etkileyen küresel bir sağlık sorunudur. Mevcut tedavi yöntemleri, spermatogenezi başlatamadıkları için germ hücresi üretemeyen erkek hastalar için yetersiz kalmaktadır. Bu hastalarda donör kaynaklı spermatojenik hücrelerin kullanımında etik kısıtlamalar bulunmaktadır. Vücut dışında ise otolog spermatojenik hücrelerin farklılaşma ve devamlılığı testis mikroçevresinin modellenmesi ile desteklenebilmektedir. Bu kapsamda insan uyarılmış pluripotent kök hücre (iUPKH) kaynaklı spermatogonyumlar ve Sertoli hücrelerini içeren testinoidlerde ex vivo koşulda spermatogenez siklusu uyarılarak fonksiyonel spermatidler elde edilebilir. Bu hipotezi test etmek üzere, insan ticari dermal fibroblastlarının (iDF) iUPKH'lere yeniden programlanması, elde edilen iUPKH'lerin tek hücre katmanlı kültüründe 12 gün süreyle Sertoli hücrelerine ve 10 gün süreyle spermatogonyal kök hücrelere (SKH) farklılaştırılması, bu hücrelerin karakterizasyonlarının ardından sonrasında testinoid ve 2B ko- kültür koşullarında kültürleri spermatogenez, yapısal bütünlük açısından 7, 14 ve 21. günler baz alınarak karşılaştırılması hedeflenmiştir. Tez çalışması, disiplinler arası, rastgele seçilmiş, kontrol grubu içeren, ileriye yönelik in vitro koşullarda uygulamalı deneysel çalışma olarak tasarlanmıştır. Bu tez çalışmasında, iDF'larından pluripotentlik belirteçleri OCT4, SOX2, Nanog, SSEA4 ve TRA-1-60 işaretlenen iUPKH'ler elde edilmiştir. Çalışmanın ikinci hedefinde, karakterize iUPKH'lerinin farklılaştırılmasıyla öncül Sertoli hücresi için Anti-Müllerian hormon (AMH) ve olgun Sertoli hücresi için SRY-Box Transcription Factor 9 (SOX9) belirteçlerinin işaretlendiği Sertoli hücreleri elde edilmiştir. Çalışmanın üçüncü hedefinde, iUPKH'ler farklılaştırılarak progenitör iSKH için ID4 ve iSKH için PLZF belirteçlerini işaretlendiği SKH'ler elde edilmiştir. Çalışmanın dördüncü hedefinde farklılaştırılan insan Sertoli hücreleri ve iSKH'lerinden testinoid ve 2B ko- kültür platformları geliştirilmiş; yapısal bütünlüğün ve organizasyonun sağlandığı ZO-1, α-tubulin ve SOX9; spermatogenezin sağlandığı SCP3 işaretlenen spermatid ve fonksiyonelliği AMH ve İnhibin B seviyeleri ile değerlendirilmiştir. Bu tez kapsamında, 21. gün sonunda iUPKH kaynaklı Sertoli hücreleri ve SKH'leriden testinoidler oluşturularak, yapısal organizasyon, fonksiyonel Sertoli hücreleri ve orta verimle haploid spermatidler elde edilmiş; böylelikle infertilite araştırmalarında etik açıdan kabul edilebilir, tekrarlanabilir ve kişiselleştirilebilir bir testis modeli olarak sunulmuştur. Kan-testis bariyerini (KTB) modelleyen insan testinoidlerinin preklinik araştırmaları kliniğe taşıyabilecek translasyonel birer platform olma potansiyeli bulunmakta olup bu sistemler in vitro spermatogenez araştırmaları için yeni referans sistemleri oluşturabilir.

Infertility is a global health issue affecting one in twenty couples worldwide. Current treatments remain inadequate for male patients who cannot produce germ cells because they fail to initiate spermatogenesis. Ethical constraints also limit the use of donor-derived spermatogenic cells in these patients. However, outside the body, differentiation and continuity of autologous spermatogenic cells can be supported by modelling the testis microenvironment. In this context, by inducing the spermatogenesis cycle under ex vivo conditions in testinoids containing human induced pluripotent stem cell (hiPSC)-derived spermatogonia and Sertoli cells, functional spermatids can be obtained. To test this hypothesis, it was aimed to reprogram human commercial dermal fibroblasts (iDFs) into hiPSCs, differentiate the obtained hiPSCs into Sertoli cells for 12 days and spermatogonial stem cells (SSCs) for 10 days in monolayer culture, and after characterisation of these cells, compare their cultures in testinoid and co-culture conditions in terms of spermatogenesis and structural integrity based on days 7, 14, and 21. The thesis was designed as an interdisciplinary, randomised, prospective in vitro experimental study with a control group. In this thesis, the first objective was achieved by obtaining hiPSCs expressing pluripotency markers OCT4, SOX2, NANOG, SSEA-4, and TRA-1-60 from iDFs. For the second objective, Sertoli cells expressing AMH (for precursor Sertoli cells) and SOX9 (for mature Sertoli cells) were obtained by differentiating characterized hiPSCs. In the third objective, SSCs expressing ID4 (for progenitor SSCs) and PLZF (for SSCs) were obtained through differentiation of hiPSCs. The fourth objective involved developing testinoid and co-culture platforms from differentiated human Sertoli cells and SSCs; structural integrity and organisation were evaluated using ZO-1, α-tubulin, and SOX9; spermatogenesis was assessed through spermatids expressing SCP3; and functionality was measured via AMH and İnhibin B levels and via BTB formation. Within this thesis, by day 21, testinoids were generated from hiPSC-derived Sertoli cells and SSCs, with structural organisation, functional Sertoli cells, and haploid spermatids achieved; thus, a testis model that is ethically acceptable, reproducible, and personalised for infertility research was presented. Human testinoids modelling the blood-testis barrier have the potential to serve as translational platforms capable of transferring preclinical research to clinical application, and these systems could establish new reference models for in vitro spermatogenesis research and drug screening studies.
