Özet

Giriş ve Amaç: Bening prostat hiperlazisi(BPH), yaşlı erkekler arasında yaygın bir durumdur. Konservatif ve farmakolojik yaklaşımlar yaygın olarak kullanılmasına rağmen, cerrahi müdahaleler önemli bir tedavi seçeneği olmaya devam etmektedir. Uzun yıllar boyunca, transüretral prostat rezeksiyonu (TURP) altın standart olarak kabul edilmiştir; ancak, bu yöntem bir dizi potansiyel komplikasyonla ilişkilidir. Minimal invaziv bir alternatif olan holmium lazer prostatektomi (HoLEP), daha hızlı iyileşme, intraoperatif kan kaybının azalması ve daha düşük komplikasyon oranları gibi avantajları nedeniyle öne çıkmıştır. Bununla birlikte, HoLEP cerrahisi sırasında kullanılan önemli miktarda irrigasyon sıvısı, özellikle yaşlı hastalarda ve düşük vücut kitle indeksi (BMI) olan hastalarda intraoperatif hipotermi riskini artırır. Ayrıca, genel anestezi vücudun termoregülasyon mekanizmalarını baskılar ve hastaları hipotermiye daha yatkın hale getirir. Bu durum, hem pasif hem de aktif ısınma stratejileriyle önlenebilir; prewarming ve intraoperatif ısınmanın birlikte uygulanmasının daha etkili olduğu gösterilmiştir. Bu çalışmanın amacı, HoLEP cerrahisinde prewarming'in intraoperatif hipotermi üzerine etkisini, hipotermi süresini ve hipotermi ile ilişkili faktörleri araştırmaktır. Gereç ve Yöntem: Çalışmamız genel anestezi altında HoLEP cerrahisi geçirecek 18 yaş ve üzeri 116 hastanın dahil edildiği tek merkezli, prospektif, randomize kontrollü bir klinik çalışmadır. Hastalar bilgisayar destekli randomizasyon yöntemiyle Grup A (prewarming) ve Grup B (kontrol grubu) olmak üzere iki gruba ayrıldı. Prewarming grubundaki hastalar preoperatif alanda 10 dakika boyunca aktif olarak ısıtıldı ve bu sırada 41°C'ye ısıtılmış intravenöz sıvı verildi. Kontrol grubundaki hastalar ise preoperatif alanda aktif olarak ısıtılmadı ve damaryolundan sıvı verilmedi. Operasyon boyunca her iki grup da aktif olarak ısıtıldı ve oda sıcaklığında intravenöz sıvı verildi. İntraoperatif standart monitörizasyona ek olarak özofageal sıcaklık monitörizasyonu yapıldı. Cerrahi süresince vücut sıcaklığı ve hemodinamik parametreler düzenli aralıklarla kaydedildi. Vücut sıcaklığı < 36℃ olan hastalar hipotermik olarak değerlendirilip hipotermi başlangıç zamanı ve süresi kaydedildi.Hipotermi başlangıç zamanı ve süresi kaydedildi. Cerrahi bitiminde cerrahi süre, kullanılan intravenöz sıvı miktarı ve irigasyon sıvısı hacmi kaydedildi. Bulgular: Prewarming grubundaki hastalara kıyasla kontrol grubundaki hastalarda hipotermi olma oranı anlamlı olarak daha yüksek bulunmuştur. Prewarming uygulanan hastalarda hipotermi başlangıç zamanı anlamlı olarak daha geç bulunmuştur. Prewarming uygulanan hastaların hipotermi süresi kontrol grubuna göre anlamlı olarak daha kısadır. Univariate Lojistik Regresyon analizi sonucunda prewarming uygulanmaması hipotermi gelişimi için risk faktörü olarak bulunmuştur (p=0,013). Prewarming uygulanmaması, hipotermi gelişme riskini prewarming uygulanan hastalara kıyasla 5,296 kat artırmaktadır. Univariate lojistik regresyon analizi sonuçlarına göre; BMI değerleri, kullanılan irrigasyon sıvısı hacmi ve cerrahi sürenin hipotermi gelişimi üzerinde anlamlı bir etkisi bulunmamıştır. Sonuç: Çalışmamızda intraoperatif hipoterminin HoLEP cerrahisinde yüksek insidansa sahip olduğu ve prewarming uygulamasının bu durumu anlamlı şekilde azalttığı gösterilmiştir. Ayrıca hipoterminin yalnızca insidansı değil, başlangıç zamanı ve süresi üzerinde de olumlu etkileri olduğu saptanmıştır. Bu bulgular, HoLEP operasyonlarında termoregülasyon yönetiminin önemini vurgulamakta ve prewarming'in olası faydalarına işaret etmektedir.

Introduction and Aim: Benign prostatic hyperplasia (BPH) is a prevalent condition among elderly men. While conservative and pharmacological approaches are commonly employed, surgical interventions remain an important treatment option. For many years, transurethral resection of the prostate (TURP) has been regarded as the gold standard; however, it is associated with a number of potential complications. Holmium laser enucleation of the prostate (HoLEP), a minimally invasive alternative, has gained prominence due to its advantages, including faster recovery, reduced intraoperative blood loss, and lower complication rates. Nevertheless, the substantial amount of irrigation fluid required during HoLEP increases the risk of intraoperative hypothermia, particularly in older patients and those with a low body mass index (BMI). Moreover, general anesthesia suppresses the body's thermoregulatory mechanisms, further predisposing patients to hypothermia. This condition can be prevented by both passive and active warming strategies, with combined prewarming and intraoperative warming shown to be more effective. The present study aims to evaluate the effect of prewarming on the onset of intraoperative hypothermia during HoLEP procedures, as well as to identify factors associated with its development. Materials and Methods: Our study is a single-center, prospective, randomized controlled clinical trial including 116 patients aged 18 years and older who were scheduled to undergo HoLEP surgery under general anesthesia. Patients were randomized into two groups using a computer-assisted method: Group A (prewarming) and Group B (control group). In the prewarming group, patients were actively warmed in the preoperative area for 10 minutes, during which intravenous fluids warmed to 41°C were administered. In the control group, patients were not actively warmed in the preoperative area, and no intravenous fluids were administered. During the operation, both groups received active warming, and intravenous fluids at room temperature were infused. In addition to standard intraoperative monitoring, esophageal temperature monitoring was performed. Body temperature and hemodynamic parameters were recorded at regular intervals throughout the surgery. The onset time and duration of hypothermia were documented. A body temperature of less than 36 ℃ was defined as hypothermia, and for affected patients, the onset time and duration of hypothermia were documented. At the end of surgery, the duration of the procedure, the amount of intravenous fluid administered, and the volume of irrigation fluid used were recorded. Results: The incidence of hypothermia was significantly higher in the control group than in the prewarming group. In patients who underwent prewarming, the onset of hypothermia was found to occur significantly later. The duration of hypothermia was also significantly shorter in the prewarming group compared to the control group. Univariate logistic regression analysis identified the absence of prewarming as a risk factor for the development of hypothermia (p = 0.013). The lack of prewarming increased the likelihood of hypothermia by 5.296 times compared to patients who received prewarming. In contrast, body mass index (BMI), the volume of irrigation fluid used, and the duration of surgery were not found to be significant predictors of hypothermia in the univariate logistic regression analysis. Conclusion: Our study demonstrated that intraoperative hypothermia has a high incidence in HoLEP surgery and that the application of prewarming significantly reduces this condition. Moreover, prewarming was found to have favorable effects not only on the incidence of hypothermia but also on its onset time and duration. These findings highlight the importance of thermoregulation management in HoLEP procedures and suggest the potential benefits of prewarming.
