Özet
Aynı zamanda evrensel bir iletişim aracı olan insan eli evrimsel süreçte insanoğlunun hayatta kalabilmesinin ötesinde Dünya üzerindeki varlığını baskın tür olarak inşa edebilmesinde büyük öneme sahiptir. Eli açıp kapatmak, bir nesneyi kavramak ve alet olarak kullanabilmek gibi basit olarak görülen işlevlerin gerçekleştirilebilmesi için arkada karmaşık mekanik bir süreç yatmaktadır. Elde bulunan lumbrikal kaslar da bu mekanik sürecin bir parçasını oluşturmaktadır.. Dört adet, ince uzun kastan oluşan lumbrikal kaslar, eldeki musculus flexor digitorum profundus tendonlarından başlayarak kendilerine karşılık gelen parmakların dorsal aponeurosis'lerinin dış yan tarafında sonlanmaktadır. İlk iki lumbrikal kas n. medianus'tan innerve olurken, son iki lumbrikal kas n. ulnaris r. profundus'tan innerve olmaktadır. Yapılan çalışmalar lumbrikal kasların innervasyonu hakkında çeşitli varyasyonlar ortaya koymuştur. Lumbrikal kasların innervasyon paterninin detaylı incelenmesi ve ayrıntılı bir harita oluşturulması, bölgede yapılacak cerrahi girişimler için güvenli alanların belirlenmesinde ve botulinum toksin enjeksiyonları gibi kapalı girişimlere uygun nokta bilgisi sağlanması açısından faydalı olacaktır. Çalışmamızda Ege Üniversitesi Tıp Fakültesi Anatomi Anabilim Dalı kadavra envanterinde bulunan %10 formalin ile fikse edilmiş, ellerinde herhangi bir deformite ve patoloji bulunmayan 20 insan kadavrası kullanılmıştır. Belirlediğimiz parametreler doğrultusunda diseksiyonlar yapılmış ve kadavra üzerinde mezura ve kumpas yardımıyla kasların uzunluğu, genişliği ve kalınlığı ölçülmüştür. Kasların innervasyonunu sağlayan sinir dalları takip edilerek kaydedilmiş ve fotoğraflanmıştır. Kasların en geniş, en kalın olduğu kısımlar ve motor sinir giriş noktaları belirlenip ImageJ programı yardımıyla diseksiyon alanının hangi bölümüne denk geldiği hesaplanmıştır. Ardından lumbrikal kaslara literatürde daha önce uygulandığını görmediğimiz Modifiye Sihler tekniği uygulanmış ve kas içi sinir seyirleri izlenerek şematize edilmiştir. Kadavralar üzerinden elde ettiğimiz veriler IBM SPSS Statistics for Windows, Version 25.0.(IBM Corp. Released 2017, Armonk, NY) paket programında değerlendirilmiş ve ardından literatürde bulunan verilerle karşılaştırma yapılmıştır. Çalışmamızda diseke ettiğimiz kadavra ellerinde bulunan birinci lumbrikal ve ikinci lumbrikal kasın %100'ü n. medianus'tan, dördüncü lumbrikal kasın ise %100'ü n. ulnaris r. profundus'tan innerve olmaktadır. Üçüncü lumbrikal kas ise çalışmamızda tek çift sinir dallı innervasyona sahip olan kas olup %77,2 ile sadece n. ulnaris r. profundus'tan, %22,7 ile de hem n. medianus hem de n. ulnaris r. profundus'tan dal aldığı görülmüştür. Kasın en geniş, en kalın yerinin ölçüldüğü ve sinir giriş noktalarının yoğunlaştığı diseksiyon alanı bölümü olarak da dördüncü bölüm bulunmuştur. Modifiye Sihler tekniğinden sonra incelenen kaslarda, kasa giren sinirin 'Y' şeklinde dallanmalar gösterdiği ve en çok dallanmanın sinir giriş yerinde olduğu gözlenmiştir. Çalışmamızda Modifiye Sihler tekniğini lumbrikal kaslara ilk kez uygulamış ve kas içi sinir dağılım özellikleri görülür hale getirilmiştir. Elde edilen verilerin literatüre katkısının yanı sıra bu bölgede çalışan ve lumbrikal kaslara yönelik işlemler gerçekleştiren klinisyenlerin pratiğine yardımcı olacak kılavuz niteliği taşıdığını düşünüyoruz.

The human hand, which also serves as a universal means of communication, has played a crucial role not only in human survival throughout evolution but also in enabling humankind to establish itself as the dominant species on Earth. Seemingly simple functions such as opening and closing the hand, grasping objects, and using tools are accomplished through a complex mechanical process. The lumbrical muscles of the hand constitute an essential component of this mechanism. The lumbrical muscles consist of four slender and elongated muscles that originate from the tendons of the flexor digitorum profundus and insert into the lateral aspects of the dorsal aponeuroses of the corresponding digits. The first two lumbrical muscles are innervated by the median nerve, whereas the last two are innervated by the deep branch of the ulnar nerve. Previous studies have demonstrated various anatomical variations in the innervation of the lumbrical muscles. A detailed evaluation of their innervation patterns and the construction of a precise anatomical map would be valuable for determining safe zones during surgical interventions in this region and for identifying appropriate injection sites for closed procedures such as botulinum toxin applications. In this study, 20 human cadavers fixed in 10% formalin, no deformities or pathological conditions of the hands, were selected from the cadaver inventory of the Department of Anatomy, Faculty of Medicine, Ege University. Dissections were performed according to predetermined parameters, and the length, width, and thickness of the muscles were measured using a measuring tape and caliper. The nerve branches responsible for muscle innervation were traced, documented, and photographed. The widest and thickest regions of the muscles, as well as the motor entry points, were identified, and their locations within the dissection field were determined using ImageJ software. Subsequently, the modified Sihler technique—previously unreported in the literature for application to the lumbrical muscles—was applied, and the intramuscular nerve courses were visualized and schematized. Data obtained from the cadaveric specimens were analyzed using IBM SPSS Statistics for Windows, Version 25.0 (IBM Corp., Released 2017, Armonk, NY), and the results were compared with those reported in the literature. In our study, 100% of the first and second lumbrical muscles were innervated by the median nerve, whereas the fourth lumbrical muscle was innervated exclusively by the deep branch of the ulnar nerve. The third lumbrical muscle was the only muscle exhibiting dual innervation; it was innervated solely by the deep branch of the ulnar nerve in 77.2% of cases and by both the median nerve and the deep branch of the ulnar nerve in 22.7% of cases. The region corresponding to the fourth dissection zone was identified as the area where the muscle was vi widest and thickest and where the nerve entry points were most densely concentrated. Following the application of the modified Sihler technique, the intramuscular nerves were observed to exhibit a“Y-shaped”branching pattern, with the highest degree of branching occurring at the nerve entry site. In this study, the modified Sihler technique was applied to the lumbrical muscles for the first time, enabling visualization of their intramuscular nerve distribution patterns. We believe that the findings not only contribute to the existing anatomical literature but also provide practical guidance for clinicians performing procedures involving the lumbrical muscles.
