Özet

Bu araştırma, primipar gebelerde travmatik doğum algısı ve gebelik stres düzeyi ile doğum şekli tercihi arasındaki ilişkinin belirlenmesi amacıyla yapılmıştır. Doğumun fizyolojik bir süreç olmasına karşın, birçok kadın tarafından korku, belirsizlik ve kontrol kaybı ile ilişkilendirildiği, bu algıların gebelikte yaşanan stresle birlikte doğum şekli tercihlerini etkileyebildiği düşünülmektedir. Literatürde travmatik doğum algısı ve gebelik stresinin doğum şekli tercihi üzerindeki etkilerinin birlikte ele alındığı çalışmaların sınırlı olması, özellikle primipar gebeler üzerinde yapılan araştırmaların yetersizliği bu çalışmanın temel çıkış noktasını oluşturmuştur. Araştırma tanımlayıcı ve ilişki arayıcı tasarımda planlanmış olup Konya Başkent Üniversitesi Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Polikliniği'nde Şubat 2025–Ağustos 2025 tarihleri arasında yürütülmüştür. Çalışmanın evrenini üçüncü trimesterde bulunan düşük riskli primipar gebeler oluşturmuş, örneklem olasılıksız örnekleme yöntemlerinden gelişigüzel örnekleme ile seçilen 381 gebeden oluşmuştur. Veriler Kişisel Bilgi Formu, Travmatik Doğum Algısı Ölçeği, Gebelik Stres Derecelendirme Ölçeği ve Gebelerin Doğum Şekli Tercihi Ölçeği kullanılarak yüz yüze görüşme yöntemiyle toplanmıştır. Elde edilen verilerin analizinde tanımlayıcı istatistikler, t testi, varyans analizi, korelasyon ve çoklu regresyon analizleri kullanılmıştır. Araştırma bulgularında travmatik doğum algısı ile gebelik stres düzeyi arasında pozitif yönde anlamlı bir ilişki saptanmıştır (r=0,551, p<0,001). Gebelik stres düzeyi arttıkça sezaryen doğum tercihi eğiliminin yükseldiği belirlenmiştir (r=-0,262, p<0,001). Travmatik doğum algısının da doğum şekli tercihi üzerinde anlamlı bir yordayıcı olduğu bulunmuştur (r=-0,672, p<0,001). Ayrıca gebelik süresince duygusal/fiziksel destek alan gebelerin vajinal doğuma yönelik daha olumlu tutumlara sahip olduğu saptanmıştır (p<0,001). Sonuç olarak bu çalışma, travmatik doğum algısı ve gebelik stresinin doğum şekli tercihini önemli ölçüde etkilediğini ortaya koymuştur. Bulgular, doğum şekli tercihinin yalnızca tıbbi faktörlere değil, aynı zamanda psikososyal etmenlere bağlı olarak şekillendiğini göstermektedir. Bu doğrultuda gebelik döneminde stres düzeyinin ve travmatik algının erken dönemde değerlendirilmesi, ebelik danışmanlığı ve psikososyal destek uygulamalarının güçlendirilmesi önerilmektedir. Elde edilen sonuçların, doğum öncesi bakım hizmetlerinin geliştirilmesine ve olumlu doğum deneyimlerinin desteklenmesine katkı sağlayacağı düşünülmektedir.

This study was conducted to determine the relationship between traumatic childbirth perception, pregnancy stress level, and mode of delivery preference in primiparous pregnant women. Although childbirth is a physiological process, it is known that many women associate it with fear, uncertainty, and loss of control, these perceptions, together with stress experienced during pregnancy, may influence preferences regarding the mode of delivery. The limited number of studies in the literature addressing the combined effects of traumatic childbirth perception and pregnancy stress on delivery mode preference, as well as the insufficiency of research focusing specifically on primiparous women, constituted the main motivation for this study. The research was designed as a descriptive and correlational study and was conducted at the Obstetrics and Gynecology Outpatient Clinic of Başkent University Konya Training and Research Hospital between February 2025 and August 2025. The study population consisted of low-risk primiparous pregnant women in the third trimester, and the sample included 381 pregnant women selected using a convenience sampling method, one of the non-probability sampling techniques. Data were collected through face-to-face interviews using the Personal Information Form, the Traumatic Childbirth Perception Scale, the Pregnancy Stress Rating Scale, and the Delivery Mode Preference Scale. Descriptive statistics, t-test, analysis of variance (ANOVA), correlation, and multiple regression analyses were used in the data analysis. The findings revealed a statistically significant positive relationship between traumatic childbirth perception and pregnancy stress level (r=0,551, p<0,001). It was determined that as the level of pregnancy stress increased, the tendency to prefer cesarean delivery also increased (r=-0,262, p<0,001). Traumatic childbirth perception was also found to be a significant predictor of delivery mode preference (r=-0,672, p<0,001). In addition, pregnant women who received emotional and/or physical support during pregnancy were found to have more positive attitudes toward vaginal delivery (p<0,001). In conclusion, this study demonstrated that traumatic childbirth perception and pregnancy stress significantly affect delivery mode preference. The findings indicate that delivery mode preference is shaped not only by medical factors but also by psychosocial factors. Accordingly, it is recommended to assess stress levels and traumatic perceptions early in pregnancy and to strengthen midwifery counseling and psychosocial support practices. The results are expected to contribute to the improvement of antenatal care services and to the promotion of positive childbirth experiences.
