Özet

Amaç Ergenlik dönemi, sirkadiyen ritmin hem biyolojik değişimler hem de sosyal faktörler (ekran süresi artışı, uyku faz kayması gibi) nedeniyle belirgin şekilde baskılandığı kritik bir gelişim evresidir. Literatürde sirkadiyen genlerin DNA metilasyonu ile obezite ilişkisi çalışılmış olsa da veriler çoğunlukla yetişkin popülasyonlara dayanmakta ve ergenlik dönemine özgü genetik-epigenetik etkileşimler belirsizliğini korumaktadır. Çalışmamızda sirkadiyen ritmin ana düzenleyicisi olan BMAL1 geninin CpG metilasyon profili, rs7950226 polimorfizmini, ergen obezitesinin klinik, davranışsal ve metabolik yönleriyle bütüncül olarak incelemeyi amaçladık. Gereç ve Yöntem Çalışma İstanbul Tıp Fakültesi Etik Kurulu onaylı, kesitsel bir gözlemsel çalışmadır. Çalışmaya güç analizi yapılarak 61 obez birey (VKİ SDS > +2 ) ve yaş ile cinsiyet açısından eşleştirilmiş 61 sağlıklı kontrol (VKİ SDS -1 ile +1 arası) toplam 122 ergen dahil edilmiştir. Katılımcıların antropometrik ölçümleri yapılıp biyokimyasal sonuçlar değerlendirdi, uyku alışkanlığı, kronotip, beslenme alışkanlıkları ve anksiyete-depresyon ölçekleri uygulandı. Genetik laboratuvar çalışmaları İstanbul Tıp Fakültesi Tıbbi Biyoloji Araştırma Laboratuvarlarında yürütüldü. İstatistiksel analizler SPSS (v31) , JASP (v0.96) yazılımlarıyla gerçekleştirilmiş, genetik ilişkilendirme modelleri için SNPStats web aracı kullanılmıştır. Bulgular Tüm beş BMAL1 CpG bölgesinde obez grupta kontrol grubuna kıyasla belirgin hipometilasyon saptandı ve Bonferroni düzeltmesi sonrası tüm farklar istatistiksel olarak anlamlılığını korudu. Beş CpG bölgesinin ortalaması olan ortalama CpG metilasyonu obez grupta 2,4 iken kontrol grubunda 3,0 olarak ölçüldü (p < .001). En güçlü etkiler CpG-4 (rrb= -0.508) ve CpG-5 (rrb= -0.494) bölgelerinde gözlendi. VKİ-SDS ile tüm CpG bölgeleri arasında tutarlı ve anlamlı negatif korelasyonlar saptandı. Cinsiyet katmanlı analizde BMAL1 negatif korelasyon kızlarda belirgin ve istatistiksel olarak anlamlı bulunurken (rrb= -0.601) erkeklerde ise negatif yönlü bir eğilim olsa da Bonferroni düzeltmesi sonrası anlamlılık sınırını geçemedi. Kontrol grubunda akşamcıl kronotip ve daha uzun hafta içi uyku süresi düşük BMAL1 metilasyonuyla ilişkiliyken, bu ilişki obez grupta gözlenmedi. BMAL1 rs7950226 analizinde her iki grupta Hardy-Weinberg dengesi korundu (kontrol p=1.000 obez p= .160). Genotip ve allel frekansları açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmadı. Genetik model analizinde en uyumlu model resesif model olarak belirlendi (AIC=174.8). GG genotipi için sınırda obeziteye karşı koruyucu eğilim gözlendi (OR = 0,36; %95 GA: 0,11–1,16; p = ,076) fakat istatistiksel anlamlılığa ulaşmadı. Obez grupta insülin, HOMA-IR, LDL, trigliserid, ALT, ürik asit ve inflamatuar belirteçler yüksek, HDL ve D vitamini düşük saptandı. Sosyal medya süresi obez grupta daha uzun, beden imajı ve beslenme puanları daha düşük saptandı. Uyku süresi, sosyal jetlag ve kafein tüketiminde ise gruplar arasında fark saptanmadı. Sonuç Çalışmamız, ergen obezitesinin BMAL1 genindeki beş CpG bölgesini kapsayan tutarlı hipometilasyon örüntüsü ile ilişkili olduğunu ortaya koymaktadır. Kız ergenlerde saptanan belirgin hipometilasyon BMAL1 genindeki epigenetik değişikliğin cinsiyete özgü mekanizmalar üzerinden işleyebileceğini düşündürmektedir. Kontrol grubuyla sınırlı kalan sirkadiyen metilasyon ilişkisi taban etkisiyle obezitede bu bağlantının bozulabileceğini düşündürmektedir. BMAL1 geni rs7950226'nın resesif modelde post-hoc güç analizinin %36,3 düzeyinde kalması daha geniş örneklemlerle doğrulama yapılmadan negatif bir SNP bulgusunun kesin olarak yorumlanamayacağını göstermektedir. Objective Adolescence is a critical developmental period during which circadian rhythm is substantially disrupted by both biological changes and social factors such as increased screen exposure and sleep phase delay. Although the association between DNA methylation of circadian genes and obesity has been investigated in the literature, existing data are mainly derived from the adult population, and the genetic epigenetic interactions specific to adolescence remain unclear. In this study we aimed to examine the CpG methylation profile and rs7950226 polymorphism of the BMAL1 gene, a central regulator of circadian rhythm, together with the clinical, behavioral, and metabolic aspects of adolescent obesity in an integrated manner. Materials and Methods This study was a cross-sectional observational study approved by the Ethics Committee of İstanbul Faculty of Medicine. Based on power analysis, a total of 122 adolescents were included: 61 adolescents with obesity (BMI SDS > +2) and 61 age and sex-matched healthy controls (BMI SDS between -1 and +1). Anthropometric measurements were performed, biochemical parameters were evaluated, scales assessing sleep habits, chronotype, eating habits, and anxiety-depression were administered. All genetic laboratory analyses were performed in the Medical Biology Research Laboratories of Istanbul Faculty of Medicine. Statistical analyses were conducted using SPSS (v31) and JASP (V0.96), and the SNPStats web tool was used for genetic association models. Results Widespread hypomethylation was detected in all five BMAL1 CpG regions in the obesity group compared with the control group, and all differences remained statistically significant after Bonferroni correction. Mean CpG methylation, calculated as the average of the five CpG regions, was 2.4 in the obesity group and 3.0 in the control group (p < .001). The strongest effects were observed at the CpG-4 (rrb = -0.508) and CpG-5 (rrb= -0.494) regions. Consistent and significant negative correlations were found between BMI-SDS and all CpG regions. In sex-stratified analyses the negative correlation of BMAL1 methylation was more pronounced and statistically significant in girls (rrb= -0.601), whereas in boys although the trend was also negative, it did not remain significant after Bonferroni correction. In the control group an evening chronotype and longer weekday sleep duration were associated with lower BMAL1 methylation, but this association was not observed in the obesity group. In the analysis of BMAL1 rs7950226, Hardy-Weinberg equilibrium was preserved in both groups. No statistically significant differences were found between the groups in terms of genotype or allele frequencies. In genetic model analysis the best-fitting model was the recessive model (AIC=174.8). A borderline protective trend against obesity was observed for the GG genotype (OR=0.36, 95% CI: 0.11-1.16, p=.076), although statistical significance was not reached. In the obesity group insulin, HOMA-IR, LDL, triglycerides, ALT, uric acid, and inflammatory markers were higher, but HDL and vitamin D were lower. Social media duration was longer, and body image and nutrition scores were lower in the obesity group. No differences were found between the groups in sleep duration, social jetlag, or caffeine intake. Conclusion The present findings demonstrate that adolescent obesity is accompanied by hypomethylation across five CpG regions of the BMAL1 gene. The pronounced hypomethylation observed in adolescent girls suggests that epigenetic alterations in BMAL1 may operate through sex-specific mechanisms. The circadian-methylation association, which remained limited to the control group, suggests that this link may be disrupted in obesity, possibly through a floor effect. For BMAL1 rs7950226, the post hoc power of the recessive model remained at 36.3%, indicating that the absence of a significant SNP association should be interpreted with caution pending replication in larger cohorts.
