Özet

Kronik bir hastalık olan kanser, tanı anından itibaren tedavi ve izleme süreçleriyle bireyin yaşamını etkileyen çok boyutlu bir kriz niteliğindedir. Özellikle en çok tercih edilen tedavi yöntemlerinden biri olan kemoterapi, hastalar için fiziksel, psikososyal, ekonomik, ilişkisel dönüşümleri ve rol değişimlerini beraberinde getirmektedir. Toplumsal cinsiyet rolleri ekseninde aile içinde geleneksel olarak bakım verme, ev içi sorumlulukları yürütme ve eş, anne olma rollerini üstlenen kadınlar, kanser teşhisi aldıklarında hem hasta hem de bu rollerin taşıyıcısı olarak çift yük altında kalmaktadır. Tedavi sürecinin beraberinde getirdiği fiziksel kısıtlılıklar ve duygusal yıpranma, kadının aile içindeki alışılmış konumunu ve diğer aile üyeleriyle olan ilişkilerini etkileyebilmektedir. Bu durum yalnızca kadını değil, aile sisteminin bütününü etkileyerek aile içi denge ve işleyişte değişimlere yol açmaktadır. Bu nedenle kanser sürecinin, sosyal hizmet perspektifinden aile sistemi temelinde bütüncül bir müdahale anlayışıyla ele alınması önem taşımaktadır. Bu çalışma, kemoterapi gören evli ve çocuklu kadınların hastalık ve tedavi sürecinde aile içindeki rol ve ilişkilerinde yaşanan değişimleri kendi öznel deneyimleri üzerinden derinlemesine incelemek amacıyla nitel araştırma yöntemlerinden fenomenoloji deseni ile gerçekleştirilmiştir. Araştırmanın çalışma grubunu, amaçlı ve kartopu örnekleme yöntemlerinden ölçüt örnekleme tekniği kullanılarak belirlenen, aktif olarak kemoterapi tedavisi alan veya son kemoterapi tedavisinin üzerinden en fazla bir yıl geçmiş olan ve ailesiyle birlikte yaşayan 15 evli kadın katılımcı oluşturmaktadır. Katılımcılarla gerçekleştirilen yarı yapılandırılmış derinlemesine görüşmelerden elde edilen veriler, bilgisayar destekli nitel veri analizi programı MAXQDA kullanılarak tematik analiz yöntemiyle çözümlenmiştir. Veri analizi sürecinde, doğrudan katılımcıların hissettikleri duygular, yaşantısal benzerlikleri ve öznel deneyimleri merkeze alınarak tematik bir çerçeve oluşturulmuştur. Araştırma sonucunda, kemoterapi sürecindeki kadınların aile içi rol dağılımlarında zorunlu değişimler yaşandığı ve bu durumun kadınlar üzerinde suçluluk, yetersizlik gibi duygusal karmaşalara yol açtığı tespit edilmiştir. Elde edilen bulgular; hastalık öncesinde kadının üzerinde olan ev içi sorumlulukların ve çocuk bakımının süreç içinde eş, anne, kayınvalide veya kız kardeş gibi yakın çevreye devredildiğini, ancak bu devir sürecinin zamanla kadına geri döndüğü anlaşılmıştır. Bununla birlikte, biyopsiko-sosyal model çerçevesinde incelenen veriler doğrultusunda, eşlerin destekleyici tutumlarının kadının tedaviye uyumunu ve psikososyal iyilik halini pozitif yönde etkilediği, buna karşın rol kaybı yaşayan kadınların sosyal katılım düzeylerinin ve algılanan yaşam kalitelerinin ciddi ölçüde düştüğü sonucuna ulaşılmıştır. Elde edilen bu sonuçlar bağlamında, kemoterapi sürecindeki kadınların ve ailelerinin desteklenmesine yönelik tıbbi sosyal hizmet odaklı öneriler geliştirilmiştir.

Cancer, a chronic disease, is a multifaceted crisis that affects an individual's life from the moment of diagnosis through treatment and monitoring processes. Chemotherapy, in particular, brings about physical, psychosocial, economic, relational transformations and role changes for patients. Women, traditionally responsible for caregiving, household responsibilities, and the roles of wife and mother within the family, face a double burden upon receiving a cancer diagnosis: both as patients and as bearers of these roles. The physical limitations and emotional strain associated with treatment can affect a woman's usual position within the family and her relationships with other family members. This situation affects not only the woman but the entire family system, leading to changes in family balance and functioning. Therefore, addressing the cancer process from a social work perspective, with a holistic intervention approach based on the family system, is crucial. This study was conducted using a phenomenological design, a qualitative research method, to deeply examine the changes in roles and relationships within the family experienced by married women with children undergoing chemotherapy during the illness and treatment process, based on their subjective experiences. The study group consists of 15 married female participants who live with their families and are either actively receiving chemotherapy or have completed their last chemotherapy treatment within the past year; these participants were selected using the criterion sampling technique—a method derived from purposive and snowball sampling approaches. Data obtained from semi-structured in-depth interviews with the participants were analyzed using thematic analysis with the computer-assisted qualitative data analysis program MAXQDA. During the data analysis process, a thematic framework was created by focusing directly on the participants' feelings, experiential similarities, and subjective experiences. The research results revealed that women undergoing chemotherapy experience forced changes in their family roles, leading to emotional turmoil such as guilt and feelings of inadequacy. The findings also indicated that household responsibilities and childcare, which were primarily the woman's responsibility before the illness, were transferred to close family members such as spouses, mothers, mothers-in-law, or sisters during the course of the illness, but this transfer eventually reversed and reverted back to the woman's role. However, based on data examined within the framework of the biopsycho-social model, it was concluded that the supportive attitudes of spouses positively affect the woman's adherence to treatment and psychosocial well-being, while the levels of social participation and perceived quality of life of women experiencing role loss decreased significantly. In light of these findings, medical social work-focused recommendations have been developed to support women and their families during chemotherapy.
