Özet

ÖZET Arteria maxillaris'in Fossa infratemporalis'teki Cerrahi Anatomisi Amaç: Çalışmamızda a.maxillaris'in fossa infratemporalis'te yer alan kısmı; seyir, dallanım paterni, çap ve uzaklık ölçümleri, uzanım şekli, çevre anatomik yapılar ile ilişkileri, kemik referans noktalarına göre eksensel uzaklıkları, n.mandibularis'e göre konumu ve kendi içinde gösterdiği çeşitlenmeler ile bütüncül bir şekilde incelenmiştir. Elde edilen veriler ile a.maxillaris'in konumunun ve çevre yapılar ile ilişkilerinin daha iyi bir şekilde ortaya konulması ve bu bağlamda klinik ve cerrahi müdahalelerde iatrojenik yaralanma riskinin minimalize edilmesine katkı sağlamak hedeflenmiştir. Bütün bu süreçte gerçekleştirilen morfolojik ve morfometrik değerlendirmelerin ve dokümantasyonu sağlanan olguların; anatomik bilgi birikimini genişletmesi ve gelecek diseksiyonlarda yol gösterici olması beklenmektedir. Materyal & Metot: Formalin ile fikse on kadavranın yirmi tarafında, transfacial lateral yaklaşımla a. maxillaris'in fossa infratemporalis'te yer alan kısmı incelenmiştir. Superficial yapıların, ramus mandibulae'nın ve arcus zygomaticus'un kaldırımasını takiben; a.maxillaris'in ve çevre yapıların mikromakasla gross diseksiyonları yapılmıştır. Ramifikasyonlara ve external yapılara ait tüm çap ve uzaklık ölçümleri elektronik kaliper kumpas aracılığıyla gerçekleştirilmiştir. Plan farklıları nedeniyle çevre kemik yapılardan alınan tüm uzaklık ölçümleri eksensel özelliktedir. Bulgular: A.maxillaris'in m.pterygoideus lateralis'e göre lateral seyir oranı: %65; medial seyir oranı: 35% olarak bulunmuştur. Değişsen seyir ile farklı parametrelerin korelasyonları gösterilmiştir. A.maxillaris'in dallanım paterni değerlendirilmiş, ortak kütük yapıları oranları ile birlikte gösterilmiş ve pterygoid dalların organizasyonları sınıflandırılmıştır. A.maxillaris'in uzanım şekilleri yedi ayrı formda ele alınmış; origin noktasının konumu tragion'dan geçen horizontal eksene göre 27,4mm±3,42 inferior'da, caput mandibulae'nın tepe noktasına göre -0,325mm ±4,73mm posterior'da tanımlanmıştır. Mandibula üzerindeki noktalardan caput mandibulae en güvenilir bulunmuş ve tepe noktasından en kısa uzaklık: 17,3mm ±3,43 olarak ölçülmüştür. N.mandibularis'e göre konum: %40 lateral, 40%: n.buccalis'le çaprazlanma ve 20% oranda medial olarak belirlenmiş ve karşılaşılan çeşitlenmeler dokümente edilmiştir. Sonuç: A.maxillaris'in konumunun ve çevre yapılar ile ilişkilerinin tanımlanması invaziv girişimlerde yaralanma potansiyelinin minimalize edilmesi bakımından önemlidir.

ABSTRACT Surgical Anatomy of the Maxillary Artery in the Infratemporal Fossa Objective: In this study, the portion of the maxillary artery located in the infratemporal fossa was comprehensively examined in terms of its course, branching pattern, diameter and distance measurements, configuration, relationships with surrounding anatomical structures, axial distances relative to bony landmarks, position relative to the mandibular nerve, and variations within the artery itself. The data obtained aim to better define the location of the maxillary artery and its relationships with surrounding structures, thereby contributing to minimizing the risk of iatrogenic injury in clinical and surgical interventions. It is expected that the morphological and morphometric evaluations conducted throughout this process, along with the documented cases, will expand the body of anatomical knowledge and serve as a guide for future dissections. Materials & Methods: The portion of the maxillary artery located in the infratemporal fossa was examined in twenty sides of ten formalin-fixed cadavers using a transfacial lateral approach. Following the dissection of superficial structures, the mandibular ramus, and the zygomatic arch, gross dissections of the maxillary artery and surrounding structures were performed using micro-scissors. All diameter and distance measurements of the branches and external structures were measured using an electronic caliper. Due to planar differences, all distance measurements taken from surrounding bony structures are axial in nature. Results: The lateral course rate of the maxillary artery relative to the lateral pterygoid muscle was found to be 65%; the medial course rate was 35%. Correlations between variable courses and different parameters were demonstrated. The branching pattern of the maxillary artery was evaluated, presented alongside the ratios of common trunk structures, and the organization of the pterygoid branches was classified. The course patterns of the maxillary artery were examined in seven distinct forms; the origin point was defined as 27.4 mm ± 3.42 mm inferior to the horizontal axis passing through the tragion and -0.325 mm ± 4.73 mm posterior to the apex of the mandibular head. Among the points on the mandible, the mandibular head was found to be the most reliable, and the shortest distance from its apex was measured as 17.3 mm ± 3.43. Position relative to the mandibular nerve was found to be 40% lateral, 40% crossed laterly by the buccal nerve, and 20% medial, and the variations encountered were documented. Conclusion: Defining the position of the maxillary artery and its relationships with surrounding structures is important for minimizing the potential for injury during invasive procedures.
