Özet
Bu araştırma, yanık hastaları üzerinde uygulanan progresif gevşeme egzersizlerinin (PGE) uyku kalitesi, anksiyete düzeyi ve beden imgesi üzerindeki etkisini incelemek amacıyla gerçekleştirildi. Randomize kontrollü deneysel tasarıma sahip olan çalışma, 1 Mart 2025–28 Şubat 2026 tarihleri arasında bir eğitim ve araştırma hastanesinin yanık kliniğinde yürütüldü. Veri toplama araçları olarak Hasta Tanılama Formu, Richard-Campbell Uyku Ölçeği, Durumluk ve Sürekli Kaygı Ölçeği, Beden Algısı Ölçeği ve araştırmacı tarafından hazırlanan PGE uygulama basamaklarını içeren ses kaydı kullanıldı. Araştırmada deney ve kontrol gruplarına dahil edilen tüm hastaların uygulama öncesinde tanıtıcı özellikleri ile birlikte uyku düzeyi, durumluk ve sürekli kaygı düzeyleri ve beden algısı değerlendirildi. Kontrol grubundaki hastalar rutin tedavi ve bakım uygulamalarını almaya devam ederken, deney grubundaki hastalara PGE uygulaması ayrıntılı şekilde anlatıldı ve ilk uygulama araştırmacı eşliğinde gerçekleştirildi. Deney grubundaki hastaların egzersizi günde iki kez (sabah ve akşam) olmak üzere toplam yedi gün boyunca uygulamaları sağlandı ve bu süreç hatırlatma mesajları ile desteklendi. Bir hafta sonunda aynı veri toplama araçları kullanılarak son test ölçümleri yapıldı. Verilerin analizinde R (V4.5.0) ve SPSS (V23) programları kullanıldı, normallik Kolmogorov-Smirnov testi ile değerlendirildi. İstatistiksel analizlerde tanımlayıcı istatistiklerin yanı sıra t testi, Mann-Whitney U testi, Wilcoxon testi, Spearman korelasyon analizi ve ANCOVA kullanıldı; ayrıca uygun durumlarda Yates, Monte Carlo ve Fisher's Exact testlerinden yararlanıldı. Anlamlılık düzeyi p<0,05 olarak kabul edildi. Deney grubunda durumluk kaygı puan ortalaması uygulama öncesi 62,62±6,33 iken uygulama sonrası 30,34±6,86'ya azaldı ve bu azalma istatistiksel olarak anlamlıdır (p<0,001). Kontrol grubunda ise anlamlı bir değişiklik saptanmadı (p=0,501). Sürekli kaygı puan ortalaması deney grubunda 53,48±5,93'ten 36,86±5,81'e azalırken (p<0,001), kontrol grubunda daha sınırlı bir azalma olduğu belirlendi (p=0,027). Gruplar arası karşılaştırmada, deney grubundaki azalma anlamlı düzeyde daha yüksek olduğu belirlendi (p<0,001). Deney grubunda Richard-Campbell Uyku Ölçeği puan ortalaması 39,96±11,14'ten 81,18±11,55'e arttığı ve bu artışın anlamlı olduğu belirlendi (p<0,001). Kontrol grubunda ise anlamlı bir değişim görülmedi (p=0,940). Uygulama sonrası deney grubunun uyku puanlarının kontrol grubuna göre anlamlı düzeyde daha yüksek olduğu saptandı (p<0,001). Beden algısı puanı deney grubunda 108,56±16,94'ten 155,26±20,98'e arttığı ve bu artışın istatistiksel olarak anlamlı olduğu belirlendi (p<0,001). Kontrol grubunda ise anlamlı bir değişiklik saptanmadı (p=0,604). Gruplar arası karşılaştırmada deney grubunun son test puanlarının daha yüksek olduğu belirlendi (p<0,001). Ayrıca, deney grubunda beden algısı ile durumluk ve sürekli kaygı arasında orta düzeyde negatif, uyku kalitesi ile ise orta düzeyde pozitif yönde anlamlı ilişki bulundu. Sonuç olarak, PGE'nin yanık hastalarında uyku kalitesini artırdığı, durumluk ve sürekli kaygı düzeylerini azalttığı ve beden imgesini olumlu yönde geliştirdiği belirlendi. Bu doğrultuda, yanık kliniklerinde PGE'ne hemşirelik girişimlerinde yer verilmesi, hastalara öğretilmesi ve düzenli uygulamanın izlem ve hatırlatma sistemleri ile desteklenmesi önerilmektedir. Ayrıca, farklı örneklemlerle ve uzun dönemli izlem içeren çalışmaların planlanması önerilmektedir.

ABSTRACT M.A. Thesis EFFECTS OF PROGRESSIVE RELAXATION EXERCISES ON SLEEP, ANXIETY AND BODY IMAGE IN BURN PATIENTS This study was conducted to examine the effects of progressive relaxation exercises (PRE) on sleep quality, anxiety levels, and body image in patients with burns. The study, designed as a randomized controlled experimental trial, was carried out in the burn unit of a training and research hospital between March 1, 2025, and February 28, 2026. Data collection instruments included the Patient Identification Form, the Richard-Campbell Sleep Questionnaire, the State-Trait Anxiety Inventory, the Body Image Scale, and an audio recording prepared by the researcher containing the steps of the PRE application. In the study, the sociodemographic characteristics, sleep levels, state and trait anxiety levels, and body image perceptions of all patients included in the experimental and control groups were assessed before the intervention. While patients in the control group continued to receive routine treatment and care, the PRE intervention was explained in detail to the patients in the experimental group, and the first session was conducted under the supervision of the researcher. Patients in the experimental group were instructed to perform the exercises twice daily (morning and evening) for a total of seven days, and this process was supported by reminder messages. After one week, post-test measurements were conducted using the same data collection tools. Data were analyzed using R (version 4.5.0) and SPSS (version 23). Normality was assessed using the Kolmogorov–Smirnov test. In addition to descriptive statistics, independent samples t-test, Mann–Whitney U test, Wilcoxon signed-rank test, Spearman correlation analysis, and ANCOVA were used. Yates correction, Monte Carlo simulation, and Fisher's Exact test were applied where appropriate. Statistical significance was accepted at p<0.05. In the experimental group, the mean state anxiety score decreased from 62.62±6.33 before the intervention to 30.34±6.86 after the intervention, and this decrease was statistically significant (p<0.001). No significant change was observed in the control group (p=0.501). The mean trait anxiety score in the experimental group decreased from 53.48±5.93 to 36.86±5.81 (p<0.001), whereas a more limited decrease was observed in the control group (p=0.027). In the between-group comparison, the reduction in the experimental group was significantly greater (p<0.001). In the experimental group, the mean Richard-Campbell Sleep Questionnaire score increased from 39.96±11.14 to 81.18±11.55, and this increase was statistically significant (p<0.001). No significant change was observed in the control group (p=0.940). Post-intervention sleep scores in the experimental group were significantly higher than those in the control group (p<0.001). The body image score in the experimental group increased from 108.56±16.94 to 155.26±20.98, and this increase was statistically significant (p<0.001). No significant change was observed in the control group (p=0.604). In the between-group comparison, post-test scores of the experimental group were significantly higher (p<0.001). Additionally, in the experimental group, body image was found to be moderately negatively correlated with state and trait anxiety, and moderately positively correlated with sleep quality. In conclusion, PRE was found to improve sleep quality, reduce state and trait anxiety levels, and positively enhance body image in patients with burns. Accordingly, it is recommended that PRE be incorporated into nursing interventions in burn units, that patients be educated on its use, and that adherence to regular practice be supported through monitoring and reminder systems. Furthermore, future studies involving different samples and long-term follow-up are recommended.
