Özet
- 97 - ÖZET 1988-1989 yıllarında Gülhane Askeri Tıp Akademisi Fiziksel Tıp ve Rehabilitasyon Kliniğinde yatan 20 hemiplejik ya da hemiparazik hasta incelenmiştir. On hastada, spastik kaslara egzersiz öncesi sabahları 30 dk. buz uygulaması yapıldı. Diğer on hastaya da antagonist kaslara tetanik faradik stimülasyon uygulandı. Her iki ajan da hastalara 15 gün uygulanmıştır. Hastaların durumları tedavinin 1. gününde, 8. gününde ve 15. gününde, tedaviden önce ve sonra el bileğinin ekstansiyon yönündeki aktif hareket miktarı goniometre ile ölçülerek ve pasif germeye karşı gelişen cevabı da Modifiye Ashworth Skalası ile değerlendirildi. Her iki ajan için tedavi öncesi-sonrası arasındaki farklılık, 1. günde, 8. günde ve 15. günde istatistiksel olarak önemli bulunmasına rağmen; elektrik stimülasyonuyla tedavide fark istatistiksel olarak daha önemlidir (P K 0.001). Bu sonuca göre, spastik hastalarda antagonist kaslara tetanik faradik stimülasyon buz uygulamasına göre, egzersiz tedavisi öncesinde daha etkin bir gevşetici ajandır.

- 98 - SUMMARY Comparison of exercise applications after the electrical stimulation and cold application to the wrist and finger spasticity treatment of Hemiplegic patients. Between 1988-1989, 2 0 spastic hemiplegic or hemiparesis hospitilized in Gülhane Military Academy of Medicine were examined. Ice application was carried out for 30 minutes before morning exercises on spastic muscles of ten patients. Tetanic. faradic stimulation was applied on antagonist muscles of another ten patients. Both agents were applied to the patients for 15 days. Conditions of the patients were evaluated before and after the 1., 8. and 1 5. day of the treatment by measuring the amount of movement of the wrist in the extension direction by goniometer and their reaction to passive tension was evaluated by Modified Ashworth Scale. Even though the differences between the pre-treatment and post-treatment for both of the agents during 1., 8. and 15. days were statistically significant; difference of electrical stimulated treatment is more significant (P< 0.001). According* to this result, for spastic patients applica tion of tetanic faradic stimulation to antagonist muscles prior to exercise treatment is a more effective relaxant agent compared to the application of ice.
