Özet
ÖZET Behçet hastalığı, mukokutan, oküler, artiküler, gastrointestinal, nörolojik ve vasküler bozukluklarla seyreden kronik, multisistemik hastalıktır. Behçet hastalığının spesifik bir tedavisi yoktur. Bir çok farklı tedavi modelleri hastalıkta kullanılmaktadır. Bu çalışmada, mukokutan Behçet hastalığı tedavisinde, 1.5 mg/gün oral kolşisinin etkinliği ve yan etkileri 3 mg/gün oral ketoifen ile randomize tek-kör yöntemle karşılaştırmıştır. Uluslararası Behçet Hastalığı Çalışma Grubunun kriterlerine göre tanı konulmuş 44 Behçet hastası değerlendirilmiştir. Hastalar iki gruba ayrılmış ve ayda bir kez görülerek izlenmiştir. Diğer mukokutan bulgular, ortalama oral ve genital ulserasyon sayı ve çapları tedaviden önce ve izlem sonrasında istatistiksel olarak karşılaştırılmıştır. Kolşisin tedavisinden sonra 22 Behçet hastasında ortalama oral aft sayı ve çaplarında, ortalama genital userasyon sayı ve çaplarında, patergi pozitifliğinde anlamlı azalma bulunmuştur. Ketotifen tedavisinden sonra, 22 Behçet hastasında ortalama oral aft sayı ve çaplarında, ortalama genital ulserasyon sayı ve çaplarında ve akneiform lezyonlarda anlamlı azalma bulunmuştur. Kolşisin ve ketotifen tedavisi uygulanan hasta grupları arasında, klinik bulgular karşılaştırıldığında, istatistiksel olarak anlamlı fark saptanmamıştır. Bu çalışmanın sonuçlan, ketotıfenin etkili ve güvenli bir tedavi şekli olabileceğini ve mukokutan Behçet hastalığı belirtilerinin tedavisinde kolşisin kadar etkili olduğunu göstermiştir.

SUMMARY Behçet' s disease (BD) is a chronic multisystemic disorder whose main manifestations include mucocutaneous, ocular, articular, gastrointestinal, neurologic and vascular abnormalities. There is no spesific treatment for BD. Many different treatment modalities have been used in the management of the disease. In this study, the efficacy and adverse effects of oral colchicine, 1.5 mg/day, in the treatment of mucocutaneous manifestations of BD were compared with those of oral ketotifen, 3 mg/day, in a randomized and single-masked study. Fourty four patients with Behcet's disease, who were diagnosed by criteria of the International Study Group for Behçet' s Disease were evaluated. Patients were divided into two groups and were examined at monthly intervals. Besides other mucocutaneous manifestations, the average number and size of oral and genital ulcers were compared statistically before the therapy and at the end of the follow-up period. In 22 patients with BD, after the colchicine therapy, the average number and size of oral and genital ulcers, and acneiform lesions diminished significantly. In 22 patients with BD, after the ketotifen therapy, the average number and size of oral and genital ulcers, and pathergy positivity diminished significantly. When the clinical manifestations compared among the patients groups with BD who were administered colchicine and ketotifen, no statistically significant difference was found. The results of the current study show that ketotifen may be an efficient and safe treatment modality and is as effective as colchicine in the treatment of mucocutaneous manifestations of BD.
