Özet

ÖZET Çalışmamızda 40 Romatoid Artrit (RA) li hasta ve 50 sağlıklı kişide plazma çinko, bakır ve magnezyum düzeyleri Atomik Absorbsiyon Spektrof otometresi ile saptanmıştır. Tam kandan lökosit ve eritrositler modifiye Boyum yöntemiyle pü- rifiye edilerek lökosit ve eritrosit içi aynı eser element tayinleri yapılmıştır. Ten ok sikam ilacı verilen 15 RA li hastada tedavi sonrası plazma, lökosit ve eritrosit içi eser element tayinleri yeniden yapılmış, hasta gruplarının verile ri kontrol grubu ile mukayese edilerek değerlendirilmiştir. Kontrol grubu plazma ortalama değerleri; Zn: x=128.800+ 2.833 /ug/100 mi, Cu: xc96. 740+1. 800 ytıg/100 mi, Mg : xsl2. 3B0+0. 223 mg/ı, lökosit İÇİ (ml/107 hücrede) Zn : x:0. 183+0.007 mg/1, Cu : \z0.012+0.001 mg/1, Mg : x=0. 264+0. 023 mg/1, eritrosit içi (ml/10 hücrede) Zn : x = 0. 115+0. 003 mg/1, Cu: x=0. 008+0. 001 mg/1, Mg : x=0. 145+0. 008 mg/1. İstatistiksel değerlendirmede, RA grubu ile tenoksikam verilen RA li grupta kontrol grubuna göre plazma, lökosit ve eritrosit içi çinko önemli derecede (p *L 0.001) düşük, plazma,11 lökosit ve eritrosit içi bakır ile plazma ve lökosit içi magnezyum önemli derecede (p -£0.001) yüksek, eritrosit içi magnezyum RA li hastalarda farksız (p > 0.05), tenoksikam alan grupta ise önemli derecede yüksek (p < 0.01) olarak bu lunmuştur. Sonuçta RA li hastalardaki plazma, lökosit ve eritro sit içi eser element düzeylerindeki önemli değişikliklerin, klinik aktivite ile ilgili olabileceği kanısına varılmış, tenoksikam verilmesinin bu elementlerin düzeyleri üzerinde önemli ölçüde etkili olmadığı tesbit edilmiş, eser element değerlendirmelerinin sağlıklı ve güvenilir yapılabilmesi için plazma ile birlikte eritrosit ve lökosit içi tayinleri nin de gerekli olduğu kanaatine varılmıştır. Ill SUMMARY We have determined the levels of zinc, copper and magnesium in plasma of 40 patients with Rheumatoid Arthritis (RA) and 50 healthy men by Atomic Absorbtion Spectrophotometric metods. The determination of above trace elements in leucocytes and erythrocytes have been made after the isolation of them from whole blood, by using modified Boyum's metod. After the treatment with tenoxicam determinations of trace elements in plasma and also in leucocytes and erythrocytes have been repeated in 15 RA patients again and the results have been compared with the control group. The average plasma levels in controls are; Zn: x = 128. 800+2. 833 /jg/100 ml, Cu: x = 96. 740 + 1. 800 ybg/100 ml, Mg: x = 12. 380+0. 223 mg/1, leucocyte (ml/107 cells) Zn: x=0. 183+0. 007 mg/1, Cu: x=0. 012+0. 001 mg/1, Mg : X-0.264+ 0.023 mg/1, erytrocyte (ml/107 cells) Zn: x=0. 115+0. 003 mg/1, Cu: x=0. 008+0. 001 mg/1, Mg : x=0. 145+0. 008 mg/1. The average results the tenoxicam given RA patients are; Zn: x=103. 133+4. 264 ^g/100 ml, Cu: x = 131. 800+5. 124 g/ 100 ml, Mg: x*15. 840+0. 297 mg/1, leucocyte Zn: x=0. 090+0. 007 mg/1, Cu: x-0. 021+0. 003 mg/1, Mg: x=0. 543+0. 051 mg/1, erytrocyte Zn: x=0. 067+0. 009 mg/1, Cu: x=0. 014+0. 002 mg/1, Mg: x-0. 204 + 0. 022 mg/1.IV By the statistical evaluation; plasma, leucocyte and erytrocyte zinc concentrations were significantly lover at all patients (p ««£0.001), while plasma leucocyte and erytro cyte copper levels and plasma and leucocyte magnesium concentrations were significantly higher in both groups with antreated and tenoxicam given RA patients (p *£. 0.001), than controls. But, erytrocyte magnesium levels were found similar in both patients and controls thus in the treated patients with tenoxicam, the magnesium levels were significantly higher than controls (p -eCO.Ol). As a result, the variation of levels of trace elements in plasma, leucocytes and erytrocytes may be due to the clinical activity, and the effect of tenoxicam on the trace element levels were not important. In order to make a correct evaluation, erytrocyte and leucocyte trace element deter minations have to be performed simultaneously with plasma.
