Özet
Bu çalışma, demanslı hastaya bakım verenlerin bakım yükünü incelemek amacıyla tanımlayıcı ve kapsadığı zamana göre kesitsel olarak 1 Ocak 2008 - 31 Haziran 2008 tarihlerinde Ege Üniversitesi Tıp Fakültesi Hastanesi, Dokuz Eylül Üniversitesi Hastanesi, Buca Seyfi Demirsoy Devlet Hastanesi, Buca Tabdi Tıp Merkezi ve İzmir Alzheimer Derneğine kayıtlı demanslı hasta yakınları ve bakım vericileri ile yapılmıştır. Bu evrenden demanslı hastaya bakım veren 305 bakım verici ve 305 hasta araştırmanın örneklemini oluşturmaktadırAraştırma kapsamına alınan bakım verenlerin %31.1'i 46-55 yaş grubu, eğitim düzeyi %37.4'ü ilkokul/ilköğretim (ortaokul) mezunu, %41.3'ünün hastası annesi, hastaya bakım verme süresi %51.5'i 1-5 yıl, %67.5'inin hasta ile beraber yaşadığı bildirilmiştirÇalışmamız kapsamına alınan hastaların %63.6'sı kadın, %43.3'ü 71-80 yaş grubu, bakım verenin akrabalık durumunun %45.6'sının hastanın kızı, hastalık süreleri incelendiğinde %63'ünün 1-5 yıl olduğu, hastalık evrelerine göre %40'ının erken evrede olduğu bildirilmiştirBakım verenlerin yaş grupları ile sosyal Bakım Yükü Ölçeği (BYÖ) puan ortalamaları; cinsiyetleri ile gelişimsel, duygusal ve toplam BYÖ puan ortalamaları; hasta ile yakınlık dereceleri ile zaman-bağımlılık ve sosyal BYÖ puan ortalamaları; bakım süreleri ile gelişimsel ve toplam BYÖ puan ortalamaları arasında anlamlı fark olduğu saptanmıştır.Hastaların yaş grupları ile bakım vericinin zaman-bağımlılık, duygusal ve toplam BYÖ puan ortalamaları; cinsiyetleri ile bakım vericinin sosyal BYÖ puan ortalamaları; eğitim düzeyleri ile bakım vericinin sosyal BYÖ puan ortalamaları; hastalık tanıları ile bakım vericinin zaman bağımlılık, gelişimsel, fiziksel, sosyal, duygusal ve toplam BYÖ puan ortalamaları; hastalık evresi ile bakım vericinin zaman-bağımlılık, gelişimsel, fiziksel, sosyal, duygusal ve toplam BYÖ puan ortalamaları arasında anlamlı fark olduğu saptanmıştırKronik bir hastalık olan ve uzun süreli tedavi, bakım gerektiren demans hastalarını ve onlara bakım verenleri etkileyen faktörler göz önüne alınarak demansın kontrol altına alınmasına yönelik ?eğitim-danışmanlık programları? ve düzenli sağlık kontrolleri düzenlenmesi, bakım verenlere yönelik danışmanlık ve eğitim hizmetlerinin yaygınlaştırılması, eğitim programının hekim, hemşire, diyetisyen, fizyoterapist, psikiyatrist ve sosyal hizmet uzmanından oluşan rehabilitasyon ekibi tarafından verilmesi önerilmiştir

This study was carried out to examine the burden on caregivers providing care for patients with dementia. This is a descriptive and according to the time crossectional study. It was carried out between 1 January 2008 and 31 June 2008. The relatives of the patients with dementia and their caregivers who were registered to the Neurology Polyclinic of the Ege University Medical Faculty, Dokuz Eylül University Hospital the Neurology Polyclinic of the Buca Seyfi Demirsoy State Hospital, the Neurology Polyclinic of the Buca Tabdi Medical Center and Izmir Alzheimer Association were comprised the scope of the research. A total of 305 patients and 305 caregivers who comprisedOf the caregivers included in the research 31.1% were in 46-55 years age group, 37.4% were elementary/intermediate school graduates (education level), 41.3% the patient was the mother, the length of care giving 1-5 years 51.5, %67.5 % were living with the patientOf the patients included in the research 63.6% were females, 43.3% were in 71-80 years age group, the status of kinship was patient?s daughter in 45.6%, the length of disease was between 1-5 years in 63%, 40% were in early stage (according to the stages of the diseaseSignificant differences were detected between caregivers? age groups and the mean scores of social Burden of Care Scale (BCS); between gender and the mean scores of developmental, emotional and total BCS; between the kinship with the patient and the mean scores of time-dependency and social BCS; between length of care and the mean scores of developmental and total BCSSignificant differences were also established between patients? age groups and caregivers? the mean scores of time-dependency, emotional and total BCS; between their gender and caregivers? the mean scores of social BCS; between their education levels and caregivers? the mean scores of social BCS; between their diagnoses and caregivers? the mean scores of time-dependency, developmental, physical, social, emotional and total BCS; between the stage of the disease and caregivers? the mean scores of time-dependency, developmental, physical, social, emotional and total BCSWhen the chronic nature of the disease and patients with dementia who require long-term treatment and care and the factors affecting their care providers are considered, it can be suggested that ?training-counseling programs? and regular health controls directed toward controlling the dementia be arranged, this training program be provided by a rehabilitation team involving a physician, a nurse, a dietitian, a physiotherapist, a psychiatrist and a social worker
