Özet

AMAÇ: Komplikasyonsuz katarakt cerrahisi sonrası (Fakoemülsifikasyon + arka kamara göz içi lensi (AKGİL) implantasyonu) hastalarda makula kalınlığını noninvaziv olarak optik koherens tomografi (OCT) ve fundus otofloresans (FAF) yöntemiyle değerlendirmektir.MATERYAL-METOD: İ.Ü. İstanbul Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Katarakt Departmanında Ekim 2008 - Haziran 2009 tarihleri arasında juvenil ve senil katarakt tanısıyla komplikasyonsuz fakoemülsifikasyon + AKGİL implantasyonu uygulanan 29 hastanın 40 gözü çalışmaya alındı. Hastaların santral makula analizleri OCT ve FAF ile preop, postop 1.gün, 1.hafta, 1.ay, 2.ay, 3.ay ve 6.ayda kaydedildi.BULGULAR: 29 hastada; 2 hastanın 3 gözünde (% 7.5) OCT ile kistik makula ödemi (KMÖ) saptandı. KMÖ olmayanlarda vizyon preop: 0,43 ± 0.21, postop; 1.gün: 0.51 ± 0,26, 1.hafta: 0,72 ± 0,27, 3.hafta: 0,84 ± 0,21, 3.ay: 0,90 ± 0,15, 6.ay: 0,91 ± 0,14 düzeyindeydi. Postop; 1.gün ile 1.hafta, 1.hafta ile 3.hafta ve 3.hafta ile 3.ay vizyonları arasında istatistiksel olarak anlamlı artış saptanmıştır. KMÖ saptananlarda vizyon preop: 0.40 ± 0,14, postop; 1.gün: 0,21 ± 0,14, 1.hafta: 0,56 ± 0,23, 3.hafta: 0,56 ± 0,23, 2.ay: 0,70 ± 0,20, 3.ay: 0,70 ± 0,20, 6.ay: 0.83 ± 0,05 düzeyinde bulunmuştur. KMÖ saptanmayanlarda makula kalınlıkları preop: 174,46 ± 20,79 , postop; 1.gün: 179,49 ± 22,54 , 1.hafta: 177,43 ± 21,33 , 3.hafta: 178,65 ± 19,07 , 3.ay: 172,73 ± 13,58 , 6.ay: 168,76 ± 10,25 düzeyinde saptanmıştır. Postop; 1.gün ile 6.ay, 3.hafta ile 6. ay ve 5.ay ile 6.ay arasında anlamlı fark vardır. KMÖ saptananlarda makula kalınlıkları preop: 189,67 ? ± 23,02, postop; 1.gün: 432,62 ± 361,43 , 1.hafta: 206,00 ± 28,82 , 3.hafta: 343,67 ± 123,14 , 2.ay: 345,33 ± 196,95 , 3.ay: 219,33 ± 49,69 , 6.ay: 200,67 ± 36,89 ? bulunmuştur.KMÖ saptadığımız 3 gözün 2'sinde hiperotofloresans saptadık 1 gözde ise saptayamadık. KMÖ gelişmeyen grupta ilk 3 ay makula kalınlığındaki orta derecede artışın vizyonda azalmaya sebep olmadığını ve postop 6.ayda makula kalınlığının yaklaşık olarak preop döneme döndüğünü saptadıkSONUÇ: Komplikasyonsuz katarakt cerrahisi geçiren hastalarda makula değerlendirmesinde OCT ile FAF'ın korele olduğunu, noninvaziv, hızlı, rahat bir yöntem olduğunu, tanıda ve tedavinin takibinde kullanılabileceğini düşünüyoruz.

PURPOSE: Evaluation of the macular thickness non-invasively using optic coherens tomography (OCT) and fundus autofluorescence (FAF) patients following uncomplicated cataract surgery (phacoemulsification + posterior chamber intraocular lens implantation ((PCİOL))MATERIALS-METHODS: 40 eyes of 29 patients undergoing uncomplicated phacoemulsification with PCIOL implantation with the diagnosis of juvenile or senile cataract at the Dept. Ophthalmology İstanbul University Faculty of Medicine between October 2008- June 2009 were included in the study. Central macular thickness of the patients were analysed with OCT and FAF preoperatively and at day 1, week 1, month 1 and 2, months 3 and 6 postoperatively.RESULTS: Cystic macular edema (CME) was detected with OCT at 3 eyes ( 7.5%) of 2 patients. Mean visuel acuities in patients without CME were 0,43 ± 0.21 preoperatively, 0.51 ± 0,26 at day 1, 1.hafta: 0,72 ± 0,27 at week 1, 0,84 ± 0,21 at week 3, 0,90 ± 0,15 at month 3 and 0,91 ± 0,14 at month 6 postoperatively. Statistically significant increased were noted in the visual acuity. Between day 1 and week 1, week 1 and week 3 and week 3 and month 3 postoperatively. Visual acuity of the patients with CME; 0.40 ± 0,14 preoperatively, 0,21 ± 0,14 at day 1, 0,56 ± 0,23 at week 1, 0,56 ± 0,23 at week 3, 0,70 ± 0,20 at month 2, 0,70 ± 0,20 at month 3, 0.83 ± 0,05 at month 6 postoperatively. Mean macular thickness of the patients without CME were 174,46 ± 20,79 preoperatively, 179,49 ± 22,54 at day 1, 177,43 ± 21,33 at week 1, 178,65 ± 19,07 at week 3, 172,73 ± 13,58 at month 3 and 168,76 ± 10,25 at month 6 postoperatively. There are significant differences between day 1 and month 6, week 3 and month 6 and month 5 and month 6 postoperatively. In patients with CME, macular thickness measured with OCT were as follows: 189,67 ? ± 23,02 preoperatively, 432,62 ± 361,43 at day 1, 206,00 ± 28,82 at week 1 , 343,67 ± 123,14 at week 3, 345,33 ± 196,95 at month 2, 219,33 ± 49,69 at month 3 and 200,67 ± 36,89 month 6 postoperatively.We found hyperautofluorescence in 2 of 3 eyes with CME detected but didn't find this in 1 eye. This study found that the moderate increase in macular thickness in the first 3 months in eyes without CME didn?t cause any decrease in vision and macular thickness returned to preoperative values at 6 months postoperatively.CONCLUSION: We suggest that OCT and FAF are correlated in the evaluation of the macula in patients undergoing uneventful cataract surgery and they are both non-invasive, rapid and comfortable methods which can be used both in diagnosis and in follow-up of the treatment.
