Özet
Amaç: Bu çalışmada, farklı demans evrelerindeki Alzheimer hastaları ile sağlıklı kontrol grubunun saç ve tırnak örneklerinde eser element düzeyleri incelenerek Alzheimer Hastalığı?nın eser elementlerle ilişkisi belirlenmeye çalışılmıştır. Gereç ve yöntem: Yirmibir hafif, 20 orta, 21 ağır düzeyde olmak üzere toplam altmışiki Alzheimer hastası ile 60 sağlıklı kontrol vakası çalışmaya alındı. Saç ve tırnaklardaki Sodyum (Na), Magnezyum (Mg), Alüminyum (Al), Potasyum (K), Kalsiyum (Ca), Manganez (Mn), Demir (Fe), Kobalt (Co), Bakır (Cu), Çinko (Zn), Selenyum (Se), Kadmiyum (Cd), Civa (Hg) ve Kurşun (Pb) eser elementlerinin düzeyi ICP-MS (İndüktif Eşlemeli Plazma Kütle Spektrometrisi) cihazı ile çalışıldı. Çalışma grupları arasında yaş, cinsiyet, eğitim ve yaşanan çevre yönlerinden anlamlı bir fark yoktu. Alzheimer hastalarının saç ve tırnaklarında sağlıklı kontrol vakalarına göre eser element değişiklikleri ve varsa bu değişikliklerin hastalık derecesi ile ilişkisi incelendi. Ayrıca eser element düzeyleri ile yaş ve hastalık süresi arasında bir ilişki olup olmadığı değerlendirildi. Hasta ve kontrol gruplarının eser element değerleri bağımsız gruplar için t testi ile karşılaştırıldı. Farklı şiddetteki hastalık grupları ile benzer sayıdaki kontrol vakası grubunun eser element değerleri birbirleriyle tek yönlü ANOVA testi ile karşılaştırıldı. Bulgular: Hasta ve kontrol grubu arasında tırnakta ölçülen eser element düzeyleri karşılaştırıldığında toplam hasta grubunda tırnakta Mn (p<0.001), Cu (p<0.001), Cd (p<0.005), Se (p<0.05), Hg (p<0.05) seviyelerinin anlamlı düşük; Pb (p<0.05), Fe (p<0.001) seviyelerinin anlamlı yüksek olduğu saptandı. Hasta ve kontrol grupları arasında saçta ölçülen eser element düzeyleri karşılaştırıldığında ise Na seviyesinin (p<0.001) anlamlı yüksek, Pb (p<0.001) ve Al (p<0.05) seviyesinin anlamlı düşük olduğu bulundu. Hasta ve kontrol grubunda, yaş ile eser element düzeyleri arasında anlamlı bir ilişki saptanmazken; hasta grubunda Se (p=0.05), Cd ve Hg (p<0.05) element düzeylerinin hastalık süresi ile anlamlı ilişki gösterdiği bulundu. Saç ve tırnak örneklerinde hastalık evreleri yönünden anlamlı bir farklılık görülmedi. Tırnakta Mn ve Fe ile saçta Pb elementlerinde bazı hastalık evreleri ile kontrol grubu arasında anlamlı farklılık olduğu bulundu (Mn için kontrol-evre 1 hasta, p=0.001; Mn kontrol-evre 2 hasta, p<0.05; Mn kontrol-evre 3 hasta, p=0.005; Fe için kontrol-evre 1 hasta, p<0.005; Pb için kontrol-evre 3 hasta, p<0.05). Sonuç: Eser elementlerde Alzheimer Hastalığı?nda birtakım değişiklikler görülmektedir. Gözlenen bu değişiklikler hastalığın oluşmasında eser elementlerin de katkıda bulunduğunu desteklemektedir. Bu açıdan Mn, Fe, Cu, Na en önemli elementler gibi gözükmektedir. Farklı şiddetteki Alzheimer Hastalığı gruplarında bu elementler açısından farklılık görülmemesi, eser elementlerin muhtemelen hastalığı başlatmakta etkili olup progresyonunda primer faktör olmadığını, daha çok yardımcı ve kolaylaştırıcı rol oynadığını düşündürmektedir.

Objective: In this study, a relationship between Alzheimer?s Disease and trace elements was determined in control subjects and different stages of Alzheimer?s patients, measuring trace element levels in hair and nail. Materials and methods: A total of sixty-two Alzheimer's patients, of whom 21 were in mild, 20 were in moderate, and 21 were is severe stage, and 60 healthy controls were included. Sodium (Na), magnesium (Mg), aluminum (Al), potassium (K), calcium (Ca), manganese (Mn), iron (Fe), cobalt (Co), copper (Cu), zinc (Zn), selenium (Se), cadmium (Cd), mercury (Hg) and lead (Pb) levels of trace elements were measured in hair and nails using ICP-MS (Inductively Matched Plasma Mass Spectrometry). There was no significant difference between the Alzheimer?s patients and control subjects according to the rural-urban ecotone, age, sex, or education. Trace element changes in hair and nails of Alzheimer's patients compared with healthy control subjects and, if any, the relationship between these changes and the stage of disease were examined. In addition, if there is a relation between trace element levels and age, and disease duration, were analyzed. Trace element values of patient and control groups were compared using t test for independent groups. Trace element levels in similar number of control subject were compared with those in similar number of different stages of Alzheimer?s patients using one-way ANOVA test. Results: Significantly lower nail levels of Mn (p <0.001), Cu (p <0.001), Cd (p <0.005), Se (p <0.05), and Hg (p <0.05) and significantly higher levels of Pb (p <0.05), Fe (p <0.001) nail levels were found in a total of Alzheimer?s patients than control subjects. The levels of Na (p <0.001) measured in the hair were found to be significantly higher and those of Pb (p <0.001) and Al (p <0.05) were found to be significantly lower when compared between the patient and control groups. While there was no significant relationship between age and trace element levels in both the patient and control groups, it was found that Se (p = 0.05), Cd and Hg (p <0.05) levels were significantly correlated with disease duration in the disease group. No significant difference was found in hair and nail samples in terms of stages of the disease. Nail Mn and Fe, and hair Pb levels were found to be significantly different between some stages of the disease and the control group. (stages 1, 2 and 3 for Mn, ps=0.001, below of 0.05, 0.005, respectively; stage 1 for Fe, p <0.005; stage 3 for Pb, p <0.05). Conclusion: Certain changes of trace element levels in Alzheimer's disease have been observed. These changes observed suggest that the trace elements contributed to the patogenesis of the disease. According to our results, Mn, Fe, Cu, and Na seem to be the most important elements. It has been thought that trace elements are probably effective in initiating of disease, however they are not the primary factor for progression of the disease, rather they have a role of facilitating of disease since we did not observed any difference in the trace elements between different stage of the Alzheimer?s Disease.
