Özet
Bu araştırma ile sistemik bir hastalık olan romatoid artrit (RA) hastalığının işitme sistemini ve tinnitus semptomunu etkileme düzeyinin araştırılması; hastalığın süresi, hastaların yaşı ve cinsiyetleri ile işitme sağlığı ve tinnitus arasında bir bağlantı olup olmadığının tespit edilmesi hedeflenmiştir. Araştırmanın katılımcılarını 2015 yılında K.Maraş Sütçü İmam Üniversitesi Sağlık Uygulama ve Araştırma Hastanesi Romatoloji ve KBB Polikliniği'ne başvuran hastalar oluşturmuştur. Romatoloji polikliniğinde RA tanısı ile tedavi gören 50 hastadan bir vaka grubu oluşturulmuştur. Hastalara 18 - 60 yaş arası sınırlaması getirilmiş; hastalık süresine ise bir sınırlama getirilmemiştir. Ayrıca KBB Polikliniği'ne başvuran hiçbir sistemik hastalığı ve düzenli ilaç kullanımı olmayan; kulak muayenesi normal olan 18-60 yaş arası 50 hastadan da bir kontrol grubu oluşturulmuştur. Bu hastalara Odyoloji Biriminde tympanogram, stapes akustik refleksi testi, tuba östaki fonksiyon testleri, saf ses odyometrisi, konuşma odyometrisi, OAE testleri (TEOAE ve DPOAE) uygulanmıştır. Test sonuçları SPSS istatistik paket programı ile değerlendirilmiştir. Araştırma sonucunda, RA'lı hastaların kontrol grubuna göre, tinnitus şikayetinin değişmediği, işitme sağlığının anlamlı bir şekilde bozulmadığı görülmüştür. Ayrıca hastalık ile tinnitus ve işitme arasındaki ilişkinin hastalık süresi ve hastanın yaşı ile ilgili olduğu kanısına varılmıştır. Hastalığın süresinin artması ve kişinin yaşının ilerlemesi ile birlikte işitme kaybı ve tinnitus şikayetinde bir artış olduğu gözlenmiştir. 35yaş üstü, 5 yıldan fazla RA tanısı alan hastalarda işitme kaybı ve tinnitusun görülme oranı daha yüksek elde edilmiştir.

In this research; rheumatoid arthritis disease, is a systemic disease, investigations of the effect level of the auditory system and the symptoms of tinnitus; the purpose of this investigation was to determine whether the relationship between the duration of the disease, the age and sex of the patients with the tinnitus and hearing health. Universe of the research has established by patients applied to Rheumatology and ENT Clinic of Kahramanmaraş Sütçü İmam University Health Practice and Research Hospital in 2015. A case group was formed from 50 patients treated with RA diagnosis in rheumatology clinic. 18-60 age limitation was brought to the patients; on the other hand, a limitation on the duration of disease was not brought. In addition, a control group was performed by matching 50 patients applied to ENT department, without any systemic disease and without any use of drugs regularly, between 18-60 years of age with normal ear examination. Tympanogram , the stapes acoustic reflex testing, eustechian tube function test , pure tone audiometry, speech audiometry, OAE testing ( TEOAE and DPOAE) were administered to these patients in Audiology unit. The test results were analyzed with SPSS statistical software package . As a result of research, the patients with RA compared to the control group , did not change the complaints of tinnitus and hasnt been found that a significant deterioration of the hearing health. Also the relationship between the disease with tinnitus and hearing health is related to patient's age and duration of the disease. An increase in hearing loss and tinnitus complaints were observed with the duration of the disease and progression of age. Hearing loss and tinnitus incidence obtained higher in RA patients diagnosed before 5 years or more, over 35 years.
