Özet
Amaç: Akut inme sonrası üst ekstremite kas tonusu flask olan hastalarda kinezyo bantlama uygulamasının ağrı, fonksiyon, genel sağlık değerlendirmesi ve depresyon üzerine etkilerini araştırmaktır. Gereç ve yöntem: Çalışmaya iskemik inme sonrası üst ekstremitede kas tonusu flask olan 26 hasta alındı. Hastalar basit örnekleme ile randomize edilerek tedavi grubu ve sham grubu olarak iki gruba ayrıldı. Her iki gruba konvansiyonel egzersiz ve gerekliliğinde splintlemeye ek olarak 13 hastaya el üzerine kinezyo bantlama, 13 hastaya sham tipi bantlama yapıldı. Hastalarda nörofizyolojik iyileşme takibi için Brunnstrom değerlendirmesi, ağrı düzeyini değerlendirmek için Vizüel Analog Skala (VAS), motor fonksiyon takibi için Fugl Meyer Değerlendirme Ölçeği (FMDÖ), genel sağlık değerlendirmesi için Health Assessment quesstionnaire (HAQ) ve depresyon düzeyini değerlendirme için Beck Depresyon Ölçeği kullanıldı. Bulgular: Kinezyo grubunda tedavi sonrası 3. hafta-6. hafta Brunnstromm üst skoru tedavi öncesine göre anlamlı (p ˂ 0.05) artış göstermiştir. Her iki grupta tedavi sonrası 3. hafta-6. hafta VAS skoru tedavi öncesine göre anlamlı (p ˂ 0.05) düşüş göstermiş, tedavi sonrası 3. hafta-6. hafta Fuglmeyer Oturma Pozisyonu üst ekstremite skoru ve el skoru tedavi öncesine göre anlamlı (p ˂ 0.05) artış göstermiştir. Kinezyo grubunda tedavi sonrası 3. hafta-6. hafta Fuglmeyer El Bileği skoru tedavi öncesine göre anlamlı (p ˂ 0.05) artış göstermiş, tedavi sonrası 6.hafta HAQ skoru ve Beck Depresyon Ölçeği Sham grubundan anlamlı (p ˂ 0.05) olarak daha düşük saptanmıştır. Sonuç: Akut inme hastalarında üst ekstremitede kinezyo bantlama tedavisi sonrası fonksiyonel iyileşmede artış, ağrıda azalma, genel sağlık değerlendirmesi ve depresyonda anlamlı olarak iyileşme saptanmıştır. Akut inme hastalarında el üzerine kinezyo bantlama uygulamasını üst ekstremite rehabilitasyonunda önermekteyiz.

Objective: The aim of this study was to evaluate the effects of kinesio taping on pain, function, general health evaluation and depression in acute stroke patients with flask upper limb Methods: Twenty-six patients with upper extremity muscle tone flask were included in the study. The patients were randomized by simple random sampling and divided into two groups as treatment group and sham group. In addition to conventional exercise and splinting in both groups, 13 patients were taped with kinesio, other 13 patients were taped sham-type. Brunnstrom was used to evaluate neurophysiological recovery, visual analog scale (VAS) for assessing pain level, Fugl Meyer Rating Scale for motor function (FML), Health Assessment questionnaire (HAQ) for general health evaluation and Beck Depression Scale for evaluating depression level. Results: 3rd-6th week post-treatment Brunnstromm upper extremity score showed significant increase in kinesio group compared to pre-treatment (p ˂ 0.05). Both group showed significant decrease in 3rd-6th week VAS score(p ˂ 0.05) and significant(p ˂ 0.05) increase in post-treatment 3rd-6th week Fugl-Meyer sitting position upper extremity and hand scores compared to pre-treatment. In kinesio group, third-sixth week after treatment, Fugl-Meyer Wrist score showed significant (p ˂ 0.05) increase compared to pretreatment. In addition, post-treatment 6th week HAQ score and Beck Depression inventory were significantly (p ˂ 0.05) low compared to sham group. Conclusion: Kinesio taping of the upper extremity in acute stroke patients established improvement in functional recovery, decrease in pain as well as development in general health assessment and depression. We recommend the application of kinesio taping of the hand in acute stroke patients for upper extremity rehabilitation.
