Özet

Bu çalışmanın amacı inme geçirmiş hastaların bakım verenlerinde oluşabilecek ruhsal rahatsızlıklardan olan major depresif bozukluk (MDB) ve travma sonrası stres bozukluğu (TSSB) gelişimini ve bu bozukluklarla ilişkili olabilecek risk etmenlerini araştırmaktır. Çalışma 2013 yılı Nisan ve Haziran aylarında bir tıp fakültesi hastanesi fiziksel tedavi ve rehabilitasyon servisi ve rehabilitasyon merkezi ile özel bir rehabilitasyon merkezinde takip edilen, 86 inme hastasının bakım veren yakınları ile yürütülmüştür. Katılımcılar Sosyodemografik Form, Genel Sağlık Anketi (GSA) 12, DSM IV klinik değerlendirilmesinin TSSB ve MDB modülleri kullanılarak değerlendirilmişlerdir. The aim of this study is to investigate the development of mental ilnesses that can occur in caregivers of stroke patients like PTSD and MDD and risk factors associated with this disorders. The study was conducted with 86 caregivers of stroke patients whom were treated at a university hospital's phsycal therapy and rehabilitation ward and a rehabilitation center connected to it and a private rehabilitation center between April 2013 and June 2013. Sociodemographic form, general health questionnaire 12, traumatic experiences list, PTSD and MDD modules of SCID I, were used during the assesment of the participants. The data was analyzed using cluster analysis, T test, Mann Whitney U test and ki square test. In analysis of the data a statitistic package program was used. P< 0,05 significance was accepted in interpretation of results. In our study 37.2 % were diagnosed MDD and 20.9% were diagnosed PTSD according to the DSM IV criteria. As the result of the cluster analysis, caregivers were divided into two groups with MDD, PTSD diagnoses and significantly different GSA scores. When the data of stroke patients were compared with the cluster grops, patients' sex, age and the duration of the illness showed no significant relationship. When the data of the caregivers were compared with the cluster groups, caregivers' sex, employment, sharing the same house with the patient, the duration of caregiving, history of a psychiatric disorder, history of a psychiatric disorder in first degree relatives, financial burden of medical expenses showed a significant relationship.Our study has shown that 37.2 % of the caregivers have met DSM criteria for MDD and 20.9% for PTSD. The study aimed to define the risk factors that may be associated with aforementioned psychopathologies. The high psycopathology rates of the caregivers of stroke patients reveals the importance of intervention attempts for relatives of these patients.
