Özet
Giriş ve Amaç: Anestezinin genel olarak termoregülasyonu bozması, açık vücut kavitelerinden ısı kaybı olması, ameliyathanelerin soğuk ortamı ve hastalara intravenöz sıvı ve kan verilmesi nedenleriyle ameliyat olan hastaların çoğunda perioperatif hipotermi gelişir. Hipotermi sık görülen ancak, önlenebilecek bir sorundur. Aynı zamanda genel anestezinin postoperatif dönemde de solunum fonksiyonlarını etkilediği bilinmektedir. Çalışmamızda hastanemizde genel anestezi ile laringeal maske uygulanarak küçük ve orta cerrahi girişim yapılan hastalarda intraoperatif vücut ısısı yönetiminin postoperatif solunumsal komplikasyonlara ve postoperatif tepe ekspiratuar akış hızına etkisini araştırmayı amaçladık. Materyal Metod: Genel anestezi alan ASA (American Society of Anesthesiologists) I, II ve III, 18-60 yaş arası, operasyon süresi minimum 90 dakika olan 90 hastada gerçekleştirildi. Hipotermi gözlenen ve hipotermi gözlenmeyen hastalarda preoperatif, postoperatif 1. saat, 6. saat, 24. saatte ölçülen tepe ekspiratuar akış hızı ve vücut sıcaklıkları, intraoperatif ve postoperatif hemodinamik ölçümler, ventilasyon basınçları açısından karşılaştırıldı. Bulgular: Hastalarımızın 21(%23,3')'inde hipotermi gözlendi. Gruplar arası Grup I (İntraoperatif hipotermi gözlenmeyen hastalar) / Grup II (İntraoperatif hipotermi gözlenen hastalar) karşılaştırmalarda; cinsiyet, yaş, boy, vücut ağırlığı, VKI (Vücut Kitle İndeksi), ASA skorlaması ve ek hastalık yönünden istatistiksel bir fark olmadığı bulunmuştur (p>0,05). Aynı zamanda operasyon süresi, anestezi süresi, solunum hızı, kusma, ek komplikasyonlar, derlenmeden servise taburculuk süresi ve hastaneden taburculuk süresi yönünden istatistiksel bir fark olmadığı bulunurken (p>0,05), kritik solunumsal olay ve bulantı yönünden istatistiksel olarak anlamlı bir fark olduğu (p<0,05) bulunmuştur. Gruplar arasında beklenen, preop, postop 6. ve 24. saatlerdeki TEAH (Tepe Ekspiratuar Akış Hızı) değerleri yönünden istatistiksel olarak anlamlı bir fark olmadığı bulunurken (p>0,05), postop 1. saat değerleri yönünden istatistiksel olarak anlamlı bir fark olduğu bulunmuştur (p<0,05). Postop 1. saat TEAH /preop TEAH, postop 6. saat TEAH /preop TEAH ve postop 24. saat TEAH /preop TEAH oranları yönünden istatistiksel olarak anlamlı bir fark olduğu bulunmuştur (p<0,05). Fark bulunan üç oranda da Grup I hastaların değerlerinin daha yüksek olduğu bulunmuştur. Derlenme odası 15. ve 30. dakikalarda ölçülen SpO2 değerleri yönünden istatistiksel olarak anlamlı bir fark olduğu bulunmuştur (p<0,05). Fark bulunan iki ölçüm zamanında da Grup I hastaların SpO2 değerlerinin daha yüksek olduğu bulunmuştur. Tartışma ve Sonuç: Küçük cerrahi işlemlerde bile, hafif düzey hipoterminin, derlenme döneminde spirometrik ölçümlerden tepe ekspiratuar akış hızını (TEAH) etkileyerek ve derlenmeyi geciktirerek kritik solunumsal olay gelişme ihtimalini arttırabileceğini düşünüyoruz. Vücut ısısının tüm anestezi işlemlerinde monitörize edilmesi ve ihtiyaç halinde ısıtma yöntemlerinin kullanılması hipotermi insidansını ve gelişebilecek solunumsal olumsuz etkileri azaltabilir.

Introduction and Objective: Perioperative hypothermia develops in the majority of patients who undergo an operation, because anesthesia generally disrupts thermoregulation, loss of heat from open body cavities, cold environment of operating rooms and infusion of intravenous fluids and blood for patients. Hypothermia is a common problem, but that can be avoided. It is also known that general anesthesia affects respiratory functions in the postoperative period. In our study, we aimed to analyze the effect of intraoperative body temperature management on postoperative respiratory complications and postoperative peak expiratory flow rate in patients, which are undergoing small and medium surgical procedure by applying general anesthesia with laryngeal mask. Materials and Methods: Our reseach was done in 90 patients which are under general anesthesia ASA (American Society of Anesthesiologists) I, II and III, aged 18-60 years with a minimum of 90 minutes operation time. Patients with and without hypothermia were compared in terms of parameters; peak expiratory flow rate and body temperature of measured preoperatively, 1 hour, 6 hours, 24 hours postoperatively, intraoperative and postoperative hemodynamic measurements, ventilatory pressures . Results: 21 (23.3%) of the patients had observed hypothermia. In comparisions between groups : Group I (Patients without intraoperative hypothermia) / Group II (Patients with intraoperative hypothermia) there was no statistical difference in terms of sex, age, height, weight, BMI (Body Mass Index), ASA (American Society of Anesthesiologists) scoring and additional disease. (p>0,05). At the same time, it was found that there was no statistical difference (p> 0,05) in terms of operation time, duration of anesthesia, respiratory rate, vomiting, additional complications, duration of discharge from recovery room to clinical service and duration of discharge from hospital (p> 0,05), a statistically significant difference in terms of critical respiratory event and nausea (p <0.05). There was no statistically significant difference (p> 0,05) between the groups in terms of predicted, preop, postoperative 6th and 24th hour PEFR (peak expiratory flow rate) values and it was found that there was a statistically significant difference in postoperative 1 hour values (p <0.05). There was a statistically significant difference between postoperative 1st hour PEFR/ preop PEFR, postoperative 6th hour PEFR / preop PEFR and postoperative 24th hour PEFR / preop PEFR ratios (p <0,05).The values of Group I patients were found to be higher than in the three different groups.It was found a statistically significant difference in terms of SpO2 values measured at 15th and 30th minutes in recovery room (p <0.05). SpO2 values of Group I patients were also found higher at two different measurement times. Dıscussion and Conclusion: We think that mild hypothermia, even in small surgical procedures, increased possibility of development critical respiratory event, by affecting the peak expiratory flow rate (PEFR) from the spirometric measurements during the awaking period and by delaying the awaking. Monitorizing body temperature during all anesthesia procedures and using heating methods as needed can reduce the incidence of hypothermia and the respiratory adverse effects that may develop.
