Özet

Modifiye Zorunlu Kullanım Tedavisinin İnmeli Hastalarda Sağ ve Sol Hemisfer Hasarlarında Üst Ekstremite Fonksiyonlarina Etkisi: Tek –Kör Randomize Kontrollü Çalışma Amaç: Subakut ve kronik dönem inmeli hastalarda modifiye zorunlu kullanım tedavisinin sağ/sol hemisfer hasarlarında üst ekstremite fonksiyonlarına ve yaşam kalitesi üzerine olan etkisini saptamaktır. Hastalar ve Yöntem: Prospektif, randomize kontrollü ve tek kör olarak planlanan çalışmamıza, 20 sağ ve 20 sol hemiplejisi bulunan toplam 40 hasta dahil edildi. Hastalar üç gruba randomize edildi. Birinci gruba 10 sağ inme hastası ve ikinci gruba 10 sol inme hastası dahil edildi ve haftanın 5 günü, günde 1 saat profesyonel destek, 3 saat ev programı şeklinde; 4'er kişilik gruplarla toplam 10 seans modifiye zorunlu kullanım tedavisi (mZKT) uygulandı. Hastaların etkilenmemiş elleri uyanık oldukları sürenin %50'sinde bir eldiven yardımı ile kısıtlandı. Kontrol grubuna rehabilitasyon ünitesinde yatarak rehabilitasyon programı bekleyen 10 sağ ve 10 sol inme hastası dahil edildi. Hastalar tedavi öncesi, tedavi sonrası ve tedavi sonrası 3. ayda Kutu-Blok Test, şekillendirme egzersizlerinden seçilen 30 saniyedeki küp dizme, kart çevirme ve nesne yakalama sayıları ve görev odaklı egzersizlerden seçilen su dolu bir bardağı tutma, ağıza götürme ve geri bırakma süresi, Motor Aktivite Günlüğü (MAG), İnme Etki Ölçeği (İEÖ), Fugl-Meyer Motor Değerlendirme Skalası ile değerlendirildi. Bulgular: Fugl-Meyer Motor Değerlendirme Skalası sol hemipleji grubunda tedavi öncesi ve tedavi sonrası 3. ay kontrol değerlerinde istatistiksel açıdan anlamlı artmıştır (p=0,040). Kontrol grubu ile kıyaslandığında; mZKT-sol hemipleji grubunda Fugl-Meyer Motor Değerlendirme Skalasında kontrol grubuna kıyasla istatistiksel olarak anlamlı artış izlenmiştir (p=0,002). Effect of Modified Constraint Induced Movement Therapy on Upper Extremity Functions of Stroke Patients with Right/Left Hemisphere Damage: Single Blind Randomized Controlled Trial Objective: To analyze the effect of modified constraint induced movement therapy on upper extremity functions and quality of life of subacute and chronic phase stroke patients with right/left hemispheric damage. Patients and Methods: A total of 40 patients (20 patients with right hemiplegia and 20 patients with left hemiplegia) were enrolled in our prospective, randomized controlled and single blinded trial. Patients were randomized in to three groups. The first group consists of 10 patients with right hemiplegia and the second group consists of 10 patients with left hemiplegia. First and second group of patients had 10 sessions of modified constraint induced movement therapy (mCIMT) as groups of 4 patients for 5 days a week. Each patient had modified constraint induced movement therapy 1 hour a day with professional support and performed home program by themselves 3 hours a day. Patients' less affected hands were limited by a glove for the %50 of the time they were awake. 10 patients with right hemiplegia and 10 patients with left hemiplegia formed the control group and these patients were in queue for inpatient rehabilitation programme. Patients were examined before treatment, just after treatment and 3 months after treatment using Box-Block Test, cubes lined, card turned and object gripped in 30 seconds, time it takes to grip and carry a water filled glass to their mouth and putting it back, Motor Activity Log (MAL), Stroke Impact Scale, Fugl-Meyer Motor Assessment Scale. Findings: There was statistically significant improvement in Fugl-Meyer Motor Assessment Scale results at 3 months after treatment compared to pre-treatment results in left hemiplegia group.
