Özet
Amaç: HPV enfeksiyonları ve HPV aşıları konusunda hem toplumda hem de sağlık çalışanlarındaki bilgi düzeyinin yeterli düzeyde olmadığı belirtilmiştir. Bu çalışmada aile hekimliği asistan hekimlerinde anket doldurma yöntemiyle HPV enfeksiyonları ve HPV aşısı konusundaki bilgi düzeylerinin belirlenmesi ve eksikliklerin irdelenmesi amaçlanmıştır. Yöntem: Çalışmaya tıp fakültesi hastanesinde Aile Hekimliği Anabilim Dalında çalışan 208 Aile hekimliği uzmanlığı ve 102 Sözleşmeli Aile Hekimliği Uzmanlığı asistan hekimi olmak üzere toplam 310 katılımcı dahil edilmiştir. Her katılımcıdan HPV ve korunma yöntemleri ile ilgili 25 sorudan oluşan ve tarafımızca düzenlenmiş olan HPV farkındalık anketinin doldurulması istenmiştir. Bulgular: Ankete katılanların 167'si (%53.9) erkek, 143'ü (%46.1) kadındı. Ortanca yaş 29 [IQR:4] idi. Katılımcıların 207'si (%67.4) HPV aşısını önerdiklerini belirtmiştir. HPV aşısı önermeyenlerin en sık gerekçeleri HPV aşısı hakkında yeterli bilgi ve tecrübeye sahip olmama (%17.4), aşının sağlık bakanlığı rutin aşı programında olmaması (% 16.1), aşının pahalı olması (%15.8) olarak ileri sürülmüştür. SAHU asistan hekimleri HPV enfeksiyonunun kendiliğinden iyileşmeyeceği (p<0.001), cinsel birleşme dışında sürtünme ya da WC yoluyla bulaşmayacağı (p=0.047) yanıtlarını anlamlı olarak daha yüksek oranda vermişlerdir. SAHU asistan hekimlerinde HPV aşısı önerme oranı AHU asistan hekimlerine göre anlamlı yüksek bulunmuştur (p=0.006). HPV aşısını önermeyenlerde HPV enfeksiyonunun prezervatif ile korunulabileceğini düşünme oranı aşıyı önerenlere göre anlamlı yüksek (p=0.017); aşının önerildiği yaş grubunun 9-26 olduğunu düşünme (p=0.015) ve genital hijyenin HPV enfeksiyondan korunmada etkili olduğu düşüncesi (p=0.04) oranları ise anlamlı düşük bulunmuştur. HPV aşısını önermeyenlerde aşının bakanlık aşı programında olduğunu bilmeme oranı aşıyı önerenlere göre anlamlı yüksek olarak saptanmıştır (p=0.015). Sonuç: Çalışmamızda elde edilen bulgular asistan hekimlerin HPV enfeksiyonları, serviks kanseri ve HPV aşısı konularındaki bilgi düzeylerinin çok yüksek olmadığını HPV aşısını destekleme oranlarının düşük olduğunu, HPV aşısı konusundaki bilgi ve destek oranlarının eğitim aldıkları birime göre farklılıklara neden olduğunu göstermektedir. Bu durum özellikle asistan hekimlerin HPV konusunda standart olarak eğitilmeleri gerektiğini göstermektedir. Ayrıca çalışmamız verileri HPV aşısının rutin aşı programında olmaması ve ücretinin yüksek olmasının hekimlerin aşı konusundaki desteğini olumsuz etkilediğini göstermektedir. Bu nedenle aşının rutin aşı programına dahil edilmesi yararlı olacaktır.

Objective: It has been stated that the level of knowledge of human papilloma virus (HPV) infections and HPV vaccines is insufficient both in the community and in healthcare workers. In this study, it was aimed to determine the knowledge levels of HPV infections and HPV vaccines in family medicine residents and to examine the deficiencies. Methods: A total of 208 research assistants working in the Department of Family Medicine at Medical Faculty Hospital (AHU assistant physicians) and 102 research assistants working in the Department of Contractual Family Medicine at Medical Faculty Hospital SAHU residents working in the Department of Family Medicine at ....... medical faculty hospital were included in the study. Each participant was asked to fill in the HPV awareness questionnaire prepared by us which consists of 22 questions about HPV and prevention methods. Results: A total of 167 (53.9%) of the participants were male. The median age was 29 [IQR: 4] years. A total of 207 of the participants (67.4%) stated that they recommended HPV vaccine. The most common reasons for those who do not recommend HPV vaccine were stated as not having sufficient knowledge and experience about the HPV vaccine (17.4%), the vaccine not being in the routine vaccination schedule of the Ministry of Health (16.1%), and the vaccine being expensive (15.8%). SAHU resident physicians gave a significantly higher rate of responses that HPV infection would not heal spontaneously (p <0.001), it would not be transmitted through friction or WC other than sexual intercourse (p=0.047). The rate of HPV vaccine recommendation among SAHU residents was found to be significantly higher than AHU residents (p=0.006). Those who do not recommend HPV vaccine have a significantly higher rate of thinking that HPV infection can be protected with a condom compared to those who recommend the vaccine (p=0.017); the rates of thinking that the age group for which the vaccine was recommended is 9- 26 (p=0.015) and the thought that genital hygiene is effective in preventing HPV infection (p=0.04) were found to be significantly lower. The rate of not knowing that the vaccine was in the ministry vaccination schedule was found to be significantly higher in those who did not recommend the HPV vaccine compared to those who recommended the vaccine (p=0.015). Conclusion: The findings obtained in our study show that the level of knowledge of resident physicians on HPV infections, cervical cancer and HPV vaccine is not very high, their support rate for HPV vaccine is low, and their knowledge and support rates on HPV vaccine cause differences according to the unit they are trained in. This situation shows that especially resident physicians should be trained about HPV as a standard. In addition, our study findings show that the HPV vaccine is not included in the routine vaccination schedule and its cost is high, negatively affecting physicians' support for vaccination. Therefore, it would be beneficial to include the vaccine in the routine vaccination schedule.
