Özet

Giriş ve amaç:İdrar yolu enfeksiyonu (İYE) çocukluk çağında solunum sistemi enfeksiyonlarından sonra 2. sıklıkta görülür. İki yaş altında semptomlar daha siliktir ve tanı koymak güçleşir. İdrar kültürünün sonuçlanması 48-72 saat süreceğinden tam idrar tetkiki analizi İYE'yi belirlemede daha hızlı ve kolay uygulanabilir bir testtir. Çalışmamızla hastanemiz çocuk acil kliniğine başvuran ve İYE düşünülerek idrar tetkiki alınan 1-24 ay arası hastalarda tam idrar tetkiki (TİT) parametrelerinin İYE için sensitivite, spesifisite, pozitif prediktif değer (PPV) ve negatif prediktif değerlerini (NPV) ortaya koymayı amaçladık. Bu değerler hastalarda dehidrasyon, yüksek ateş olmasına ve hastaların yaş gruplarına ve cinsiyete göre alt gruplara ayrılarak incelenmiştir. Gereç ve yöntemler:Çalışmaya2013 Ocak– 2019 Haziran arasında SBÜ Dr. Sami Ulus Kadın Doğum, Çocuk Sağlığı Hastalıkları Eğitim ve Araştırma Hastanesi, Çocuk Acil Kliniğine başvuran 1 ay-24 ay arası hastalardanİYE düşünülerek mesane kateterizasyonu ile temin edilen idrardan tam idrar tetkiki çalışılıp, aynı idrardan 2 saat içerisinde idrar kültür ekimi yapılmış olan 4990 hasta alındı. İYE; herhangi bir TİT anormalliği(lökosit esteraz, nitrit, mikroskobik piyüri veya bakteriüriden herhangi birinin pozitifliği) olmasının yanında idrar kültüründe ≥50.000 CFU/mL üreme olması olarak tanımlandı. Hastaların dosyaları taranarak veriler SPSS 22.0İstatistik Paket Programı ile değerlendirildi. Verilerin sıklık analizleri yapılarak ki kare testi ile değerlendirildi ve p<0,05 değerleri anlamlı kabul edildi. Bulgular: Hastaların % 45,6'sı erkek, %54,4'ü kızdır. İYE tanısı kız hastaların % 9,1'ine erkek hastaların % 11,9'una konulmuştur. İYE tanısı konulma oranı erkeklerde kızlara göre yüksek bulundu (p=0,001). Çalışmaya alınan hastaların büyük çoğunluğu (% 80,5) 1 ay-1 yaş arasında ve ortalama yaş 7,1 ay (1-24 ay) idi. İdrar kültüründe ≥50.000 CFU/mLüreme, hastaların % 14,9'unda görüldü ve üreme olan hastaların çoğu (% 87,5) 12 aylık ve daha küçüktü. İdrar kültürlerinde en sık üreyen 4 mikroorganizma sırası ile E. coli, Klebsiella, Enterokok ve Proteus idi. İlk iki sıra her iki cinste de aynı iken Proteus erkeklerde, Enterokok kızlarda 3. sırada idi. 12 aylıktan büyük hastalarda Proteus % 12,9 oran ile E. coli'den sonra 2. sıklıkta üredi. Hastaların % 39,4'ünde başvuruda ateş şikayeti mevcutken % 6,1'inde dehidrasyon saptandı. İYE tanımımız gereği tüm gruplarda kombine idrar tetkikinin sensitivitesi ve NPV'si % 100'dür. Parametreler ayrı ayrı incelendiğinde nitrit en spesifik ve PPV'si en yüksek tetkiktir. Tüm alt gruplarda mikroskobik piyüri en sensitif ve NPV'si en yüksek tetkiktir. LE veya nitritin sensitivitesi tek başına LE'ye göre artarken, spesifisitesi, PPV ve NPV'si tek başına LE'ye yakındır ve bu nedenle dipstik testi İYE'yi belirlemede yüksek oranda yol göstericidir. Sonuç:Çocuklarda sık görülen, mortalite ve morbiditesi yüksek olanidrar yoluenfeksiyonlarının erken tanı ve tedavisi çok önemlidir. Nitrit ve mikroskobik bakteriüri varlığı İYE'yi desteklemede daha yararlı iken, mikroskobik piyüri yokluğu İYE'yi dışlamada daha yararlıdır. TİT parametreleri doğru yorumlandığında gerçek İYE'yi öngörmeyi ve erken tedavi başlamayı sağlar. Böylece hastaların ilerleyen dönemde böbrek skarlaşması, hipertansiyon ve son dönem böbrek yetmezliği riskleri azalacaktır.

Introduction and aim: Urinary tract infection was the most common second disease after respiration system infections in the childhood. Its symptoms are weak and difficult to diagnose in the children under the ages of two. Complete urine analysis is a more rapid and easily applicable test in detecting UTI since the result of urine culture is obtained within 42-72 hours. The aim of this study is to reveal sensitivity, specificity, positive predictive values (PPV) and negative predictive values (NPV) of complete urine analysis parameters for UTI in the patients between 1 month to 2 years of age, who came to our hospital's children emergency department and had urine analysis to detect whether they had UTI. These values were examined by categorizing them according to the presence of dehydration, high fever in the patients and the age groups and gender of the patients. Materials and Methods: The study population was composed of 4990 patients between 1 month to 2 years of age, who came to Paediatric Emergency Department at SBÜ Dr. Sami Ulus Maternity, Paediatrics Training and Research Hospital between January 2013 and June 2019, and whose complete urine analysis were obtained by bladder catheterization considering the possibility of UTI and urine culture was cultivated from the same urine within 2 hours. UTI was defined as the presence of urine culture growth ≥ 50.000 CFU/mL besides the presence of complete urine analysis abnormality (any positivity of leukocyte esterase, nitrite, microscopic pyuria and bacteriuria). The data were recorded by scanning the patients' files and it was analyzed through SPSS statistical package (version 22.0). Chi squared test was used to analyze the frequencies and p<0.05 was considered as statistically significant. Results: 45.6% of the patients were male and 54.4% were female. UTI was diagnosed in 9.1% of female patients and 11,9% of male patients. The rate of UTI diagnosis was found to be higher in male patients compared to female patients (p=0.001). The majority of the patients (80.5%) included in the study were between 1 month and 1 year of age and the mean age was 7.1 months (1-24 months). Urine culture growth ≥50.000 CFU/mL was found in 14.9% of the patients and most of these patients (87.5%) were 12 months and younger. 4 microorganisms most frequently reproducing in urine culture were E. coli, Klebsiella, Enterococcus and Proteus respectively. The first two microorganisms in this order were similar for both genders while Proteus was in the third one for the male patients and Enterococcus in female patients. Proteus was the second in the frequency of reproduction after E. coli with the rate of 12.9% in the patients older than 12 months.39.4% of the patients complained of fever when they came to the hospital at first, while dehydration was detected in 6.1% of the patients. In accordance with the definition of UTI, the sensitivity and NPV of combined urine analysis in all groups was 100%. Nitrite test was the most specific with the highest PPV to detect UTI when the parameters were examined separately. Microscopic pyuria was the most sensitive test with the highest NPV in all subgroups. While the sensitivity of LE or nitrite increases separately, its specificity, PPV and NPV approximate to LE separately, therefore, dipstick test is the highly indicative to detect UTI. Conclusion: Early diagnosis and treatment of urinary tract infection are encountered frequently in children and has a high mortality and morbidity. While the presence of nitrite and microscopic bacteriuria is more effective in supporting the detection of UTI, the absence of microscopic pyuria is more effective in rejecting the presence of UTI. When complete urine analysis parameters are interpreted correctly, it enables to predict actual UTI and to start early treatment for it. Thus, the risks of renal scarring, hypertension and end stage renal failure for the patients will decrease.
