Özet
Amaç:İntrakranial anevrizmaların endovasküler tedavisi öncesi bakılan ADP duyarlılık değerleriyle perioperatiftromboembolik komplikasyonlar arasındaki ilişkiyi ortaya koymak ve bu komplikasyonun önüne geçmek amacıyla yeni bir eşik değeri bulmak amaçlandı. Gereç ve Yöntem:Mart 2017 ile Mart 2020 arasında İstanbul Fatih Sultan Mehmet EğitimAraştırma Hastanesi, Radyoloji Bölümü, GirişimselNöroradyolojiünitesindeintrakranial anevrizma nedeniyle elektif olarak endovasküler tedavi uygulanan ve preoperatif ADP test değeri bulunan 197 hasta retrospektif olarak tespit edilerek hastaya ve anevrizmaya dair parametreler değerlendirildi. Özellikle preoperatif ADP test değerleri ve perioperatiftromboembolik komplikasyon gelişip gelişmediği tespit edildi. Bulgular:Çalışmamızda perioperatiftromboembolik komplikasyon gelişen ve gelişmeyen olguların preoperatif ADP test değerleri arasında anlamlı farklılık saptanmadı. Çift stent+koil ve balon+stent+koil tedavileri komplikasyon gelişen grupta anlamlı olarak yüksek bulundu. Kadın cinsiyetin tromboembolik komplikasyon gelişimi riskinin daha fazla olduğu , akım çevirici stent tedavisinin kullanılmasının da preoperatif komplikasyon riskini azalttığı saptandı. Sonuç: ADP direnci saptanmayan olgularda ADP inhibisyon oranının perioperatiftromboembolik komplikasyon gelişimi riski ile ilişkili olmadığı görüldü. Bu nedenle çalışmamızda amaçladığımız tromboembolik komplikasyon riskinin daha aza indirebilecek bir referans değer bulunamadı. Perioperatiftromboembolikkomplikasyon gelişiminde kadın cinsiyetin perioperatif komplikasyon riskini arttırdığı ,tedavi olarak akım çevirici kullanılmasının ise azalttığı ve istatistiksel olarak anlamlı olduğu saptandı.

Introduction and aim: It was aimed to reveal the relationship between ADP sensitivity values measured before endovascular treatment of intracranial aneurysms and perioperative thromboembolic complications and tofind a new threshold value in order to prevent this complication. Materials and method: Between March 2017 and March 2020, 197 patients who underwent elective endovascular treatment for intracranial aneurysm in Istanbul Fatih Sultan Mehmet Training and Research Hospital, Department of Radiology, Interventional Neuroradiology unit and had preoperative ADP test values were identified retrospectively and the parameters of thepatient and theaneurysm were evaluated.Inparticular, preoperative ADP testvalues andperioperativethromboemboliccomplicationsweredetermined. Results:In our study, no significant difference was found between the preoperative ADP test values of the cases with and without perioperative thromboembolic complications. Doublestent+coil and balloon+stent+coil treatments were found to be significantly higher in the group with complications. It was determined that female gender had a higher risk of developing thromboembolic complications, and the use of flow-converting stent therapy also reduced the risk of perioperative complications. Conclusion:It was observed that the rate of ADP inhibition was not associated with the risk of developing perioperative thromboembolic complications in cases without ADP resistance. Therefore, we could not find a reference value that could reduce the risk of thromboembolic complications that we aimed for in our study. It was determined that female gender increased the risk of perioperative complications in the development of perioperative thromboembolic complications, whiletheuse of a current converter as a treatment decreased it and was statistically significant.
