Özet

Amaç: Solunum yolu enfeksiyonları hem çocuklarda hem de yetişkinlerde en yaygın morbidite ve mortalite nedenlerindendir. Çalışmamızda, solunum yolu patojenlerinin hızlı ve etkin tespitini sağlayan multipleks PCR yöntemiyle elde edilen sonuçlar etkenlere, mevsimlere, yaş gruplarına, klinik ve radyolojik özelliklere göre değerlendirilmiştir. Gereç ve Yöntem: 1 Ocak 2018- 31 Aralık 2019 tarihleri arasında solunum yolu semptomları ile başvuran hastalardan (n: 5151) nazofarengeal sürüntü örnekleri alınmıştır. Örnekler, 17'si virüs ve 3'ü bakteri olmak üzere 20 patojeni saptayabilen FilmArray Solunum Paneli (BioFire Diagnostics, Inc., Salt Lake City, UT, USA) ile test edilmiştir. Pozitif bulunan hastaların klinik bilgileri hastane veri tabanından elde edilmiştir. Eşzamanlı istenen toraks Bilgisayarlı Tomografileri (BT), laboratuvar bulgularından haberi olmayan iki radyolog tarafından birbirlerinden bağımsız olarak değerlendirilmiş, sonrasında fikir birliğiyle bir sonuca ulaştırılmıştır. Bulgular: Test edilen 5151 örneğin 1243'ünde (%24.1) en az bir patojen saptanmıştır. Pozitif örneklerin çoğunu 18 yaş altı hasta grubu oluşturmaktadır (738, %59.4). En sık Rhino/enterovirüsler (RV/EV) (569, %11.0) tespit edilmiştir. RV/EV'yi influenza virüsleri (A veya B) (253, %4.9) ve parainfluenza virüsler (PIV) (172, %3.3) takip etmiştir. En yaygın patojen kombinasyonu RV/EV ve PIV olmak üzere 202 örnekte (%3.9) birden fazla patojen tespit edilmiştir. Multipatojen içeren örnekler en sık 18 yaş altı hasta grubunda (150/2915, %5.1) saptanmıştır (p<0.05). Mevsimsel pikler, RV/EV enfeksiyonları için Eylül ve Mart, influenza A için Ocak ve PIV için Haziran aylarında ortaya çıkmıştır. Ateş, öksürük ve nefes darlığı saptanan en yaygın semptomlar olmakla birlikte öksürük, B. pertussis enfeksiyonlarında anlamlı olarak yüksek bulunmuştur (p<0.05). Patojen saptanan hastaların %34.0'ında BT görüntülemesi mevcut olup SYE tanısı hastaların %15.0'ınde radyolojik bulgularla da desteklenmektedir. Ancak saptanan bulgular patojene göre anlamlı farklılık göstermemektedir (p>0.05). 1243 hastanın %35.7'sine sadece antibiyotik, %3.8'ine sadece antiviral, %7.6'sına hem antibiyotik hem de antiviral reçete edilmiş, %52.9'una ise herhangi bir antimikrobiyal reçete edilmemiştir. Antibiyotik reçete edilen hastaların sadece %6.7'sinde etken bakteriyel bir patojendir. İnfluenza virüsle enfekte hastaların ise sadece %47.4'üne antiviral başlanmıştır. Sonuç: SYE'de klinik veya radyolojik bulgular tanıya yardımcı olsa da bulgular patojenlere özgü olmamaktadır. Multipleks PCR temelli yöntemler ile solunum yolu patojenlerin tanısına ilişkin laboratuvar sonuçları hızlı bir şekilde verilmesine rağmen bu sonuçların tedavi pratiğine yansıması beklenilen düzeyde olamamıştır. Laboratuvar sonuçlarının etkili kullanılması ve akılcı antimikrobiyal kullanımı konusunda klinisyenlerle daha yakın iş birliği gerekmektedir.

Objectives: Respiratory tract infections (RTI) are the most common cause of morbidity and mortality in both children and adults. In this retrospective study, we evaluated the results obtained by a multiplex-PCR assay, which enables rapid and efficient detection of a wide number of respiratory tract pathogens, according to seasons, age groups, clinical characteristics and radiological features. Materials/methods: Nasopharyngeal swab specimens were collected from patients presenting respiratory symptoms who attended Marmara University Pendik Training and Research Hospital between January 1, 2018 and December 31, 2019. The samples were tested with the FilmArray Respiratory Panel (BioFire Diagnostics, Inc., Salt Lake City, UT, USA), a multiplex PCR assay that targets 20 pathogens, including 17 viruses and 3 bacteria. Clinical data of patients with positive results were obtained from their medical records. Without knowing the laboratory results, two radiologists independently reviewed the Computed Tomography (CT) of the patients and reached a consensual diagnosis. Results: Among the 5151 specimens tested, 1243 (24.1%) were found positive for at least one pathogen. The majority of positive samples were from patients under the age of 18 (738, 59.4%). Rhino/enteroviruses (RV/EV) (569, 11.0%) were detected most often, followed by influenza viruses (A or B) (253, 4.9%) and parainfluenza viruses (PIV) (172, 3.3%). Multiple pathogens were detected in 202 samples (3.9%) with RV/EV and PIV being the most common combination. The multi-pathogen-positive samples were found most frequently in patients under the age of 18 (150, 5.1%) (p <0.05). The peak months were September and March for RV/EV infections, January for influenza A and June for PIV. Fever, dyspnea and cough were the most common symptoms, among which cough was found to be significantly higher in B. pertussis infections (p <0.05). CT scans were existent only for 34.0% of the patients with positive results, and the diagnosis of RTI was supported by radiological findings in 15.0% of the patients. However, there was no statistically significant difference between pathogens and radiological findings (p> 0.05). Of 1243 patients, 35.7% were prescribed only antibiotics, 3.8% only antivirals, 7.6% both antibiotics and antivirals, and 52.9% were not prescribed any antimicrobials. In only 6.7% of the patients who were prescribed antibiotics, the agent was a bacterial pathogen. Also, antiviral was initiated in just 47.4% of the patients infected with influenza viruses. Conclusion: Although clinical or radiological findings help in diagnosis of RTI, they are not specific for pathogens. Despite the fact that laboratory results for the diagnosis of RTI were reported rapidly by using multiplex PCR-based methods, the reflection of these results in treatment practice was not at the expected level. A closer cooperation with clinicians is required for efficient use of laboratory results and rational use of antimicrobials.
