Özet
Amaç: Bu prospektif randomize kontrollü deneysel çalışmanın amacı; kalça kırığı hastalarına yönelik geliştirilen deliryum önleyici bakım protokolünün ağrı, fonksiyonel durum, uyku kalitesi ve deliryumu önleme üzerine etkinliğinin değerlendirilmesidir. Gereç ve Yöntem: Bu araştırma, bir eğitim ve araştırma hastanesinin ortopedi ve travmatoloji kliniğinde yürütülmüştür. Araştırma, blok randomizasyon yöntemiyle müdahale ve kontrol grubuna ayrılan toplam 80 hasta ile gerçekleştirilmiştir. Araştırmanın verileri; Hasta Bilgi Formu, Mini-Cog Testi, YB-Konfüzyon Değerlendirme Yöntemi (CAM-ICU), Görsel Analog Skala (GAS), Barthel İndeksi (BI), Mini Nütrisyonel Değerlendirme Kısa Form (MNA-SF) ve Richards Campbell Uyku Ölçeği (RCSQ) kullanılarak toplanmıştır. Müdahale grubundaki hastalara, literatür desteği ile geliştirilmiş olan deliryum önleyici bakım protokolü doğrultusunda hemşirelik bakımı sağlanırken; kontrol grubundaki hastalara ise klinik prosedürü doğrultusunda rutin hemşirelik bakımı sağlanmıştır. Verilerin değerlendirilmesinde; sayı, yüzde, ortalama, standart sapma, ortanca, Interquartile Range ile Student's t testi, Mann Whitney U testi, Pearson ki-kare testi, Fisher düzeltmeli ki-kare testi, ANOVA testi, Paired Sample t test, Friedman testi, Wilcoxon testi ve Cronbach α katsayısı kullanılmıştır. Bulgular: Müdahale ve kontrol gruplarının preoperatif, postoperatif 1. gün ve taburculuk öncesi ağrı ve fonksiyonel durumları benzerdir (p>0,05). Aim: The aim of this prospective, randomized controlled experimantal trial is to evaluate the effectiveness of delirium preventive care protocol on pain, functional status, sleep quality and delirium prevention in patients with hip fracture. Materials and Methods: This research was conducted at a training and research hospital's orthopedics and traumatology department. The study sample was compromised of 80 patients with hip fracture. Intervention and control groups were applied after block randomization. Study data was collected by using Patient Information Form, Mini-Cog Test, Confusion Assessment Method, Visual Analogue Scale, Barthel Index, Mini Nutritional Assessment-Short Form and Richards Campbell Sleep Questionnaire. The patients in the intervention group were provided with nursing care in accordance with the delirium preventive care protocol developed with the support of literature. In the control group, routine nursing care was provided in line with the clinical procedure. The data was analyzed by using number, percentage, mean, standard deviation, median, Interquartile Range, Student's t test, Mann-Witney U test, Pearson's chi square test, Fisher corrected chi-square test, ANOVA test, Paired Sample t test, Friedman test, Wilcoxon test and Cronbach α coefficient.
