Özet
Son yıllarda tele-terapinin pek çok dil ve konuşma bozukluğu için uygun bir sunum biçimi olduğu ortaya çıkmıştır. Ses bozukluklarının değerlendirilmesi ve terapisine dair birçok özellik, bu hizmetlerde tele-terapi seçeneğini önemli hale getirmektedir. Çalışmanın amacı, vokal nodülü olan yetişkinlerde tele-terapi ve yüz yüze sunulan ses terapisi programının akustik, aerodinamik ve algısal ses parametreleri üstündeki etkisini karşılaştırmak ve tele-terapi grubu katılımcılarının online terapi deneyimlerini keşfetmektir. Çalışmada, karma araştırma yöntemi olan eş zamanlı desen kullanılmıştır. Nicel araştırma kapsamında, kontrol gruplu, randomize, öntest, sontest, izleme; tam-deneysel model; nitel araştırmada ise fenomenolojik yaklaşım tercih edilmiştir. Çalışmaya vokal nodül tanısı olan 14 yetişkin birey katılmış ve katılımcılar gruplara rastgele atanmıştır. İki gruba da vokal fonksiyon egzersizleri ve ses hijyenini içeren bir program sunulmuştur. Nicel araştırmada iki grup, üç farklı evrede, algısal, aerodinamik ve akustik parametreler bakımından karşılaştırılmıştır. Nitel araştırma kapsamında katılımcı deneyimleri ve görüşleri yarı-yapılandırılmış görüşmelerle incelenmiştir. Nicel bulgularda, iki grubun akustik, aerodinamik ve algısal ses parametrelerine ait ortalama değerleri incelendiğinde; tüm katılımcıların terapi sonu puanları, terapi öncesi puanlarına kıyasla ses kalitesinde iyileşme meydana geldiğini göstermiştir. Nitel bulgularda ise egzersizleri anlamada sorun ve teknolojik sorunlar gibi bazı olumsuz temalar ortaya çıkmıştır. Olumlu temalar ise verimlilik, konfor, keyif, esneklik, maliyetin düşmesi ve terapiste ulaşım kolaylığı şeklindedir. Nicel ve nitel bulgular birlikte incelendiğinde, tele-terapi ile gerçekleştirilen ses terapisi uygulamalarının, yüz yüze sunulan ses terapisi uygulamalarına güçlü bir alternatif olduğu görülmektedir. Ancak olumsuz temaları, hizmet iyileştirme kapsamında, ileriki çalışmalar ile detaylandıracak kanıt temelli uygulamalara ihtiyaç vardır.

In recent years, it has emerged that teletherapy is an appropriate form of presentation for many language and speech disorders. Many features of the evaluation and therapy of voice disorders make the teletherapy important in these services. The aim of the study is to compare the effects of tele-therapy and face-to-face voice therapy program on acoustic, aerodynamic and perceptual parameters in adults with vocal nodules and to explore the online therapy experiences of teletherapy group participants. In the study, simultaneous design, a mixed research method, was used. Within the scope of quantitative research, control group, randomized, pretest, posttest, follow-up; full-experimental model; In qualitative research, the phenomenological approach was preferred. 14 adult individuals with a vocal nodules participated in the study and the participants were randomly assigned to the groups. A program including vocal function exercises and vocal hygiene was presented to both groups. In the quantitative study, two groups were compared in terms of perceptual, aerodynamic and acoustic parameters in three different phases. Within the scope of qualitative research, participant experiences and opinions were examined through semi-structured interviews. In quantitative findings, when the average values of the acoustic, aerodynamic and perceptual parameters of the two groups were examined; all participants' post-test scores showed improvement in voice quality compared to their pre-test scores. In the qualitative findings, some negative themes such as problems in understanding the exercises and technological problems emerged. Positive themes are efficiency, comfort, enjoyment, flexibility, cost reduction and ease of access to the therapist. When the quantitative and qualitative findings are examined together, it is seen that voice therapy performed with teletherapy are a strong alternative to face-to-face voice therapy. However, there is a need for evidence-based practices that will detail the negative themes with further studies within the scope of service improvement.
