Background: Pregnancy and the postpartum period are both related to happiness and distress due to biological and psychological changes when the new mothers try to adjust new lives with a vulnerable infant which leads to various mental health problems, including the Obsessive-Compulsive Disorder (OCD). Accordingly, during the pregnancy and postpartum period, most of the women have harm-related intrusive thoughts regarding the infant, which may lead to obsessive-compulsive symptomology. Previous reviews have focused on the limited aspects of postpartum OCD. The current review aims to investigate the postpartum OCD comprehensively by focusing on the several facets of the disorder such as the nature of the intrusive thoughts, new mothers' unique experiences of having OC symptoms and the mother-infant interactions while mothers have harm-related intrusions. Methods: A comprehensive literature review was conducted on several search engines and databases such as PsychINFO, BMC Psychiatry, ScienceDirect, ProQuest and EBSCOhost. Based on a specific inclusion and exclusion criteria, studies published between 2015 and 2020 June were selected for comprehensive critical analysis. Results: Seven recent research studies with 601 participants in total were critically reviewed. Two of the studies were related to the nature of the harm-related intrusive thoughts during the postpartum period, one of them was related to triggers of intrusive thoughts, two of them were qualitative studies that related to experiences of postpartum women and two of them were related to mother-infant interaction under the influence of the harm-related intrusive thoughts. As a result, the vast majority of pregnant and postpartum women experienced infant-related harm intrusions. Infant crying was found to be one of the factors that trigger the intrusions. Also, mothers perceived their motherhood negatively because POSTPARTUM OCD AND INFANT-RELATED HARM INTRUSIONS harm-thoughts were not congruent with the ideal parenting, which dissonance caused clinically significant levels of distress and compulsive or avoidance behaviours. Moreover, consequently, mother-infant interactions were negatively affected as mothers limited their parenting behaviours to prevent the dreaded outcome from happening by avoiding the direct relationship with the infant. Limitations: A small of studies which have small sample sizes and participants from the limited demographics were included in the current literature review study. Recruitment methods were based on the self-selection, which causes women with severe symptomology to not to participate in the study. Furthermore, studies had not longitudinal designs which limit the understanding of the trajectory of postpartum OCD and women's experiences. Conclusions: Infant-related intrusive thoughts are highly prevalent during the pregnancy and postpartum period. Women experience high levels of anxiety and distress, which prevent them from enjoying their parenting and interaction with their infants. Thus, OCD symptoms during these vulnerable periods should be screened carefully during and after the pregnancy by the mental health practitioners and obstetricians. Also, pregnant and postpartum women should be provided with training related to infant-related intrusive thoughts to prevent extreme levels of stress and adverse child outcomes in the long run.
