Özet

Amaç: Pandemi sürecinde birinci basamak sağlık hizmetlerine başvuran Hipertansiyon hastalarının tedavilerine uyumlarını ve pandemi sürecindeki kontrollerinin durumunu tespit etmektir. Yöntem:Kesitsel tipte olan çalışma 1 Şubat-30 Nisan 2021 ayları arasında Hatay iline bağlı izinleri alınmış belirli ASM'lere herhangi bir nedenle başvuran 18 yaş üzeri Hipertansiyon hastalarına yapılmıştır.Çalışmada sosyodemografik ve Hipertansiyon hastalığı ile ilgili özelliklerin sorulduğu anket ve Hill-Bone Hipertansiyon Tedavisi Uyum Ölçeği kullanılmıştır. Bulgular:Çalışmamıza 361 hasta katıldı. Katılımcıların yaş ortanca değeri 67 (20-89)' idi. %50,1 katılımcının doktora kontrole gitme durumunu pandemi öncesi ve pandemi sırasında aynı düzeyde devam ettirdiği, %48,8 katılımcının ise kontrol sıklığını artırdığı bulundu. HBHTUÖtoplam puan ortanca değeri 5 (0-24)'tür.0 alıp tam uyumlu olan katılımcı sayısı %1,4'idi. Medikal alt gruba tam uyumlu olan %26,6 beslenme alt grubuna %14,1 ve görüşme alt grubuna göre tam uyumlu olan ise %19,7 katılımcı vardı. Pandemi döneminde sigara içme miktarınıazaltan katılımcıların tedaviye uyumları yüksek bulundu(arttıran-azaltan katılımcılar p=0,04,aynı düzeyde-azaltanlar p=0,01).Yaşam tarzı değişikliklerinden tuzsuz-yağsız diyet uygulayanların(p<0,001), hem diyet hem de egzersiz uyguladım diyenlerin (p=0,001) ve bu seçeneklerden en az birini uygulayanların (p=0,01) HBHTUÖ total puan skorları anlamlı şekilde düşük bulundu. Pandemiden önce düzenli şekilde Ht hastalığı için kontrole gidenlerin HBHTUÖ medikal,görüşme ve total puan skorlarının daha düşük olduğu görüldü(p=0,005,p<0,001,p<0,001). Sonuçlar:Katılımcıların pandemi döneminde HBHTUÖ'ne göre tedaviye uyum oranları literatürdeki çalışmalarla karşılaştırılınca daha düşük olarak bulunmuş oluphekimlerimizin ve hastaların tedaviye uyumlarını artırmak için daha çok çaba sarfedilmesi gerekmektedir.

Aim:To determine the compliance with the treatment of Hypertension patients who apply to primary health care services during the pandemic process and the status of their controls during the pandemic process. Methods:The cross-sectional study was conducted on Hypertension patients over the age of 18 who applied to certain primary care centers in Hatay province between February 1 and April 30, 2021 for any reason. In the study, a questionnaire asking about sociodemographic and hypertension disease characteristics and the Hill Bone Hypertension Treatment Compliance Scale were used Results:361 patients participated in the study. The median age of the participants was 67 (20-89).It was found that %50.1 of the participants maintained their visit to the doctor at the same level before and during the pandemic, while %48.8of the participants increased the frequency of control.The median value of the HBHTUÖ total score is 5 (0-24).The number of participants who received 0 and were fully compatible is %1.4. There are %26.6 participants who are fully compatible with the medical subgroup. There was %14.1 participants in the nutrition subgroup and %19.7 in the interview subgroup who are fully compliant.Participants who reduced the amount of smoking during the pandemic period had higher adherence to treatment (participants who increased-decreased p=0.04, those who increased-decreased p=0.01).Among the lifestyle changes, those who followed a salt free diet (p<0.001), those who said they had both diet and exercise (p=0.001), and those who applied at least one of these options (p=0,01) had significantly lower HBHTUÖ total scores.It was observed that the HBHTÖ medical, interview and total scores of those who regularly went to control for Ht disease before the pandemic were lower (p=0.005, p<0.001,p<0.001). Conclusion: Compared to the studies in the literature, the rates of adherence to treatment according to the HBHTUÖ of the participants during the pandemic period were found to be lower.More efforts are needed to increase the compliance of our physicians and patients to treatment.
