Background: Breastfeeding (BF) has been shown to be beneficial for both mother and infant. However, the role of BF on postpartum weight loss is unclear. Therefore, the purpose of this systematic review was to examine the effects of breastfeeding on maternal weight loss. Methods: An electronic search between 2001 and 2016 was performed to identify observational studies, randomized control trials, and cohort, non-randomized, cross sectional and case-control studies, written only in English language, from the databases MEDLINE (EbscoHost), PubMed, and Google Scholar. Healthy breastfeeding women aged 15 to 45 years were included. Studies conducted with women >45 years of age, with individuals with pre-existing clinical conditions, drug addiction, human immunodeficiency virus, and postpartum depression, and those receiving specific weight loss treatment were also excluded. The methodological quality of each study was assessed by using the Quality Criteria Checklist. Results: A total of nine studies were eligible for inclusion. Six of the nine studies found a significant association between maternal weight loss and breastfeeding. Two studies investigated body fat composition; one study found an inverse correlation between exclusive breastfeeding and maternal triceps skin-fold thickness (TST) and mid-arm circumference (MAC) measurements. In the other study, women who breastfed had significantly increased fat mass and percent body fat. Conclusion: The effect of breastfeeding on postpartum weight loss can be significant. However, more studies are needed to explore and determine the association between breastfeeding and maternal weight loss.
