Özet
Amaç: Tiroid nodülleri, tiroid bezinin en sık görülen hastalığıdır. Bu nodüllerin, bezdeki mikro inflamasyon sonucu geliştiği düşünülür. Bu inflamasyonun temel ögeleri salınan mediyatörler ve lökosit subgruplarıdır. Anormal immün yanıt sonucu gelişen birçok hastalıkta inflamatuar cevabı değerlendirmede Nötrofil/Lenfosit Oranı (NLO), Trombosit/Lenfosit Oranı (TLO) ve Lenfosit/Monosit Oranı (LMO) araştırılmıştır. Biz de çalışmamızda biyopsi yapılan nodüllerin, benign ve malign ayırımında hematolojik inflamatuar parametrelerin nasıl değiştiğini araştırmayı amaçladık. Materyal ve metod: Çalışmamızda Harran Üniversitesi Tıp Fakültesi Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Kliniği'nde Ocak 2016-Ocak 2021 tarihleri arasında biyopsi yapılmış nodül hastaları dahil edildi. Çalışmaya dahil edilenlerin kayıtları retrospektif olarak yaş, cinsiyet, üre, kreatin, alanin aminotransferaz (ALT), nötrofil, lenfosit, monosit, NLO, TLO, LMO, hemotokrit (HCT), hemoglobin (HBG), tiroid stimulan hormon (TSH), serbest T3, serbest T4 değerleri ve tiroid ince iğne aspirasyon biopsisi (TİİAB) sitopatoloji sonuçları kaydedildi. Bulgular: Çalışmaya dahil edilen 586 hastanın 545'i (%93.1) benign, 41'i (%6.9) malignite şüpheli idi. Benign sitolojisi olanların 453'ü (%83.1) kadın ve 92'si (%16.9) erkek olup, malignite şüpheli olanların 34'ü (%82.9) kadın ve 7'si (%17.1) erkek idi. Yaş ortalaması benign olanlarda 40,82 ± 11,07 yıl olup, malignite şüpheli olanlarda 41,46 ± 11,59 yıl olarak saptandı. Sitoloji sonucu malignite yönünden şüpheli olguların NLO ve TLO değerleri, benign gruptakilerden daha yüksek bulundu ve istatistiksel olarak anlamlı saptandı (p < 0,05 ) ancak LMO değeri benign grupta daha yüksek olup, istatistiksel olarak anlamlı fark saptanmadı (p > 0,05). X Sonuç: Tiroid nodüllerinde radyolojik tanı ile birlikte hematolojik parametrelerin değerlenirilmesi invaziv girişim öncesi yol gösterici olabilir. Bunu için daha geniş kapsamlı ileri çalışmalara ihtiyaç vardır.

Aim: Thyroid nodules are the most common disease of the thyroid gland. These nodules are thought to develop as a result of micro-inflammation in the gland. The main components of this inflammation are released mediators and leukocyte subgroups. Neutrophil/Lymphocyte Ratio (NLR), Platelet/Lymphocyte Ratio (TLR) and Lymphocyte/Monocyte Ratio (LMR) have been investigated in evaluating the inflammatory response in many diseases that develop as a result of abnormal immune response. In our study, we aimed to investigate how hematological inflammatory parameters change in the differentiation of benign and malignant nodules with thyroid fine-needle aspiration biopsy (TFNAB). Material and Method: In our study, nodule patients who underwent biopsy between January 2016 and January 2021 in the Endocrinology and Metabolic Diseases Clinic of Harran University Medical Faculty Hospital were included. Retrospective data of those included in the study consisting of age, gender, urea, creatine, alanine aminotransferase (ALT), neutrophil, lymphocyte, monocytes, NLR, TLR, LMR, hematocrit (HCT), Hemoglobin (HBG), thyroid stimulating hormone (TSH), free T3, free T4 values and FNAB cytopathology results were recorded. Results: Of the 586 patients included in the study, 545 (93.1%) were benign and 41 (6.9%) were suspected of having a malignancy. Of those with benign cytology, 453 (83.1%) were women and 92 (16.9%) were men, and 34 (82.9%) of those with suspected malignancy were women and 7 (17.1%) were men. The mean age of the group with benign cytology results was 40.82 ± 11.07 years and the mean age of the group with suspected malignancy was 41.46 ±11.59 years. The NLR and TLR values of the cases whose cytology results were suspected of having a malignancy were found to be higher than those in the benign group and were found to be statistically significant (sign <0,05 ); however, the LMR value was higher in the benign group and no statistically significant difference was found (sign >0,05). XII Conclusion: Evaluation of hematological parameters together with radiological diagnosis in thyroid nodules can help as a guide before invasive intervention. For this, further extensive studies are needed.
